Instructions
Thank you for your interest in scholarships awarded through the Boise State University Foundation.
Scholarships offered through the Boise State Foundation are intended to provide financial support to
students attending Boise State University. If you wish to apply for any of these scholarship
opportunities, you may do so by submitting this 2014-2015 Application by March 15, 2014.
In order to apply, please do the following:
 Review list of scholarships to determine if you are eligible for any scholarships offered.
 Complete the general information below;
 After reading the Consent and Certification, sign and date it;
 Mark the box for each scholarship for which you are qualified and want to apply. If you
received this scholarship in 2013-2014, please state that on the application.
 Include a copy of the first page only of your current Student Aid Report if financial need is
included in the criteria for the scholarship(s) for which you are applying (you can access your
Student Aid Report at www.fafsa.ed.gov);
 Include any additional information required in the scholarship criteria below;
 College transcripts from all schools attended including Boise State University (unofficial copies
are acceptable);
 High school transcript if incoming freshman;
 Essay (no more than one page of 12 point type) stating why you should be selected to receive
the scholarship. Individual scholarship criteria below may have additional essay requirements.
 Recommended: two letters of reference along with their respective completed Reference
Evaluation Form in sealed envelopes as described on the Reference Evaluation Form.
Individual scholarship criteria below may have additional reference requirements.
 Print and mail or hand deliver one packet with all of the required information to the following
by 5:00 p.m. on March 15, 2014. Incomplete and late applications may not be considered.

Consent and Certification
Your signature is required below. Without the required signature, your Application is not
complete and it cannot be processed by the Boise State University Foundation.
To determine your eligibility for scholarship assistance from Boise State University Foundation
(“BSUF”), BSUF will request that Boise State University (“BSU”) provide BSUF with relevant
information, including but not limited to your GPA, grade level, gender, age, major, ethnicity, financial
information, and other pertinent information (collectively, “Personal Information”). By signing this
application, you acknowledge that Boise State University Foundation requires such Personal
Information for the evaluation, awarding, and continued provision of scholarship assistance, and you
hereby authorize BSU to provide such Personal Information as may be requested by BSUF for this
purpose.
The information provided on or with this form, and the Personal Information provided by BSU
(collectively, “Scholarship Information”), will only be used by BSUF and/or its agents for purposes of
evaluating your eligibility for scholarship assistance from BSUF. By signing this application, you give
BSUF permission to use the Scholarship Information for these purposes.
BSUF and/or its agents will not share Scholarship Information with any other party without your prior
