This is for qualified neighborhood events such as educational or informational workshops and seminars. The scheduling of event will become subject to the approval of the PPCA Board. Facility must be secured, locked and vacant by 10:00pm. Be sure to allow time for set-up and clean-up within your rental time. Security Deposit - $150 (One Hundred Fifty Dollars) is required to reserve a date. Full payment in the form of Cashier’s Check or Money Order made payable to Pacific Palisades Community Association. SPORTS HALF DAY RENTAL (MANDATORY TO HIRE HPD OFFICER(S) IF SERVING ALCOHOL – see page 2) Fridays, Saturdays & Sundays from 8:00am to 4:00pm. This is for qualified neighborhood Recreational and Physical sports events such as football, baseball, basketball, karate, etc. A valid member MUST sponsor and attend the event. Facility must be secured, locked and vacant by 5:00pm. Be sure to allow time for set-up and clean-up within your rental time. Security Deposit - $200 (Two Hundred Dollars) is required to reserve a date. Full payment in the form of Cashier’s Check or Money Order made payable to Pacific Palisades Community Association. Rental Fee - $100 (Two Hundred Dollars) is required at time of security deposit in the form of Cashier’s Check or Money Order made payable to Pacific Palisades Community Association. HPD SPECIAL DUTY OFFICER INFORMATION HONLULU POLICE DEPARTMENT SPECIAL DUTY SECTION 529-3610 It is the responsibility of the Renter, to hire HPD Special Duty Officer(s) and request a copy of the Officer(s) work order. A copy of the officer(s) work order needs to be submitted to the office manager within thirty days of your event. Officer(s) must remain on duty from the start of event until center renter has secured and locked facility at the end of event. The Renter must obey all state alcohol regulations and is responsible for fines should those laws be violated. No brown bagging is permitted by guest. The Renter assumes responsibility that no one under the of 21 can be served or consume alcohol beverages; any intoxicated guest must not be permitted to drive home when leaving their function; and the Renter assumes full responsibility for guest when alcohol is served. Special Duty rates are set by the Honolulu Police Department to ensure consistency and parity. Officer(s) to provide appropriate safety and security oversight throughout the entire event in accordance with the provisions of the HPD Special Duty Section. Your entire security deposit will be retained for violation if a HPD Special Duty Officer is not present during your rental. age RESERVATION POLICY ALL RENTALS are taken up to 12 months in advance. CANCELLATION POLICY $25.00 Administration Fee will be retained from your security deposit for cancellations more than 60 days before event. $50.00 Administration Fee will be retained from your security deposit for cancellations less than 60 days before event. 100% of Security Deposit will be retained for cancellations less than 30 days before event. SECURITY DEPOSIT REFUND POLICY Following inspection of the rental, any refundable security deposit is issued by a PPCA check, usually within 10 days after the date of the rental. Security Deposit is to be picked up at the PPCA office, from 1:30 to 5:30 pm, Monday through Friday. INITIALS __________ (Rev 03/4/10) 2. RENTAL RULES IT IS THE RENTER’S RESPONSIBILITY TO INSURE THAT GUESTS ABIDE BY THE RULES. THE RENTER . . . . . must be a member with the Pacific Palisades Community Association, except for Non-Fee Rentals. is financially responsible for any and all damages occurred at the facility and property while in possession of the rental keys. If furnishings (table $70.00 or chair $25.00), fixtures, equipment, etc., are damage, your security deposit will be applied to replace those items. If cost exceeds your security deposit, you will be billed the remaining cost to replace those items. must be 21 years of age or older. Any rentals for those younger than 21 years old must have an adult supervision for which the adult is responsible for all terms of this contract. is responsible to control and avoid guest from loitering or congregating outside of fenced area surrounding the commuity center during or after events so that neighbors are not affected by noise, lights, music, etc. must not use nails, hooks, tacks, screws, tape or any decorations that will cause damages to the facility, furnishings, equipment or property. If any damages are caused, your security deposit will be retained to replace the cost. If cost exceeds your security deposit, you will be billed the remaining cost to replace damages. A $30.00 violation fee will be retained from your security deposit if any signs or banners are not removed. Confetti is not permitted. is responsible to obtain the rental keys from the PPCA representative at 8:00 am on the day of rental or a $25.00 violation fee will be retained from your security deposit. is responsible to return the rental keys to the PPCA Representative at 8:00 am the day after rental or a $25.00 violation fee will be retained from your security deposit. must make sure that all the facility lights are turned off (except sensor light located in the covered pavilion), all doors and padlock are secured upon leaving or a $100.00 