Coolidge Court is one of the nation's only affordable low-income supportive housing
programs for young adults exiting the foster care system. Coolidge Court provides safe
housing and on-site support services for 18 tenants with mental health diagnoses. Over
40% of residents are currently enrolled in college, and many are successfully employed.

Transitions provides comprehensive and individualized services to youth transitioning
out of foster, residential, or family care and into adulthood. The agency established this
program in 2002, after extensive experience with youth who were unprepared for the
transition from foster care and group homes to independent living. The program currently
serves 40 foster youth, ages 16-21.

Covenant House California
Rights of Passage Transitional Living Program
Rights of Passage is a 32-bed congregate in Hollywood and a 6-bed program in Oakland, which
offers a transitional living program that emphasizes the development of life skills that will
eventually allow youth to achieve and maintain an independent lifestyle in their own permanent
housing.

Contra Costa County Health Services Transitional Housing
Appian House is a 6-bed transitional living program for youth ages 18-21 who are homeless or
exiting the foster care system. Residents are provided guidance and support to complete their
education and/or maintain employment that will eventually lead to permanent housing.
Over the course of the 18-month program, each youth participates in the creation of a
personalized Individual Development Plan (IDP) with his or her case manager. Independent
living skills, such as money management, employment search, nutrition, and interpersonal life
skills workshops are offered and included in each youth’s IDP7.

To support their commitment to completing LACC training, participants are offered the
opportunity to live in an LACC-sponsored Transitional Living Apartment. Here they live rentfree with the caveat that 50% of their paycheck goes into a savings account. While residing in the

Rights of Passage is a 32-bed congregate in Hollywood and a 6-bed program in Oakland, which
offers a transitional living program that emphasizes the development of life skills that will

Supportive Apartment Program
Supportive Apartments Program is a 16-bed, scattered site apartment program. The program
provides less direct supervision and support than youth receive in the Rights of Passage program,
and it teaches greater independence by encouraging youth to use resources available in the
community. Working with a case manager, youth do all the things that anyone who lives
independently must do including holding a job, maintaining an apartment, meal planning and
preparation, and adhering to a budget.

Rising Tide is an 18-month transitional housing program for emancipated youth from foster care
between the ages of 18-21. The purpose of the program is to provide motivated young people
with 18-24 months of affordable apartment housing, job placement, educational opportunities,
and support services to help them make a successful transition to independence. An on-site
residential counselor provides guidance, advice, and counseling and is there to intervene in any
crises. Young people pay rent according to their financial ability.
Rising Tide residents:









Pay a reduced rent for a furnished apartment shared with a roommate.
Experience an independent living environment with continued support from
Rising Tide’s staff and volunteers.
Have a mentor for support and to assist with the development of
independent living skills.
Meet with staff on a regular basis to monitor progress in the program.
Attend group meetings once a week for training and to discuss program
issues.
Have opportunities to receive incentives for program participation.
Attend weekend team-building retreats and activities.
Experience the opportunity to work and retain long-term employment.
Are encouraged to attend college or trade school.
Orangewood Children's Foundation uses two residential apartment complexes for this program,
Flanders Pointe in the city of Tustin, and Orange Tree in Garden Grove, both in the Los Angeles
area. A number of apartments are set aside from the general public units for use by the youth. An
onsite residential counselor provides structured guidance, advice, and counseling sessions on a
variety of topics and plays a critical role in crisis intervention. Youth pay rent on a sliding scale,
according to their financial ability. By the end of the program, the youth move forward to life on
their own with confidence36.

Supported Housing Program (SHP), also known as Healthy Transitions Project
Provides emancipated youth from foster care with access to safe, affordable housing,
where they have the opportunity to develop and practice life skills to achieve long-term
self-sufficiency. SHP participants live in 2-bedroom apartments in the East Bay and
receive a wide range of services and support. These include financial assistance to pay
housing start-up costs, monthly rental subsidies, weekly in-home case management,
weekly life skills training, economic literacy training, transportation assistance, monthly
food vouchers, community building peer events, and health advocacy. SHP provides safe,
affordable housing to 86 youth and 32 children annually.

