Prices: $115 first child, $75 second child, and $50 for all successive children
(signed up during the same season from the same household)
Additional cost is a mandatory membership in the USAW of $41 for each child (to be purchased by you) and a
$50 volunteer check that will be reimbursed after you have worked a minimum of 5 hours of volunteer service.
Price increase as of October 15, 2013 to $125
**A $150 check is required upon receipt of each uniform. This check will be returned to you when the full
uniform is cleaned and returned to PTWC **

I, the parent/guardian of the child named above herby give my approval for his/her participation in Par Troy Wrestling Club. I have read and agree to
the parent code of conduct as well as read and reviewed with my child the child’s code of conduct. I agree to conduct myself in a respectful manner at
all times, absent of any anger, outbursts or any other behavior not exemplary, and will portray good nature, sportsmanship and good character needed
and expected around children in our care. I realize there is a risk of injury to children playing and participating in sports activity, especially contact and
competitive sports. I assume all risks and hazards consequential to such participation. I do herby release , absolve, indemnify and agree to hold
harmless the Par Troy Wrestling Club, its Board, Coaches, Officers, Trustees, Staff, Instructors, Sponsors, Commissioners, Participants and persons
transporting and/or supervising and/or coaching my child. I the undersigned, intending to be legally bound, herby for myself, executors and
administrators waive and release any and all claims for damages I/we may have against the Par Troy Wrestling Club. I have visited the PTWC website

DEPOSIT AND USAW WRESTLING CARD
Required fees:
A USAW membership is mandatory. A USAW wrestling card is required by all wrestlers. This is insurance for your child. If you would
like more information please visit their website at ww.usaw.org.
It your responsibility to purchase a USAW membership online.

Print 2 copies of your card. One for yourself and one for PTWC. (Your child will NOT be allowed to attend any
practices or matches without this card)
Volunteer Assignments Work Bond- $50
PTWC is a successful program in great part due to the dedication and hard work of our coaches, volunteers and parents. We rely on
everyone taking part and volunteering to help our organization especially during our home matches. Every family is asked to
volunteer 5 hours of time to satisfy their $50 work bond.
Ways to satisfy your work bond include concession stand, coaching, set up and break downs of mats before and after home matches
and tournaments.

For people who are not registered for a work assignment by November 15 we will make the assignments. Once you fulfill your
assignment please submit a signed volunteer form (available on our website) and your work bond will be reimbursed. *We
recommend you sign up for a time that is during your child’s match as you will already be there.*
Fundraising: Our biggest fundraiser is a 50/50 raffle. Each family is required to sell one book of 10 tickets priced at $5 each. The
winning ticket will be drawn at our end of year banquet.
Uniform Deposit – $150 post dated check is required. NO UNIFORM WILL BE GIVEN WITHOUT DEPOSIT CHECK. This check will
only be cashed if uniform is damaged, altered or not returned.
Required equipment:
Each child is required to wear headgear and wrestling shoes. If your child has braces a mouth guard is also required. This equipment
is available at most sporting good stores such as Dover Sport Center, Sports Authority and Modell’s or online. Suggested practice
wear is T-Shirt and shorts or sweatpants with no pockets. Please keep in mind we try to have our wrestlers in black at matches so
they look uniform. You are not required to purchase PTWC clothes. If you are, however, purchasing any type of pants or shorts black
would be a good choice.
We will communicate regarding programs, times, dates, schedules, locations and all other important information through website and
email. ***ALL INFORMATION IS LISTED ON WEBSITE**** including practice schedules and locations, matches, times, and
directions to all matches and events.
We have been strong members of the New Jersey Grade School League, Passaic County Youth Wrestling League, North Jersey
Youth Wrestling League, North West Jersey Midget Wrestling League, King of the Mat, New Jersey Future Star League and a USAW
wrestling sanctioned organization with over multiple state place winners and State Champions. All coaches are USAW Bronze
Certified.
