CMA/PFA Dual Designation Program consists of 4 modules, Governance and Public Policy; Public Finance; Financial and Performance Reporting; and Audit and Assurance. To be eligible to receive the PFA designation candidates must complete each of the 4 course modules successfully with a minimum grade of at least 60%. The 4 modules need not be completed in one year nor in any particular order. If a candidate is unsuccessful in any module or modules, that module must be successfully completed before being able to use the PFA designation. This program is offered in distance learning format. Distance Learning candidates will be provided with DVD’s of the lectures and are required to submit their assignments and/or sit for their examinations on the dates listed on this application. CMA®/PFA – APPLICATION FEE STRUCTURE The fees for the CMA/PFA Program are $7,000.00 (plus HST) payable in full or in four instalments per the table below: Module 1 Governance and Public Policy GPP Final Assessment due October 18th (3,000 word essay) Module 1 fee* - $1,900.00 + $247.00 HST = $2147.00 The application and fee is due August 8, 2012. Module 2 Public Finance PF Examination – November 24th (2½ hour examination) Module 2 fee* - $1,300.00 + $169.00 HST = $1469.00 The application and fee is due October 3, 2012. Module 3 Audit and Assurance A&A Final Assessment due February 14th (3,000 word essay) Module 3 fee* - $1,900.00 + $247.00 HST = $2147.00 The application and fee is due November 8, 2012. Module 4 Financial and Performance Reporting FPR Examination – April 20th (3 hour examination) Module 4 fee* - $1,900.00 + $247.00 HST = $2147.00 The application and fee is due February 09, 2013. *Program fees cover the study materials, marking of examinations or assignments. All fees are subject to change without notice. Page 1 CMA®/PFA – APPLICATION REPEATING THE PROGRAM Candidates, who are unsuccessful in any of the program modules, will have one final attempt to re-take the module(s) examination or assignment. Candidates may rewrite each failed examination and assignment the following year or re-take the full module(s). To enrol for the retesting a written request must be received by CMA Ontario which includes name, member number, module number, location preferred, and a cheque payable to Certified Management Accountants of Ontario for $300.00 + $39.00 HST = $339.00 for each examination or assignment. Registration for retesting should be in our office no later than four weeks prior to each examination or assignment date; applications for each examination or assignment should be submitted individually. A location confirmation and/or assignment guidelines will be mailed to you when your request has been processed. REQUEST FOR CANCELLATION AND REFUND POLICY Cancellation requests are subject to the notification deadlines and administrative fees. Payment by either cheque or credit card is acceptable. If paying by cheque, please mail the cheque, payable to Certified Management Accountants of Ontario, and this form to the address on the front of this application. If paying by credit card, you may submit this form by fax to 416-977-2128 or by mail to the address on the front of this application. Requests for cancellation will be accepted only when the request is postmarked no later than two weeks beyond the enrolment deadline date and the request must be submitted in writing.
