This paper sets out the relevant and material interests of the members of the North Hampshire CCG Shadow
Governing Body. It represents the Register of Interests as required by the Standing Orders in accordance with the NHS Code of Accountability.

1.1 The NHS Code of Accountability requires North Hampshire CCG Shadow
Governing Body members and other Committee members to declare interests which are relevant and material to the North Hampshire CCG Shadow
Governing Body of which they are a member. All existing North Hampshire
CCG Shadow Governing Body members should declare such interests. Any
North Hampshire CCG Shadow Governing Body members appointed subsequently should do so on appointment.
1.2 Interests which should be regarded as "relevant and material" are:
Directorships, including non-executive directorships held in private companies or PLCs (with the exception of those of dormant companies);
Ownership or part-ownership of private companies, businesses or consultancies likely or possibly seeking to do business with the NHS;
Majority or controlling shareholdings in organisations likely or possibly seeking to do business with the NHS;
A position of authority in a charity or voluntary organisation in the field of health and social care;
Any connection with a voluntary or other organisation contracting for NHS services;
Research funding/grants that may be received by an individual or their department;
Interests in pooled funds that are under separate management (any relevant company included in this fund that has a potential relationship with the PCT must be declared);
Practice Based Commissioning
1.3 The declaration should also include any other interests, which might reasonably be foreseen to constitute a possible conflict of interest in relation to a North Hampshire CCG Shadow Governing Body member’s position in the
Clinical Commissioning Group. This may include any other employment or consultancy arrangements of a material nature. It also embraces the interests of a spouse, long-term partner and any child under the age of 18.
1.4 Any Member of the North Hampshire CCG Shadow Governing Body who comes to know that the Clinical Commissioning Group has entered into or proposes to enter into a contract in which he or any person connected with him
(as defined in the Standing Orders) has any pecuniary interest, direct or indirect, the North Hampshire CCG Shadow Governing Body member shall declare his/her interest by giving notice in writing of such fact to the Clinical
Commissioning Group as soon as practicable.

1.5 The North Hampshire CCG Shadow Governing Body Chair and Executive
Director of North Hampshire CCG will ensure that a Register of Interests is established to record formally declarations of interests of North Hampshire
CCG Shadow Governing Body members, held by the North Hampshire CCG
Office Manager. Interests will be declared at each North Hampshire CCG
Shadow Governing Body meeting to ensure that they are known to all members.

1.6 Members of the North Hampshire CCG Board are reminded that the Standing
Orders explicitly require each member to reveal to any meeting any pecuniary or other relevant and material interest in relation to any matter under discussion or about to be discussed. If a conflict of interest is established you must withdraw from the meeting and play no part in the relevant discussion or decision, unless the Chair decides otherwise. Disclosure of such interest in the
Register in no way precludes or diminishes the need to disclose such an interest at the relevant part of any such meeting.
