• Offer valid only for purchase of a V600 in
conjunction with new activation on a qualified
rate plan from an authorized T-Mobile retail
store, 1-800-tmobile or online at www.tmobile.com.
• A qualified rate plan is any T-Mobile rate
plan with a monthly access fee of at least
$39.99/mo. per line of service or up to 5 lines
of a FamilyTime® plan $49.99 a month and
higher.
• Your account must be active on the qualified
rate plan when the rebate is processed to be
eligible.
• Phones purchased at Wal-Mart (with a
Wal-Mart receipt) are not eligible. Wireless
Retail service agreements (with purchase
date) through Wal-Mart are accepted.
• All phone purchases and activations MUST
take place between 11/1/04 and 12/31/04.
Purchases and activations before 11/1/04 or
after 12/31/04 are ineligible.
• Rebate fulfillment form must be postmarked
on or before 1/31/05.
• Failure to cash a rebate check within 90 days
of issuance shall void the rebate and T-Mobile
shall have no obligation to pay the rebate.
