Who may be eligible: Ontario Works participants, Ontario Disability Support Program recipients and people with low-incomes who have earnings such as wages, Employment Insurance, WSIB, OSAP, CPP, OAS GAINS and other types of pensions may be eligible for assistance. Applicants can request financial assistance with: Complete and partial dentures up to the following maximums Complete dentures $900.00 Partial dentures – Upper/Lower $450.00 Reline $150.00 Repair $110.00 To apply for Discretionary Benefits: By telephone (519) 661-5910 By fax (519) 661-6488 In writing Discretionary Benefits Program P.O. If you are not in receipt of Social Assistance, you will be required to complete an application and attend an interview. An application may be taken in your home if you are unable to attend an interview due to health reasons. Eligibility: After you have submitted your request, your eligibility will be determined based on the medical nature of the dentures and the costs of your specific needs. If you are eligible a Purchase Voucher will be mailed to you. In you are ineligible you will be advised in writing and details of the reasons for ineligibility will be outlined Supporting Documentation: You will be required to submit a copy of the estimate from a qualified denturist or dentist. Once your request and documents have been received and reviewed, you will be contacted within ten (10) business days.
