Students Wishing to Study in the United States Affidavit of Support—In order to receive a Student and Exchange Visitors Information System (SEVIS) I-20 or DS-2019 from NYIT, international students must provide a duly signed and notarized Affidavit of Support which shows that there is adequate financial support for your education at NYIT. To secure this form, contact the Office of Admissions at admissions@nyit.edu. Bank Statement—An original bank statement, written on your bank’s stationery, signed and notarized by a bank officer and showing available funds in U.S. dollars, must accompany your completed and notarized Affidavit of Support. Copies of bankbooks or monthly statements are not acceptable. 5. Letters of Recommendation—Two letters of recommendation are required for the four-year nursing program, as well as the occupational therapy, physical therapy, B.S./D.O., physician assistant and all education programs. Note, however, that letters of recommendation can add to the strength of any application. 5. You must score 500 to 550 or higher on the written exam or 173 to 213 or higher on the computer based exam, 61 to 80 on the internet-based exam. 6.
