This assignment comprises your assessment for the unit of competency BSBCMN411A
Monitor a safe workplace.
This is from the Business Services Training Package (BSB01) and can be used as credit towards a range of qualifications – for example the Certificate IV in Business (Frontline
Management) and the Certificate IV in Metalliferous Mining Operations.
A copy of the competency standard for this unit can be downloaded from the Training
Services Australia website (refer to page 4 for download instructions).
To receive a statement of attainment for BSBCMN411A you must meet the evidence requirements for the unit. This includes evidence that you:
Apply organisational management systems and procedures to Occupational Health and
Safety within work group area
Identify and comply with Occupational Health and Safety legal and organisational requirements
Apply procedures for identifying hazards in the work area
Apply procedures for assessing and controlling risks to health and safety associated with those hazards, in accordance with the hierarchy of control
Provide specific, clear and accurate information and advice on workplace hazards to the work group
Provide appropriate supervision of the work group
The assessment tasks in this assignment have been designed to assist you in providing the minimum evidence required to demonstrate competence in the above unit.
The assignment is divided into the six assessment tasks described below. Your completed assignment must be forwarded to Training Services Australia for assessment. In most cases, completion of all assessment tasks, in accordance with the instructions provided, should be sufficient for you to gain a statement of attainment for the BSBCMN411A. Your assessor will advise you if there are any additional evidence requirements that need to be met.

The assessment tasks were developed to assist you in providing evidence of your competence against each of the elements and performance criteria for the unit, and to satisfy the critical aspects of evidence. A mapping table showing the elements and performance
criteria that each assessment task addresses can be found on page 9. The instructions for
completing each assessment task begin on page 14.
It is important that you complete your assignment as soon as possible after the completion of the course. As a general rule, the longer you leave your assignment, the more you will forget of the knowledge and experience you gained on the course, and the harder it will become to complete.
There is a four month time limit on the completion of this assignment from the date of your participation in the OHS for Supervisors workshop.
If, for any reason, you are unable to complete the assignment within the specified timeframe, please contact Training Services Australia before the time limit expires. Our contact details are below.

Where possible, the assessment tasks in this assignment have been designed to provide you with flexibility and choice in how you go about your assessment. However, Training Services
Australia recognises that the assessment tasks described herein are not the only way of demonstrating competence in the unit. Should you wish to provide alternative evidence to demonstrate your competence, please discuss this with your TSA assessor.
Should you negotiate with your assessor to provide alternative evidence to what is described in this assignment, you must document the agreed evidence and have your assessor sign the agreement before you proceed. This is required to prevent potential misunderstandings.
Plagiarism is a form of cheating. It is taking and using someone else's thoughts, writings or inventions and representing them as your own.
The following list outlines some of the activities for which a participant can be accused of plagiarism:
Handing in assessments markedly similar to or copied from another student
Presenting the work of another individual or group as their own work
Handing up assessments without the adequate acknowledgement of sources used, including assessments taken totally or in part from the internet

Your assessor or a member of Training Services Australia ’s administration team will advise you when you have been assessed as competent in the units covered by this assignment.
You will then be issued with a statement of attainment for the unit.
Should you be assessed as not yet competent , your assessor will provide you with guidance about your options. These may include further training, and re-assessment.
Candidates assessed as not yet competent are provided with at least one opportunity to resubmit their assignment for assessment. In many cases, you will only be required to resubmit the part(s) of your assignment that your assessor considered unsatisfactory. Some candidates may be asked to resubmit their entire assignment. Your assessor will advise you of what is required.
Should you be dissatisfied with the assessment decision, you should initially take this up with your TSA assessor. If you remain dissatisfied, you are invited to discuss the matter with
TSA’s Principal. Should you remain dissatisfied and wish to pursue the matter further, the

Assessment Task 2 – Health and safety training
This assessment task is designed to assist you to demonstrate your competence in relation to the provision of health and safety training for your team (element 3 of unit
BSBCMN411A). Section 4 of your participant manual contains some useful ideas and information which may assist you with this task.
Please be sure to follow any organisational procedures relating to health and safety training when undertaking this task.
Part A – Training needs analysis
Carry out an analysis of the health and safety needs of your work team.
Please ensure you consider the following in your analysis as a minimum:
have all team members undergone a general health and safety induction?
have all team members had training in workplace emergency procedures?
does your team include enough people with knowledge of first aid?
have all team members been trained in the safe operation of work plant and equipment they are required to use?
You must conduct your training needs analysis and present the results in accordance with any applicable organisational requirements. A sample training needs analysis template has been attached for you to use if appropriate. You may wish to modify this or develop your own template to better suit your situation.
Part B – Plan for meeting OHS training needs
Prepare a basic training plan showing how the training needs you identified in Part A will be met. Please ensure your plan is consistent with organisational requirements.
A sample Training Action Plan Template is attached for you to use if appropriate. Please feel free to modify this to better suit your needs.
Other options for showing how you have planned to meet OHS training needs include:
preparing individual training plans for each of your staff
converting your training needs analysis documentation into a plan by adding information on the type of training to be provided, target completion dates, etc

