PREFACE
This Citizens’ Charter of the Local Government Unit (LGU) of Mabalacat is a
collaborative effort of its officials and employees and it intends to contain in codified form as
prescribed by Republic Act No. 9485, known as the Anti-Red Tape Act (ARTA) of 2007 the
various functions of, and services rendered by the departments and offices in the LGU,
together with the persons responsible for the delivery of basic services to the public in
efficient and effective fashion and in prompt period of time and performance are hereby
embodied in this Charter.
This Citizen’s Charter of Mabalacat reaffirms the commitment of the municipality’s
leaders, officials and employees to implement quality management programs, streamlined
procedures in the delivery of basic services and accepted work standards glared towards
enhancement and transparency, accountability and accessibility to the public.
With this work, it is expected that the discharge of function in every department,
particularly the frontliners of the LGU shall enhance the rapport between the LGU’s,
workforce and the public clientele in so far as public service is concerned.

MESSAGE
I take pride in the collaborative efforts of the officials and employees of the Local
Government Unit (LGU) of Mabalacat, Pampanga in crafting our own version of the
Citizens' Charter which is mandated to be done under Republic Act No. 9485, otherwise
known as the Anti Red Tape Act of 2007 or ARTA.
This Charter encompasses the scope and range of work of the front line offices of our
LGU aimed at giving our clientele commendable measure of satisfaction thru prompt
delivery of services required of us and documents requested.
The flowcharts and functions shown therein enable the transacting public to follow
through with ease and convenience the transactions they are involved in and get prompt,
satisfactory results.
In closing out this Citizens' Charter, the tenets of transparency and accountability
were duly factored in.

INTRODUCTION RATIONALE
Act No. 9485 or An Act to Improve Efficiency in the Delivery of Government
R epublic
Service to the Public by Reducing Bureaucratic Red Tape Preventing Graft and
Corruption and Providing Penalties therefore, otherwise known as the ANTI-RED TAPE
ACT (ARTA) OF 2007.
The Act was signed into law by President Gloria Macapagal-Arroyo on June 2, 2007
and took effect on September 5, 2008. It provides that all government agencies including
departments, bureaus, offices, instrumentalities or government-owned and/or controlled
corporations (GOCCs), or local government units (LGUs) shall set up their respective
standards to be known as the Citizens’ Charter.
The Act also aims to promote efficiency and transparency in government with regard
to the manner of transacting with the public requiring each office to simplify frontline service
procedures, formulate service standards to be observed in every transaction and make known
these standards to the clients/citizens.
By defining the step by step procedure and reducing the number of signatories in
every transaction, the task of eliminating red tape is a glimmer of hope if a Citizens’ Charter
is set up in every agency and LGU.
This is the Mabalacat Citizens’ Charter our pledge and our tool to shield the citizenry
from the social menace, that is corruption.

MANDATE
T
o serve primarily as a general purpose government for the coordination and delivery of
basic, regular and delivery of effective governance of the inhabitants within its
territorial jurisdiction.
VISION STATEMENT
A
peaceful, politically mature and morally reformed society dedicated to alleviate the
plight of the less-fortunate citizenry who are wallowing in the quagmire of poverty; a
society that is concerned with the protection and conservation of natural resources; a society
that could give more to those whose have less without decreasing the gains of those who
have more; a society of law abiding citizens living in the atmosphere of peace and order.
MISSION STATEMENT

romote local autonomy by developing and strengthening the capabilities of the
Municipal Government in order to effectively provide the necessary basic services to the
people; institutionalize people empowerment by enjoining them to participate in local
governance in order to achieve a progressive and self-reliant community.
IMPROVEMENT MECHANISM
I
nstallation of Public Assistance and Complaints Desk; Setting-up of
Suggestion/Complaints Box at the lobby; and providing of Client Feedback Form. Any
aggrieved or unsatisfied transacting public may report the erring personnel to the Human
Resource Management Office (HRMO), who in turn may conduct investigation, reconcile all
issues and suggest amends, located at the Ground Floor of the Municipal Hall, Xevera
Tabun, Mabalacat, Pampanga, or simply drop your comment/complaint in the
Suggestion/Complaint Box. Received complains/suggestions will be treated with strict
confidentiality.