violation fee will be retained from your security deposit. is responsible for keys and padlock if damaged or lost, your security deposit will be retained to replace the cost. An inspection of the center will be done at 8:00 am the day after rental for any violations or damages to the facility, furnishings, equipment or property. is responsible for all guest on the property during the rental and that they only use those areas of the facility that are specified on the contract. Your entire security deposit will be retained for violation of unauthorized use of the swimming pool. is responsible in compliance with the PPCA Noise Ordinance. All rentals are required to play music in moderation so that it does not disturb area residents. Noise Ordinance is strictly enforced at 9:00 pm for Friday & Saturday Events. All red covered electrical outlets located in the covered pavilion area are set on a timer to go off at 9:00 pm It is the Renter’s responsibility to STOP all entertainment and END your function at this time. Your entire security deposit will be retained for violation of any noise complaints from HPD after 10:00 pm is responsible for any illegal parking having to do with vehicles belonging to participants involved in said activity. is responsible to oversee that all Inflatable‘s, Bouncer’s, or Keiki Rides are connected or powered with a generator. Your entire security deposit will be retained for violation of any electrical outlet use. is responsible to inform all participants that “Street Parking Only” is available on a first come, first serve basis. . 1 hour loading and unloading driveway entrance from Komo Mai Drive. Your entire security deposit will be retained for violation of any vehicles parked on driveway for more than 1 hour. INITIALS __________ (Rev 03/4/10) 3. RENTAL RULES (cont.) IT IS THE RENTER’S RESPONSIBILITY TO INSURE THAT GUESTS ABIDE BY THE RULES. THE RENTER . . . . . is responsible for any illegal disposal of cooking oil and/or grease, failure to follow clean up procedures, including the dumping of trash, moving center stage from original location, center not vacated by 10:00 pm for Full Day Rentals or 5:00 pm for Half Day Rentals, use of water slides or water activities involving excessive use of water. Your entire security deposit will be retained for any of these violations. THE MANAGEMENT. . . . . has the right to be present or visit the center during the rental to make sure all the rules and policies are being followed. If during the visit the management finds any rule violations the manager has the right to enforce rules. may change any such rules from time to time without notice. has installed surveillance cameras in and around the community center. The cameras are placed in the entry points, parking lot and throughout the facility, providing management with valuable insight into any situation that may occur and to contest rule violations. has hired a center monitor to monitor activities during scheduled events. EQUIPMENT INCLUDED WITH RENTAL Center Stage Playground / Multi Activity Gym / Swing Set 320 Folding Chairs (metal) 51 Folding Tables (6 feet length) 2 Refrigerators / Freezer, Stove / Oven 2 Stainless Steel Tables (for kitchen use only) 5 33-Gallon Trash Containers 3 Brooms, 2 Dust Pans, 3 Mops and 1 Mop Bucket . CLEAN UP PROCEDURES Trash and debris - must be removed from the facility, playground, surrounding areas of the property, and outside of fenced area, including the sidewalk and surrounding streets. Tables - remove tape, decorations and wipe clean. Stack by folding legs facing up in 2 stacks (26/25). Chairs - remove tape, decorations and wipe clean. Stack by folding and hanging upside down with PPCA words facing forward in 4 sets of 10 (40 chairs per rack). Trash - must be bagged and removed from property when you leave. No trash may be left behind. Postings - signs, posters, banners, etc., must be removed. Personal Items - presents, food trays, decorations, balloons, tape, etc., must be removed. Furnishings & Equipment - return to proper locations as found at start of rental. Sweep & Mop – all covered areas of center pavilion & kitchen (Do Not Hose). Kitchen - wipe sink, counters, stove/oven, 2 stainless steel tables, refrigerators/freezers. Bathroom – hose floors, toilets, urinal & shower areas. FACILITY WAIVER RELEASE I certify that the information I have given is accurate. I have read the information for the renter, rental, security deposit and rental fee; policy for reservation and cancellation; rental rules and clean up procedures pertaining to the use of the PPCA facility, furnishings, equipment and property. Renter will be responsible for all injuries caused by such use. Renter shall be responsible for reimbursing the PPCA for any violations, loss or damages to the facility, furnishings, equipment or property caused by such use. In consideration of participation as specified at location shown on the above dates and times, Renter does hereby release and hold harmless PPCA from any and all liability or claims for damage or injury to person or property of the undersigned due to Renter’s use of said facility or property, by reason of any act or omission by the PPCA, or any of its officers, agents or employees or the condition of its property.