This assessment task is designed to assist you to demonstrate your competence in providing information to your staff and helping to satisfy their training needs. The evidence you generate from undertaking this task should relate primarily to elements 1 and 3 of the unit BSBCMN411A.
Section 4 of your participant manual contains some useful ideas and information which may assist you with this task.
Part A – Induction session
Conduct a health and safety induction for one of your team members. This should preferably be for a relatively new team member who has not yet undergone an induction. Alternatively, you may feel that one or more of your staff would benefit from a refresher induction.
If no staff require an induction, or are likely to require an induction in the near future, you may provide a copy of a previous induction you have carried out.
Please include with your assignment, a copy of the induction checklist, signed by yourself and the staff member who was inducted. Please ensure you clearly indicate where, as part of the induction, you addressed:
1. Relevant provisions of health and safety legislation, codes of practice, etc
2. Information on the organisation’s policies, programs and procedures
3. Information on workplace hazards, risks and risk control measures
4. Evacuation procedures and various emergency procedures (e.g., fire)
5. First aid
6. Reporting procedures for workplace hazards and health and safety incidents

Part B – Coaching / training session
Conduct a short coaching / training session with one or more of your staff on a health and safety issue. Ideally, you should use the session to address a training need you identified as part of the previous assessment task.
Examples of topics that might be suitable include:
how to operate a piece of machinery / equipment
how to undertake a work process correctly
how to use PPE
how to report workplace incidents / accidents
emergency evacuation procedures
Please ensure you include in your session an overview and explanation of the topic, and where appropriate, a demonstration, and opportunities for the participant(s) to practice.
The session could be conducted as part of a team toolbox meeting if appropriate.

Assessment Task 5 – Health and safety record keeping
This assessment task is designed to assist you to demonstrate your competence in relation to implementing and monitoring your organisation’s procedures for maintaining health and safety records (element 6 of unit BSBCMN411A). Section 7 of your participant manual contains some useful ideas and information which may assist you with this task.
Part A – Reporting workplace injuries / disease
1. Briefly outline your responsibilities as a frontline manager for completing and maintaining records of occupational injury and disease in your workplace.
2. Provide a copy of an incident / injury / disease report which you have completed in accordance with organisational re quirements. If this is a report of a ‘real’ incident, for confidentiality purposes you should blank out the names of the parties involved.
If you have never completed an incident / injury / disease report before, you may prepare a ‘sample’ report for a potential workplace incident. Please ensure this is clearly identified as a ‘sample only’ so there is no risk that it could be mistakenly taken as being evidence of an actual incident.

Examples of the types of records you may wish to use include:

Assessment Task 6 – Third party report
For this assessment task you are required to obtain a third party report from your supervisor or a representative from your organisation’s safety department.
The purpose of this report is to help confirm that you understand and apply your organisation’s policies and procedures for OHS, in accordance with the requirements of
BSBCMN411A Monitor a safe workplace .
The suggested approach for completing this task is to:
1. Provide your third party with a copy of the attached instructions (page 26) and the third
party report documentation (starting on page 27).
2. Arrange a time to meet with them to discuss the third party report. We estimate that the meeting would typically take between ten and thirty minutes, depending on how familiar the third party is with your work in relation to OHS. We recommend that you bring a copy of your completed assignment to the meeting in case the third party would like to see some examples of the evidence you have produced.
3. Ask the third party to fill out the report (including signature and date at the bottom of the second page) and get them to return it to you for inclusion in your assignment.

In order to make a judgement about the candidate’s ability to ‘Monitor a safe workplace’ we are required to collect evidence from a range of contexts and over a period of time.
Some of the main sources of evidence that we use are samples of the candidate’s work products, and the candidate’s responses to questions about how they undertake certain tasks and activities in the workplace. While this generally gives us a good indication of the candidate’s overall knowledge and ability, it is sometimes difficult to judge how well the candidate understands and follows the organisation’s specific requirements for occupational health and safety.
Consequently, we request that you assist in making this judgement by completing this third party evidence report. We value your contribution and ask that you answer the questions honestly.
We recommend that you have a short discussion with the candidate about each of the points in the attached checklist as a way of satisfying yourself that the candidate meets the organisation’s requirements. You may also wish to take a look at some of the evidence the candidate has prepared for their assignment as a way of double checking that the candidate has the required knowledge and understanding.
If you believe that the candidate’s knowledge, understanding and performance is inadequate in some of the areas covered in this report, we request that you work with the candidate to help improve their performance in these areas to a satisfactory level. Alternatively, you may wish to speak with a consultant at Training Services Australia about additional training you believe would be required to help the candidate reach the required standard of performance.
