Historical Background
Before 1937, the treatment of fractures in large and small animals was confined mainly to casts and splints.
Between this period and the mid-1960s, fixation by Stader and Kirschner-Ehmer splints, bone sutures, and the
occasional Sherman or Lane bone plate was added to the veterinary armamentarium. The functional and anatomical
results were less than desirable in many cases, and nonunion, malunion, and stiff joints were not uncommon.
This mirrored the state of the art of internal fixation of fractures in man at the time.
Fracture treatment, however, took a tremendous leap forward in 1958 when the Swiss Association for the Study of
Internal Fixation (in German: Arbeitsgemeinschaft für Osteosynthesefragen—AO) defined the biomechanical principles
for successful internal fixation of fractures, and implemented these principles with the creation of an entire system of
stainless steel implants and instruments. Their applied fracture research was carried out in the Laboratory for
Experimental Surgery in Davos, Switzerland, based on metallurgical expertise gleaned from the watch-making industry
and expert European toolmakers.
Documentation in Bern of more than 100,000 clinical cases using these AO methods, and based on a comprehensive
classification of fractures, confirmed the reliability and quality of the system. The central theme was the rapid return of
function after fracture treatment. This was accomplished by anatomical reduction, stable internal fixation with implants
utilizing interfragmentary compression, buttressing, medullary splintage, and preservation or enhancement of the blood
supply in the fracture zone by early exercise and early bone grafting. There was usually no necessity for casts or
external splintage. Emphasis was also placed on preoperative planning, operative asepsis, gentle soft tissue handling
and atraumatic exposure of the fractured bone, and early postoperative mobilization of the part and the patient.
Protected weight-bearing and careful radiographic monitoring until union was achieved completed the regime.
The bridge between human and veterinary fracture treatment, and indeed orthopaedics, began to be built early in the
1940s by Kuntscher with the experimental work on intramedullary nailing in dogs. Dr. Jacques Jenny, as a resident in
Zurich in 1943, assisted Dr. H. Knoll and Dr. H. Willenegger with the first clinical canine femoral intramedullary nailing.
In 1965, Drs. J. Jenny and R.B. Hohn, encouraged by Dr. Howard Rosen, introduced AO techniques in large and small
animal orthopaedic surgery at the University of Pennsylvania and the Animal Medical Center in New York, respectively,
utilizing human AO principles and instrumentation.
Following participation in 1969 at a human AO course in Davos, a number of active American and Swiss veterinary
surgeons initiated courses for veterinarians at Bettlach, Waldenburg and Davos in Switzerland, and at the Ohio State
University in Columbus, Ohio, in the United States. Veterinary AO sections were started in these two countries with the
special help of Fritz Straumann, and were united with groups from other countries as an international organization in
1970. Basic and Advanced courses have been held regularly in Switzerland, the United States, and other countries.
Research led to the design of new implants which would conform to the anatomical and functional requirements of
animals, and to the definition of techniques specific for the various animal species. The original AO principles have
proven to be as successful in large and small animals as in humans. This has been clearly demonstrated by analysis
of the documentation of the treatment and follow-up of thousands of animal fractures.

Synthes offers a lifetime warranty for catalog instruments purchased from Synthes. This warranty presumes that the
instruments are used with Synthes implants under normal operating conditions, and with established AO techniques.
Excepted from this warranty are items which have been abused and those that become unserviceable through prolonged use.

Warranty is limited to the repair or replacement of instruments.
•
Repair or replacement is dependent upon return of the instrument to Synthes.

Excluded from this warranty are all cutting instruments, i.e. drill bits, taps, chisels, fixed head reamers, osteotomes,

Information on Synthes Stainless Steel Implants
Synthes implants are produced from implant-quality type 316L stainless steel, which typically contains 62.5% iron, 17.6 %
chromium, 14.5% nickel, 2.8 % molybdenum, and minor alloy additions. A low carbon composition is specified to assure that
the material will be free from susceptibility to intergranular corrosion.
Implant-quality 316L stainless steel meets the metallurgical requirements established by the American Society for Testing and
Materials (ASTM) and the International Organization for Standardization (ISO). The major implant industry standards include
ASTM F 138 (bar and wire), ASTM F 139 (sheet and strip), and ISO 5832-1. Implant-quality material must also meet stringent
Synthes internal specifications for composition limits, microstructure uniformity, and mechanical properties. All implant-quality
material received from approved suppliers is recertified by Synthes (USA) to provide an extra measure of Quality Assurance that
metallurgical requirements have been met. Other implantable grades of stainless steel such as 22Cr-13Ni-5Mn have also been
used for specialized applications.
Certain implants, such as cerclage wire or pelvic reconstruction plates, are produced from AO implant-quality 316L stainless steel
in the annealed or softest condition. This is needed for applications which demand maximum contourability. The majority of 316L
stainless steel implants are produced from cold work or strengthened material. Some small diameter implants such as K-wire,
Steinmann pins, and Schanz screws may be highly cold worked to resist permanent bending deformation. Implant-quality 316L
stainless steel is completely nonmagnetic, regardless of condition.
Synthes instruments and implants are designed and manufactured to maintain optimum system performance. For this reason,
316L stainless steel implants produced by other manufacturers should not be mixed with 316L stainless steel implants. The in vivo
performance of a mixed implant system may be compromised as a result of composition differences, strength variations, and
design or workmanship factors.