CREATING, ADOPTING AND IMPLEMENTING THE CITIZEN’S
CHARTER OF MABALACAT AS PUBLIC’S GUIDE IN AVAILING
FRONT LINE SERVICES PROVIDED BY THE LOCAL
GOVERNMENT UNIT
WHEREAS, Administrative Order No. 161 dated October 5, 2006, as amended by
Executive Order No. 605 dated February 23, 2007, enjoined the implementation of a
government-wide Quality Management program which mandated, among others, the
streamlining of services and the establishment of citizens’ charter;
WHEREAS, the Anti-Red Tape Act of 2007 otherwise known as Republic Act No.
9485 was enacted to promote integrity, accountability, proper management of public affairs
and public property as well as to establish effective practices aimed the prevention of graft
and corruption in government;
WHEREAS, in accordance with this policy, local government units have been
mandated by R.A. 9485 to set up service standards to be known as Citizens’ Charter;
WHEREAS, the Municipality of Mabalacat crafted the MABALACAT CITIZENS’
CHARTER that will serve as the public’s guide in availing frontline services, such that the
step by step procedures in obtaining services, with the corresponding fees to be paid and the
person in-charge of that particular service are outlined;
WHEREAS, the MABALACAT CITIZENS’ CHARTER aims to promote
transparency in government by providing an avenue for complaints and grievances against
erring government personnel while also encouraging the public to participate in pursuit of
attaining a graft-free local government by adhering the rules in obtaining services;
WHEREAS, the local government of Mabalacat conducted a Public Consultation on
the Mabalacat Citizens’ Charter to generate maximum awareness on the provisions of the
same;
WHEREAS, the local government unit of Mabalacat will continue to undertake
programs to promote customer satisfaction and further improve service delivery to its
clientele;
NOW THEREFORE, I MARINO P. MORALES, Municipal Mayor of
Mabalacat, Pampanga, by the powers vested in me by law, do hereby order:
Section 1. Creation, Adoption and Implementation of the MABALACAT
CITIZENS’ CHARTER as public’s guide in availing Frontline Services provided by the
Local Government Unit;

Section 2. Adherence to the provisions of the Mabalacat Citizen’s Charter by all
concerned employees of the Local Government of Mabalacat to minimize and totally
eradicate Bureaucratic Red Tape;
Section 3. Providing the public an avenue for complaints and grievances against
erring government employees that compromise the integrity of public service;
Section 4. Effectivity. This Administrative Order shall take effect immediately.

ISSUANCE OF COMMUNITY TAX CERTIFICATE
A. ABOUT THE SERVICE
A Community Tax Certificate (CTC) is a basic document acquired by any
individual or citizen at least 18 years of age and above or juridical being for
identifying himself and his residence which can be used for legal transaction. Natural
and juridical person may secure a CTC.

ISSUANCE OF CERTIFICATION/TAX CLEARANCE
A. ABOUT THE SERVICE
Issuance of certification or tax clearance is a provision from the Revenue
Code Municipal Ordinance No. 9, S-2005 also known as the Revenue Code of
Mabalacat, Pampanga. It is issued for the purpose of the following:
a. As one of the requirements for medical treatment, judicial proceedings,
scholarship or a supporting document as required by any private or government
entity.
b. As one of the requirements in lieu of lost official receipt from Business/Real
Property Tax Payment.

a. Any resident individual of the municipality of Mabalacat may avail of the service.
b. Any taxpayer whether resident or non-resident who owns business or real

ISSUANCE OF REAL PROPERTY TAX RECEIPT (R.A.7160 – LGC 1991)
A. ABOUT THE SERVICE
Real Property Tax (RPT) – It is a yearly AD VALOREM TAX on real
property such as land, machinery & other improvement not specifically exempted
under the law (Sec.232 of Local Government Code). The maximum tax is 2% of the
Assessed Value.

purposes to real property owner located within the jurisdiction of the Municipality of
Mabalacat.

ISSUANCE OF CERTIFICATION OF PROPERTY HOLDINGS
A. ABOUT THE SERVICE
Certification of Property Holding is requested by property owner whose
property is located within the jurisdiction of the Municipality of Mabalacat for Estate
Tax, court proceeding, and verification of status of property purpose, and upon
request of farmer beneficiary of the Department of Agrarian Reform.

Any person, natural or juridical is in need of the said certificate.

Rule 47 of A.O. 1, S of 1993 states that: “Where a marriage license is
required, each of the contracting parties shall file separately a sworn application for
such license with the proper civil registrar…”

A. ABOUT THE SERVICE
Although it is said that the birth record is a permanent record of the person,
the law somehow allows certain modifications as when there is a legitimate event that
causes the modification of the person’s record, as in: a court order to effect a change
or correction; or, conformity with certain administrative procedures; or as a
consequence of an approved quasi-judicial function by this office. Marginal
annotation is therefore inevitable.
B. WHO CAN AVAIL OF THE SERVICE
A person whose document is registered at the MCRO of Mabalacat, once
affected by the aforesaid legal modification is entitled to the issuance of annotated
certificate of birth, marriage, or death.

