3. Procedure
Completely fill out the
SM College Scholarship Application form.

Attach clear photocopy of F138 or 4 year High School Report Card with
the Second or Third Grading period’s weighted average.
Attach Certificate of passing the entrance test in school of
choice (if available).
Total household income of at most P150,000 per year, and
Fourth year weighted average grade of at least 88% in the second or
third grading period.
Attach photocopy of latest Income Tax Return (ITR) or
Certificate of Non-filing of Income.

Customer Service Counters of SM Supermall, SM Department Store, SM
Supermarket, SM Hypermarket and Save More on or before December 20.

Qualified applicants will be notified (through telephone call/ sms message) as
to their schedule of interview and IQ test from January – March.