Causes of Corrosion and Surface Changes
Incorrect handling, exposure to various aggressive substances, and mechanical and thermal damage can significantly
decrease the corrosion resistance of the AO instruments. The following section deals with helping the user avoid a number
of possible causes of corrosion or damage.
Blood, Pus, Secretions
These surgical residues contain chloride ions which lead to corrosion, if left on instruments for an extended period of time,
especially if allowed to dry. Always clean and dry every instrument after use. Joints and tubular items should always be
rinsed and dried thoroughly.
Saline, Iodine Solutions, Water
Solutions containing halogens such as chloride and iodine ions can lead to pitting corrosion, especially when the passive
layer on instruments has been penetrated. Avoid prolonged exposure to these solutions. Tap water often contains a high
concentration of minerals which can be seen as a fine deposit on the instrument surface. Rinsing with distilled water
prevents such deposits. If deposits are present, they can generally be removed with a special nonabrasive steel cleaner.
Cleaning and Disinfecting Agents
Excessively strong concentrations of such substances, as well as strong alkaline or strong acidic solutions, can lead to
pitting corrosion. It is vital to observe the manufacturer’s instructions for suggested concentrations and exposure times.
Optimum cleaning agents are generally regarded as those with a pH value between 9.5 and 11. The temperature of such
agents should not exceed 45°C (115°F), to avoid protein coagulation and evaporation. A thorough rinsing to remove all
residue is necessary.
Abrasive Cleaning Materials
Abrasive pads, steel brushes and abrasive cleaners should never be used to clean surgical instruments. They damage the
passive layer of the instrument, which can eventually lead to corrosion.
Contact Between Instruments of Differing Alloys
Existing areas of rust can be transferred onto other instruments by an electrolytic process. Stainless steel instruments that
are in close contact for an extended period of time with items such as damaged chromium-coated instruments, and that
at the same time are dampened by an electrolyte, can develop rust at points of contact. Electrolytes can be steam, water,
ultrasonic cleaning agents, etc.
All instruments that are not made from the same alloy should be separately cleaned and sterilized. It is essential to replace
instruments that have started to corrode.
Inadequate Lubrication
Moving parts on instruments, such as joints, screw threads, etc., should be lubricated regularly. This reduces the resultant
wear that increases the risk of corrosion. Refer to the Synthes (USA) orthopaedic catalog for lubricant.
Cleaning Agent Residue in Wrapping Materials
Muslin used for wrapping instruments must not contain any residue from cleaning agents. Residue may be transferred
onto the instrument surface by steam, which can result in penetration of the protective passive layer and eventually lead
to corrosion.
Misuse of Instruments
AO instruments have been designed for specific purposes and should be used accordingly. Using an instrument incorrectly,
or for a purpose for which it was not designed, can result in mechanical overloading and permanent damage. This also
increases the likelihood of sensitivity to stress corrosion.

The AO set system permits a reasonable number of basic instruments to be used for many applications and the ability to combine
these standard devices with additionally available instruments and implants for special cases and applications.
Standard Instrument Sets
These sets contain basic instruments and implants which permit the treatment of most fractures. Their use is described in the latest
editions of the AO Principles of Fracture Management in the Dog and Cat and the AO Principles of Equine Osteosynthesis. If the
techniques described in these manuals are properly followed, the chances for successful fixation are greatly improved. It is
important that the standard instrument sets should always be complete so that during an operation an important instrument or
implant is not missing. While the number of screws or plates in the implant sets can be reduced or adapted to the usage of the
clinic, it is not advisable to delete certain lengths or certain types of plates.
Note: Because of the continuing development of the AO Instrumentation by the AO Technical Commission, it is possible that
shape, form and dimension of the instruments and implants or the contents of certain standard sets are changed from
time to time. Always refer to the latest edition of your Synthes catalog for details.
