Northwestern Illinois Community Action Agency LIHEAP Cooling Assistance Northwestern Illinois Community Action Agency, serving Stephenson and Jo Daviess counties, has received notice from the Illinois Department of Commerce and Economic Opportunity that a LIHEAP Summer Cooling Program will be implemented beginning July 15 and ending July 26, 2013 or when funds are exhausted. Assistance will be in the form of a one time payment to eligible applicants’ electric utility. If you receiving PIPP benefits for your electric utility, you are NOT eligible for LIHEAP Cooling assistance. Benefits can be provided only to income-eligible households in one of the following groups: Households that contain a member who is elderly (age 60 and above). Households that contain a member with a medical condition that can be improved by air conditioning such as respiratory, coronary, or terminal disease. Required documentation must include a doctor’s certification letter dated within the last 90 days, stating that the medical condition would be aggravated by extreme heat. Households that contain children 4 years of age or younger. Households that contain a member receiving social security disability benefits or with a pending determination case at the Social Security Administration. Required documentation must include a letter of determination from the Social Security Administration or receipt of social security disability benefit statement. Households must have income equal to or less than 150% of the poverty level as defined in the 2013 Federal Poverty Guidelines to be eligible to apply. Proof of ALL income for ALL members of the household for the 30 days prior to the application date. If a household member is self-employed, proof of income is required If a household has zero income, proof must be provided. Most recent electric bill in its entirety, which must be in an adult household members’ name. If applying because of medical need, doctor’s medical certification letter dated within past 90days.