THE CIVIL REGISTRAR
A. ABOUT THE SERVICE
Any person has the right to request for a copy of his/her registered document.
As such, upon the payment of the required fees and for as long as there is still a copy
of the document at the archives of the MCRO, the same can be issued. Otherwise, a
certification to the contrary may be validly issued. The office observes the
confidentiality rule on registered Certificates of Live Birth.
B. WHO CAN AVAIL OF THE SERVICE
Following the legal maxim, “The place of occurrence is the place of
registration”, A person born, wed and died within the jurisdiction of Mabalacat is
deemed to have been registered in this office. Hence, he/she is entitled for the
available copy issuance of his/her birth, marriage or death record once requested.
C. REQUIREMENTS
1.
If the child is a minor, his parents, guardian, or the institution legally in-charge
of him, as the case may be, shall issue the authorization required.
The duly signed valid ID of the one giving the authorization and the grantee
shall be presented for verification.

A. ABOUT THE SERVICE
The best evidence to prove the legal existence and filiations of a person is
one’s Certificate of Live Birth (COLB). The registration of a person’s birth vests in
the child his/her right to a name, nationality, identity and family. In the same manner
the registration of the Certificate of Marriage (COM) is a prima facie document to
prove the married status of the person. The registration of marriage establishes on
records the existence of the legal union between husband and wife. The Certificate of
Death (COD) proves, on the other hand, the cessation of man’s existence temporal,
and its registration presupposes the official termination of man’s existence temporal.
B. WHO CAN AVAIL OF THE SERVICE
Following the legal maxim, “The place of occurrence is the place of
registration”, all persons born, married and died within the territorial jurisdiction of
Mabalacat should be registered before the Civil Registrar’s Office of Mabalacat.
C. REQUIREMENTS
1. Four copies of the Certificate of Live Birth (with Cert of Marriage of Parents) /
Certificate of Marriage / Certificate of Death duly accomplished and signed by the
proper parties.
2. For COLB to be registered of an illegitimate child, the father should appear
personally before the MCR with his Cedula.
3. Timely registration of birth is within 30 days from its occurrence, while 15 days is

A. ABOUT THE SERVICE
The office recognizes the right of the person to register their document though
outside their prescriptive period so as to provide a credible data of facts necessary for
governmental usage. A vital event is considered for late registration if the prescriptive
period for its reporting has already lapsed. In cases of birth: within 30 days from its
occurrence; while 15 days is required for marriage; and 30 days for death. Outside of
these days prescriptions are considered delayed registration which will require sets of
needed documentary evidences.

A business establishment is required to secure a Zoning Clearance upon
application of Business Permit to ensure that the establishment is allowed in the
chosen location as per the Comprehensive Land Use Plan and Zoning Ordinance and

ISSUANCE OF LOCATIONAL CLEARANCE
A. ABOUT THE SERVICE
An enterprise and private person constructing a new building or applying for
expansion/renovation/alteration is required to secure a Locational/Zoning Clearance
at the Municipal Planning & Development Coordinator’s Office/Land Use and
Zoning Division prior to the application for Building Permit. This should be done

Site zoning is requested by a taxpayer and/or any individual to enable the
property owner to know the use of his/her parcel of land in accordance with the
approved Comprehensive Land Use Plan and Zoning Ordinance. The Zoning

A. ABOUT THE SERVICE
The issuance of Motorized Tricycle Operator’s Permit is a requirement of the
Land Transportation Office (LTO) for the legal operation of Motorized Tricycle Unit
within the area of jurisdiction of the municipality. It is also a legal requirement for the
renewal of vehicle registration at LTO.

ISSUANCE OF SOCIAL CASE STUDY REPORT
A. ABOUT THE SERVICE
Provision of medical/financial assistance to individual or family in crisis
situation, more particularly those who are admitted in the hospital and for some
reasons cannot manage to clear up their hospital accountability, individual diagnosed
with cancer, end stage renal diseases and the like, that condition requires lifetime
treatment to enable him/her to sustain his/her expensive therapeutic requirements,
bring back his/her normal functioning and help him/her deal with his/her problem by
providing well-timed and suitable assistance.
B. WHO CAN AVAIL OF THE SERVICE
Any bonafide resident of Mabalacat who belongs to marginalized group of
families and any who suffers from above cited illnesses can avail of the service.
C. REQUIREMENTS
1. Barangay Certification
2. Medical Abstract or Certificate with signature over printed name of doctor and
licensed number
3. Itemized statement of account duly signed by the billing officer for individual
seeking hospitalization assistance with corresponding promissory note to prove
the unpaid hospital billing.
4. Treatments protocol and costing of treatment for chemotherapy & hemodialysis
cases.
5. Costing of procedures for laboratory or special procedures like CT scan, MRI and
the like with the hospital acceptance letter.
