• Offer valid only for the purchase of a T-Mobile
Sidekick LX in conjunction with new activation
on a qualified rate plan from an Authorized
T-Mobile Location.
• Phones purchased at Best Buy, Wal-Mart,
Sam’s Club, Target, Walgreens or Costco
are not eligible.
• Offer not valid on FlexPay plans without a oneor two-year contract.
• A qualified rate plan is (a) any individual or
business enterprise Voice and Data rate plan
with a monthly access fee of at least $34.99/
mo. per line of service or (b) any FamilyTime*
rate plan of at least $49.99/mo. for up to
5 lines of service, or (c) a Sidekick VoiceBarred/Hearing Impaired rate plan with a
fee of $29.98/mo. per line.
• To be eligible for this rebate, your handset must
be used at least once on your T-Mobile account
prior to rebate validation, and your account
must be active at the time of rebate processing.
• Rebates may not be combined or used with any
other rebate, free, or other promotional offers.
• Rebate checks are void if not cashed within
ninety (90) days of issuance. Failure to timely
cash a rebate check will void the rebate, and
relieve T-Mobile of any further liability
or obligation to pay the rebate.
• Valid rebate requests take up to 8 weeks
from receipt to process.
• Rebate will be mailed to your current
T-Mobile billing address.
• Rebates may not be applied toward your
