This enables us to provide 24 hours a day, seven days a week coverage for serious illnesses and emergencies. Welcome to Sturdy Pediatric Associates Our goal is to provide the highest quality health care to our patients. We are providing the following information to you so that we can work together to meet your health care needs. Office Hours Our general office hours are Monday through Friday from 9:00am – 5:00pm. Phone Information Dr. Danielle Dunetz .................................. 508-226-0780 Dr. Bradford Germain .............................. 508-222-3960 Dr. Raymond Guillette ............................. 508-222-6542 Dr. Kathryn Hansen .................................. 508-222-2086 Dr. Shazia Latif ........................................... 508-222-2096 Billing Department .................................... 508-236-0101 Our office phone hours are Monday through Friday from 8:30am – 4:00pm. Our phones are forwarded to our answering service each day between 12:00pm and 1:00pm for lunch. If you require medical advice, our phone system permits you to select a triage nurse. The triage nurse can schedule appointments, provide medical advice based on each providers protocol, and/or consult with your child’s provider. To help with coverage when the doctors have left the office, calls are forwarded to either the on-call physician or the answering service. We are associated with the Nurse Telephone Triage Service which acts as a go-between for the patient and the doctors after 7:00pm. They are trained Pediatric Triage Nurses who can give advice. Depending on the situation, they will contact the on-call physician. We encourage our patients to call us for routine and well care visits during normal business hours. Weekend and evening appointments are reserved for emergent issues. What To Bring To Your Appointment You should arrive for your appointment about five minutes early to check in at the front desk. We will ask for your insurance card each time you come so we are absolutely sure we have your current insurance information. We will also verify your home address and contact information, and collect any co-pay due. If we do not receive the most current information, you may be financially responsible for your bill and asked to sign a financial waiver. Testing Services Screening laboratory testing services are offered on-site. For our patients who need more in-depth testing or hospitalization, Sturdy Pediatric Associates is affiliated with Sturdy Memorial Hospital, which offers broad access to the necessary resources and technology needed to care for our young patients. Prenatal Visits with Parents Routine Checkups Sick Visits Immunizations Parent/Child Education Scheduling Appointments Children are seen by appointment only. Remember that most children need an annual physical, particularly kids going to day care, camp, Head Start, as well as those who play sports. Be sure to call three months in advance to schedule a physical exam. We save a limited number of same-day appointments for patients who have urgent issues. These appointments are scheduled in order of availability only. Canceling Appointments We understand that unexpected issues come up that require patients to cancel and reschedule their appointments. We ask for at least a 24 hours notice when canceling or rescheduling your appointment. This will allow us time to schedule another patient who may be in need of urgent care. We also ask that you call us if you are going to be late for your appointment. Referrals Many health insurance companies require you to obtain a referral from your primary care physician before seeing a specialist. If you require a referral, please call the office and follow the informational prompts on our phone system. Release of Medical Information Federal laws require all health providers to maintain patient privacy. For patients 18 years of age and older, no health information can be released to another party without the consent of the patient, even to a parent or other relative. We cannot release medical information without the patient (or parent if under 18) signing an authorization to do so. Please see our Privacy Policy for further information. Let Us Know How We Can Do Better! Billing Procedures We submit billing claims to your insurance company within a day or so of your visit. This is why we ask to see your insurance card each time you see the doctor. Most insurance companies send payment and an itemization of any patient co-pays, co-insurance or deductibles within a month. We then send you an invoice for these items, for which we expect prompt payment. We know that some health expenses can be completely unexpected. If you cannot pay the full amount at once, please be sure to call our billing office to set up a payment plan. We also recognize that health insurance can be confusing, and our billing staff is available to help you with any questions. Please contact them directly at 508-236-0101. We strive to continually improve the service to our patients. We welcome your feedback through our patient satisfaction surveys that we mail to our families. We encourage you to fill them out when you receive them. You can also contact our Practice Manager at 508-236-0115 to discuss any issues or concerns you may have. We appreciate that you have chosen us to provide your health care. Thank you!
