** Important Information ** Spring Quarter 2013 Registration Fee Breakdown and UC Student Health Insurance Plan (UC SHIP): UC Student Health Insurance (UC SHIP) If you are assessed after the fee payment deadline date you may still submit a UC SHIP waiver. You must contact the Ashe Insurance Office at INSURSHS@ashe.ucla.edu or call at 310-825-4073, option #4 BEFORE paying your fees. UC SHIP waivers will not be granted after your fees are paid. The cost of health insurance is included in your registration fees. Health insurance is a condition of enrollment. Your registration fees include either $439.34 per term for undergraduates (No dental) or $747.65 per quarter for graduates or $1121.47 per semester (law and medical) for GSHIP (Graduate Student Health Insurance Plan includes dental coverage). Students who are already covered by a qualified health insurance policy may have this fee waived, if qualifying information is submitted by the fee payment deadline each term. Students should complete the UC SHIP waiver worksheet before attempting to complete the actual waiver. An undergraduate student may improve UC SHIP Basic benefits by enrolling in UC SHIP Plus and purchase the dental buy up for $94.79 per term. UC SHIP Plus will require enrollment for the academic policy year or when the student is first eligible. Waivers must be submitted before fee payment deadline. There are no refunds after the deadline. Waivers must be submitted every academic year. University Hepatitis B Requirement: All incoming undergraduate students who are 18 years of age and younger must fulfill the University Hepatitis B Requirement; the requirement is strictly electronic. To fulfill the requirement, you must submit your series completion date (read: date of your third Hepatitis B shot) and the contact information for the facility where you received the shot to the Ashe Center website. The link to complete the requirement is located on the Ashe Center homepage under the "New Bruins" section. Once you submit your information, you will receive a confirmation number to indicate your compliance. The requirement should ideally be fulfilled prior to your first quarter here at UCLA. Failure to fulfill the requirement within the required time period will result in a hold being placed on your registration. No paper documentation is required to fulfill the requirement; however, you may submit a copy of your immunization records to the Ashe Center for inclusion in your electronic medical record if you prefer. Recipients of Academic Apprentice Personnel Deferrals (TA Deferrals): Fee deferrals must be posted by your department before the March 18th fee payment deadline. Fee deferrals allow you to obtain services as a fully paid student. All fee or stipend awards posted to your BAR account will apply towards your fees and a refund will be issued if applicable. The remainder of your deferred Spring fees must be paid before 5:00 p.m. on May 20th. Whether you attend UCLA, take a leave of absence, or withdraw from the University, you are responsible for these registration fees, but may be eligible to receive a partial fee refund, according to the official refund schedule. Fees not paid by the deadline are subject to the late fee charge. Students Not Attending Spring Quarter 2013 Term: If you are being charged registration fees and you are NOT planning to attend UCLA for this term, you must declare your non-attendance using URSA. You may use this option ONLY if your registration fees have not been paid. Be sure to consult the Academic Policies section in the current Schedule of Classes for readmission information (available at http://www.registrar.ucla.edu/schedule/). Graduate students: declaring nonattendance does not place you on an official leave of absence. You must submit a leave of absence form from your department which then requires departmental and Graduate Division approval. Third Party View……we listened to you! Students can authorize third parties to have view-only access to their student records and to make payments towards their BAR account. A third party establishes his or her own separate UCLA Logon ID and password (available at https://logon.ucla.edu), communicates this logon information (name and UCLA Logon ID) directly to the student, and the student selects which URSA features the third party can view or access.
