INSTRUCTIONS
1. Complete all sections of this 4-page application and obtain signatures from both major advisors.
2. Retain a copy.
3. Schedule a 30 minute appointment with an L&S College Adviser to review Application; applications will be not
considered for review in the term you plan to graduate.
THE DOUBLE MAJOR APPLICATION IS A CONTRACT
This application, including the program plan you submit, is a contract between you, the major departments, and the College.
Changes to your program plan must be approved by your major and College advisers.
REQUIREMENTS
Use the checklist below to ensure a complete application. Your application will be approved only when ALL of the following
are true:
 You have completed at least one semester at UC Berkeley and your overall GPA is 2.0 or higher.
 You have completed, or included in your program plan, all university, campus, and college course requirements. This
includes American History, American Institutions, Entry-Level Writing and American Cultures.
LIST below how you have completed or plan to complete each college requirement. Please note that no more than two courses from
the same department can be used to satisfy breadth requirements:

Please note: Beginning Fall 2012, R&C seats will be restricted to freshman and sophomores only. Upper-division
students who have not satisfied R&C will face registration blocks and be unable to access an R&C course at UCB
during the fall and spring terms. Your program plan should account for these changes.

 No more than two upper-division courses overlap between the two majors.
 Your program plan includes all courses you are taking from the semester in progress through graduation. A
minimum of 13 units is required each semester unless the College has given you approval for an exception.
 You plan to graduate no later than the semester you exceed 136 units. (You are permitted one additional semester
if you do not exceed 136 units during the ninth semester of all institutions attended; AP units and units completed
before high school graduation do not count towards this 136 unit ceiling.
 Advisers in both major departments have signed the major declaration, petition for double major, and program
planning forms. These forms must be reviewed and approved by both major advisers in order to verify that your
proposed program satisfies all major requirements and conforms to anticipated departmental course offerings.
