In response to your request for participation in the subject workshop, this is to inform you that your name has been entered
as a participant of the subject workshop with following details:
Date: Dec 02, 2015 TO Dec 05, 2015
Time: 08:30 a.m. to 04:30 p.m.
Venue: CPSP Regional Center
Please confirm your availability for the workshop after seeking permission from your Head of Unit / Department at e-mail:
rc_lahore@cpsp.edu.pk, as soon as possible.
You are required to bring a template of your Synopsis for review & necessary advice. Candidates without a draft template will
not be allowed to sit in the Workshop. If the synopsis has already been submitted or even approved, bring a copy with you
for necessary discussion/proof
In case you are not able to attend the subject workshop on the above mentioned dates you are advised to fill the "Workshop
Postponement Form" and submit it along with Pay Order/Draft of Rs.500/- (Workshop Postponement Charges) in favor of
CPSP at Workshop Management Unit, CPSP, Karachi
Also note that if we do not receive the "Workshop Postponement Form" at least 05 days prior to the commencement of
workshop, your participation will be considered confirmed.

Instructions for CPSP Trainee Workshop
General Instructions:
Candidates are required to confirm their availibiltility for the workshop after seeking permission from the concerned Head
of the Unit / Department.
No photo copies of material given by facilitator will be provided.
All Mandatory Workshops Material has been placed at your e-log book from where you can retrieve and prepare yourself
for the workshops.
Attendance in all sessions of the workshop is mandatory.
The candidate will arrive before 8:30am, because if he/she arrives late the certificates will not be issued to the candidate.
If a candidate misses more than two sessions, he/she will have to repeat the workshop.
If a session or a day of workshop is missed, certificate will not be awarded until missed session / day is covered in next
workshop.
In case of postponement or change of venue, please inform 05 days in advance and fill relevant form that can be obtained
from CPSP Website or Regional Centre along with Rs: 500/- as postponement / change of venue fee.

The e-certificate of participation in the workshop shall be sent at your email address and your e-log (pdf file and e-certificate)
within a week. If e-certificate is not received after this period contact workshop email address

Take care of your valuables yourself. Workshop unit will not be held responsible for any loss.
Use of Mobile Phones during workshop is strictly prohibited.
Special Instructions for Individual Workshops:
Introduction to Computer & Internet.
It is mandatory for every participant to bring their Laptops and their USBs during Workshop.
For IT Workshop and I.T Exemption Test participants are required t bring their laptps with M.S. Office and SPSS installed.
Communication Skills.
Communication Skills Workshop Participants are required to come prepared with a Three Minutes Presentation on a topic
of their choice (Medical or Non Medical) along with laptop in order to gain maximum benefit from the workshop.
Research Methodology, Biostatistics & Dissertation Writing.
Research Workshop Candidates are required to bring a template of their Synopsis for review and necessary advice.

Primary Surgical Skills.
Jewellery, Wrist Watch or Mobile Phones is not allowed in the Primary Surgical Skills Workshop Hall
Since this workshop involves handling of animal material, you are advised to protect yourself from undo wound hazards by
observing health coverage. Kindly bring the following accessories with you:
i. Overalls (Lab coat)
ii. Surgical Cap
