We have also trained up a practice nurse to run our INR clinics which will release even more blood appointments with the HCA’s. Water Fountain – Frank thought we should look into this further, Ollie said we would bring it up at the next Business Meeting with all the partners. Art Work – We have put this ‘on hold’ at present as we haven’t yet had all the notice boards placed on the wall. Cynthia asked if the practice would be willing to hold any meetings such as the healthy living workshops etc? She attended such a course and found it very helpful but had to travel to get there. Ollie said the practice is very willing to hold such meetings / courses and currently do have smoking cessation advisors / Citizens advice and carb awareness groups up and running. Agenda Items Patient Survey - Helen has now made the alterations to the survey after suggestions were made at previous patient group meeting. Only amendment this time was the addition of ‘ Please circle all that apply’ to the beginning of survey. These will be handed out from next week at both sites. If it appears that reception don’t have time to ask patients to fill them in Cynthia, Michael and Lillian offered to come into waiting rooms and ask patients to fill them in, Fiona could help out late evening surgeries.. Helen will contact them if practice is struggling. 500 copies will be printed and hopefully by the next meeting the results will be collected and documented. Hearing Loop – Paddy asked if the practice has a heating loop? - Yes we have installed hearing loops at both sites. Follow up appointments - Paddy had pre booked a follow up appointment as requested by the GP and when she attended surgery for it she was informed that the surgery had transferred her onto another doctors list. When she was called into the room the doctor asked ‘why have you come to see me today?’ obviously Paddy suggested that she should have been informed of the transfer beforehand. Helen agreed and said that unless a doctor calls in sick or is off urgently that day then normal practice is to contact patients to inform them. Getting through via Telephone - Kath said her relative had to drive down to surgery one morning to request a visit for that day as previously they had trouble getting through on the phone. A couple of patients present said they have travelled into surgery early morning to book an appointment. Both Helen and Ollie suggested we would await the results of the patient survey as it does ask about getting through on the phone etc. Once the results are compiled this will be discussed at the partners business meeting. Confidentiality – Ron asked about confidentiality and the reception desk at LLRd. Helen said she thinks it could be a slight problem but where the desk is situated it is difficult to ask the patients to wait anywhere else except try and stand back. Some patients felt that other patients behind them in the queue would be too close. Action – Helen to organise a sign ‘on a pole’ asking patients to stand back and respect patients at the desk. Sloan Medical Centre Park Run – Ollie is trying to organise the practice to have its own ‘Park Run’, there are currently 2 already set up round Sheffield. The idea is that the practice would raise £2,500 for the set up costs of a park run and the organisation itself would match it. Once up and running the park runs are run on volunteers. The Doctors and Staff at both Little London Road and Blackstock Road are holding a sponsored 5k park run on Saturday 25th February in Graves Park at 2.30pm. Ollie asked patients present for ideas around raising money through the surgery and patients. Suggestions were a raffle for patients to purchase tickets to win prizes, ‘Spot the Ball’ or Name the Teddy Bear. For more information on the Park Runs please visit www.parkrun.org.uk Action - The practice will hold a raffle, some of the prizes being donated by patients – Frank offered a bottle of Whisky for the raffle and Michael will donate some of his paintings/drawings. We will also have a Name the Teddy game. Lots of local initiatives regarding lifestyle and healthy living etc. Michael had a good experience when referred through the prescription for exercise. Kath then went on to say the Church holds an activity morning every Tuesday at a cost of £1.00. Activities such as dodge-ball, tennis and cricket are played and coffee is included! . Previous minutes agreed. The request for drinking machines being provided in waiting rooms for patients was rejected. Confidentiality – Helen did not want to put a queuing type rope barrier at reception due to not enough space for prams etc. Helen will have a chat with receptionists for their views on how to make the reception desk at Little London road more confident. Survey results – The overall results are quite pleasing. The one area really highlighted is the access by phone to the surgery and then the lack of appointments when the patients did eventually get through. The survey results are to be taken to the next business meeting at the practice in April and any feedback or changes will be added to next patient