THE CARPENTER HOSPICE Resident Care Coordinator – Job Description Title: Resident Care Coordinator Reports to: Executive Director Summary The Resident Care Coordinator coordinates and oversees the residential care area at the Hospice. He/she coordinates and participates in all aspects of resident care as needed. The Resident Care Coordinator assures that the highest degree of quality resident care can be maintained at all times. The Resident Care Coordinator is responsible for the overall management of the human, operational, and material resources required to meet the resident care needs in conjunction with the Mission, Vision and Values of The Carpenter Hospice. Primary Responsibilities: Administration: Ensures quality assurance, regulatory and legislative compliance e.g. WHMIS, Ministry of Labour, fire etc Conducts periodic chart reviews to ensure consistency with established policies and procedures Establishes standards for the collection, access, transfer and storage of client information Assists Executive Director in budget and operational planning for residential care area Oversee tracking and statistics, such as CCAC tracking, census etc Maintain complete and accurate documentation records in compliance with provincial regulations, legislation and health policies Monitors quality improvement e.g. chart audits, health and safety audits etc Assist the Executive Director with the development of policies and procedures pertaining to resident care area Maintains a warm and collaborative environment that respects the individual and welcomes change and innovation Stock taking and supplies management Attends regional/provincial meetings as necessary Human Resource Management: 1 Provide leadership and management to the resident care staff Participates in human resource planning for Carpenter Hospice Develop appropriate schedules to ensure that the resident care is managed according to work flow Coordinates and leads staff meetings, minimum of annually Provides ongoing feedback and conducts staff performance evaluations Provide direct supervision of resident care staff Involved in the departmental hiring and orientation of new residential staff and the ongoing monitoring of the competency Revised: December 2010 THE CARPENTER HOSPICE Provides feedback on performance of resident care volunteers to Volunteer Coordinator: assists and supports Volunteer Coordinator in addressing resident care volunteer issues Clinical Practice: Develops, implements, reviews, evaluates, and updates clinical processes, procedures and supporting documents Oversees resident's health and wellness needs, and resident assessments; assists in all aspects of care delivery as needed Provides opportunities for staff debriefing related to resident or provider loss Coordinates and facilitates the research, planning, implementation and evaluation of best practices Assists with the development of individual care plans Coordinates family meetings as needed Collaborates with CCAC and other members of the inter-disciplinary team to coordinate referral and supplies as needed Deals with resident and family complaints and escalates to the Executive Director when appropriate. Ensuring all documentation is completed Oversee and ensure collaboration during resident review process Alternates the on-call duties with other members of the leadership on-call team for any clinical/staffing issues that may arise Observe residents' health status and family needs, taking actions to address concerns, and reporting all significant changes Assist with resident admission and discharge processes as necessary Oversee medication management including staff orientation to medication management and the on-going monitoring of the staff’s competency Maintain positive communication with the families and/or responsible parties of residents on a regular basis to keep them aware and informed of the status of their family member Coordinate and follow up with physicians, home health agencies, pharmacy and other services as necessary Education: Coordinates provision of education to the residents, families and Hospice Staff Coordinates student placement, mentorship and evaluation 2 Revised: December 2010 THE CARPENTER HOSPICE Position Requirements RN registration from the College of Nurses of Ontario (CNO) Bachelors of Science in Nursing (BScN) preferred Palliative care certification or equivalent preferred Minimum 3 years in palliative care Knowledgeable regarding the nursing process and its application, including the assessment, planning, implementation, and evaluation of nursing care as it relates to hospice palliative care Ability to function in a highly charged emotional environment as it pertains to death and dying Demonstrated ability to assist patients, family members or other clients with concern and empathy; respecting their confidentiality and privacy and communicating in a courteous and respectful manner A well-defined sense of diplomacy, including solid negotiation, conflict resolution, and people management skills High degree of resourcefulness, flexibility, and adaptability Able to effectively communicate both verbally and in writing Be politically and culturally sensitive High level of critical and logical thinking, analysis, and/or reasoning to identify underlying principles, reasons, or facts Strong morals and ethics, along with a commitment to staff and resident privacy Must be able to cover any shift if need arises, do weekly and monthly reports, schedules etc Supervisory experience is preferred Basic knowledge of computer applications is required Working Conditions 3 May be exposed to infectious waste conditions, etc Interacts with residents, family members, staff, visitors and other agencies Travel may be required Intermittent physical activity including walking, standing, sitting, lifting and supporting patients Revised: December 2010