Emancipation Training Center (ETC)
In addition to the Supported Housing Program, First Place provides training and
assistance through its Emancipation Training Center. The mission of the Emancipation
Training Center is to prepare youth for emancipation and support youth after discharge
from foster care. Through the ETC, 500 youth annually access education, housing, and
employment resources. Services include therapeutic case management, emancipation
planning, housing search assistance, emergency food vouchers, emergency utility
assistance, computers, recreational activities, and educational resources.

Guerrero House46
Guerrero House is a transitional housing program for homeless young adults 18-23 who are often
struggling with mental health and/or substance abuse issues. We provide food, safe shelter, case
management, and money management in a community setting where residents can learn living
skills while preparing for independent living. Case managers make referrals to vocational and
academic training, health and human resources, mental health services (including substance
abuse treatment), employment opportunities, and housing. Maximum length of stay is 18 months
and the capacity is 20 residents.

Larkin Street Youth Center Transitional Living Program47
The Lark-Inn for Youth is San Francisco’s first full-service facility for older homeless youth
ages 18-24 providing emergency shelter where young adults can stabilize their lives. It is an
entry point to any other LSYS programs they need, including case management, health care
(including mental health), substance abuse, and HIV services, as well as education, technology,
and employment training. As a youth-specific shelter, the Lark-Inn is able to meet the specific
developmental needs of young people transitioning from adolescence to adulthood. Staff
members balance compassion and accountability to develop trusting relationships with youth
while encouraging them to reach their full potential.

The Diamond Youth Shelter provides emergency overnight shelter for young people ages
12-17.

The LOFT (Larkin Opportunities for Transition) is a licensed transitional-living facility
designed to meet the unique needs of underage homeless and runaway youth.

The Ellis Street Apartments supply permanent housing, coupled with an array of support
services, including six units specifically reserved for youth diagnosed with HIV/AIDS.

LEASE is a supportive residential program for youth who have emancipated from San
Francisco's foster care system. The program uses scattered-site apartments, and
participants are linked to a range of supportive services, including employment,
education, and life skills training services.

Avenues to Independence is a unique transitional living program for young adults, ages
18 to 23. The program is designed to assist youth in making a successful transition to
independent life by offering housing for up to 18 months, education, career counseling,

Home Energy Assistance Program (HEAP)
Previous recipients of the California Low Income Home Energy Assistance Program are
encouraged to reapply if they still qualify. Eligibility for this program is based on monthly gross
household income and household size (see financial criteria, below). For an application, or more
information, please call HEAP at 971-2789, or visit the City of Santa Clara Municipal Services
Office.
Residential Rate Assistance Program (RAP)
Silicon Valley Power, Santa Clara's municipal electric utility, also offers a Rate Assistance
Program (RAP). RAP provides a 25% discount off the electric portion of the municipal utilities
bill to residents who qualify through income or medical need. Note: Those who use this program
must reapply and qualify every two years. There are two criteria for participation:
1. Financial criteria for RAP - You may apply for the 25% discount on the electric portion
of your bill if your combined income is below the limits outlined below. To apply or to

The Redevelopment Agency, Human Resources Agency, and the Santa Cruz Community
Counseling Center have partnered to provide support for a Transitional Housing Program for
Young Adults Aged 18-21 who have aged out of child welfare and foster care services. The
program is administered out of a four-bedroom house and small one-bedroom apartment on
Chanticleer Avenue that is owned by the Redevelopment Agency. There is an on-site counselor
who is housed in the apartment. The Redevelopment Agency subsidizes the housing by
foregoing market rent on both housing units and accepting instead half the previous rent revenue.
This is funded out of the Agency's Low and Moderate Income Housing Fund.

completed the six-month program and are either going to school and working part-time, or
working full-time, or have some type of income such as SSI or Social Security benefits.

Family Center serves the spiritual, emotional, and physical needs of single women and women
with children. It also incorporates the six-month minimum S.T.E.P. program.
Shelter of Hope provides emergency and transitional services for homeless single women and

with children

3 meals daily, as well as chapel services in the evening