group agenda. Helen thanked Cynthia for her help with collating the results it was greatly appreciated. Helen realised whilst collating the results that separate results should have been collected for LLrd and Blackstock as they do work slightly differently and have different opening times etc. Next years survey will be done at both sites but results collected separately. The survey results will be added to our website to be viewed and smaller copies placed on reception and in waiting rooms for patients to view. Sloan Medical Centre Park Run – Ollie has done a lot of work setting the park run up and its official now it is due to commence Saturday 14th April at 9am. The practice did a sponsored park run back in February and with the raffle (some prizes donated by patients thank you) we have raised £1,500 and Sheffield council have agreed to match our funding so we have the total amount to set up a park run. The park run will start from the car park near the animal farm and patients are very welcome to come along and either walk, jog or run the approx 5km loop. In addition to hoping to get patients jogging or walking the course we also need volunteers each Saturday. If you or you know someone who might be willing to occasionally volunteer a couple of hours on a Saturday morning then please let Ollie, Helen or Jo Eccles (receptionist) know thank you. Jodanna Gannon from Medicines Management at NHS Sheffield – Jodanna attended the meeting because NHS Sheffield are interested to hear patients views on cost effective medicines. Frank thinks that cheaper medicines are not always better for the patient. Jodanna asked what the patients understood by ‘cost effective medicines’ most thought it meant ‘cheaper’. This is to do with branded and generic medicines these are usually the same but in different packaging and sometimes the tablets look different. Maya added that her father had recently had changes to his medication and this resulted in different packaging and the tablets were a different shape and colour. Items from patients Hard of Hearing patients - Maya is part of link regarding deaf people. She thinks the surgery can do better when it comes to being aware of deaf people. Maya will give Helen the contact details for resources and training etc. Hearing Aid batteries were mentioned. These can only be collected at the Hallamshire Hospital or via a couple of doctors surgeries. Helen will contact the clinic to see if Sloan’s can be a hearing aid battery centre. Paddy asked why patients have to have an appointment with a doctor before having an ear syringe with the nurse as she felt this is a waste of time. The reason for this is that a GP must check for infections or perforated ear drums before an ear syringe. Repeat Dispensing – Ron has had problems with this and had to wait 2 weeks before he could collect the scripts. The repeat dispensing systems do cause both the doctors and receptionists numerous problems, usually practice says takes 1 week to sort a repeat dispensing request not 2. Suggestions from patients were to have a painted line that patients had to wait behind or have a queue rope system. Helen thought there wouldn’t be enough space for the patients who were in the queue with both of those options. Practice will give this some more thought. Graves Park Run – This has been running since February this year and is going well. Ollie wants to promote this again as Sloan Medical Centre is actively trying to promote physical activity within our patient population and community. Hard of Hearing – Maya asked Helen to type hard of hearing instead of deaf in any further minutes or notices etc. Problems with Repeat dispensing – At previous meeting some patients had a few slight problems with the RD service. Audrey said her latest script wasn’t signed and didn’t reach Derbyshire Lane pharmacy also some scripts not being sent to pharmacy even though it is on the patients screen. Feedback from patients - John wanted to comment that he is a fairly new patient to us and is very happy with Sloan Medical Centre, he prefers to see a doctor ‘face to face’ and doesn’t like being triaged first especially when he knows he needs to be seen. Lilian mentioned that she is really pleased with the new way we run the Anti Coagulation clinics it is much quicker and easier having a ‘one stop shop’ Chronic Disease Clinics The surgery is thinking of changing the current process for bringing patients in for their reviews when they have a chronic disease. This would be in the form of a ‘One stop shop’ chronic disease session where patients would be invited in around their birthday. We would have Healthcare assistants, practice nurses and doctors available for the patients to see. We were thinking of making it more educational in that we would open up the meeting room and have various healthcare professionals and services relating to chronic diseases and health & activity etc patients could in between their appointments have a look round various ‘stalls’, pick up leaflets or have a chat to these people. To Conclude the idea is that patients are sent a letter inviting them in to make themselves available for a couple of hours, they would see a healthcare assistant for bp, height, weight, bloods etc then go in and see the nurse who specialises in the chronic disease/s they suffer from then wait and see a doctor. The surgery will pilot this sometime in October with around 25 patients. Helen asked if any group members would volunteer to provide refreshments for these chronic disease days - John and Cynthia both volunteered (depending on the date). Ollie asked if any members of the patient group would be prepared to become ‘practice ambassadors’? or do they know anyone who may want to volunteer? Clare asked if we held ‘well man clinics’ – The practice hasn’t had well man clinics for some time now. Buddy system The surgery is also thinking about setting up a ‘buddy system’ where patients suffering with the same chronic disease can get together, chat and support each other. Cynthia has heard of good examples of this working really well. John is a carer for his partner and finds it’s difficult to find information about what he can do in certain situations. Carers support group is a good source of social chatter but not for actual facts about various illnesses etc. Audrey suggested that when a doctor sees a patient who has a condition and they feel they would benefit from either being a buddy or getting a buddy organised they could create a list. Items from patients Cancelling appointments - Ann had recently used our Text to cancel service which unfortunately didn’t go to plan as she received a DNA letter a few days later. Ann had used the service during the evening and Helen thinks that the receptionist on that desk the following morning must have forgotten to cancel the appointment. Action - Helen apologised to Ann and will speak to the Reception Supervisor at Little London Road to remind receptionists to check the mobile phone and to cancel the appointments from the screen. DNA Rates – These figures on notices around the surgery are very high, all patients present agreed this is a major problem and wondered how high non attendees could be made aware that they are blocking someone else being able to see a doctor or nurse. Helen mentioned the ways in which appointments can be cancelled without trying to get through to surgery by phone: 1. 2. 3. 4. Text to cancel service can be used 24 hours a day E Mail reception on their e mail address : sloan.reception@nhs.net 24 hrs a day On-Line with the patients username and password again can be used 24 hrs a day Obviously if a patient doesn’t have access to the above they can ring the surgery and during an afternoon is slightly less busy on phone lines. Action - Ollie suggested the DNA notices next month should show how many patients attended appointments and not how many didn’t, reverse psychology it could have an effect, Helen will try this for the September figures. Advertising within the surgery – Maya suggested the ‘old’ neon type digital display we used to have at Chesterfield Road, this would be a good way of informing patients of any news or info the practice want to notify patients of etc. Action – This will go on the agenda for the next Partners Business Meeting in October. Items from Practice Physical Activity classes etc – Sheffield International Venues are loaning us one of their physical activity & health advisors for 6 weeks starting Monday 5th November her name is Jean and she will be doing some gentle exercise classes in the meeting room at the surgery, she will be able to ring patients and invite them in to discuss a healthier lifestyle etc. ‘One Stop Shop ‘ pilot clinic The Practice is pleased to announce to the patient group that we have managed to organise a new type of clinic where patients with chronic diseases can have their annual review and get advice and education on how to control their health. The patient will be sent a letter asking them to attend surgery for a blood test at their convenience prior to the afternoon; they will then be asked to attend on the afternoon of Tuesday 6th November along with either a close family member or their carer. They will be able to see a healthcare assistant for height, weight and blood pressure check; they will also be able to see a practice nurse and a GP. Whilst they are in the surgery they will be able to speak to various service providers who the surgery have invited along to give its patients advice and help in many areas such as citizens advice, WRVS they have a (befriending service) we hope this will also encourage patients to sit and chat with other patients who suffer with the same condition whilst having a light lunch. We are asking for volunteers to help ‘buddy’ patients invited to this clinic. Finally patients will be asked to fill in a short satisfaction survey. Patients present were asked what we should call this clinic. Suggestions were - Be in control of your health, Manage your health and Positive health day. Helen and Ollie liked the sound of Positive Health Day. Maya, Ron volunteered to help on 6th November (Cynthia had already expressed an interest in being a volunteer).
