The once off establishment charge is per Optus Evolve IP VPN
service interface

The establishment charge covers the cost of installation of the
physical access and set-up of your service interface connection to
the service

The establishment charge is in addition to any non-standard
establishment charges for Direct Fibre based Access services as may
be quoted from time to time.
Flat Monthly Recurring Charges for Primary Access Services
We will only supply an individual service at the published rates in this
standard pricing table if you choose a minimum term of at least twelve
months
The prices set out in Table 2 below apply to customers who acquired the
service up to and including 23 September 2009.

2.3
Optus Evolve IP VPN Flat Rate Charge (Secondary Access Link)
Optus Evolve IP VPN Services that are designed and used as a secondary
access link for redundancy purposes in accordance with the Optus Evolve IP
VPN service description, will be charged at a discounted rate to the Optus
Evolve IP VPN charges set out above in this standard pricing table or in
your application. The level of discount will be determined by us in our sole
discretion and communicated to you by us during the contracting process.

3.1

The Optus Evolve MRS establishment charges set out below is in
addition to any establishment charges payable by you in respect of
to an Optus Evolve IP VPN service.

Router hardware procurement, installation and commissioning used
for the Optus Evolve MRS service.

Moves, Adds, Changes
(a)
Moves, Adds and Changes (MACs) only apply to existing
customers. MACs are defined as either simple or complex.
(b)
Changes or minor network reconfigurations are known as simple
MACs. A simple MAC is defined as any customer edge router
configuration change that can be executed in less than 2 working
hours and that does not involve the addition or relocation of OPI
MRS Services to a new or existing premise, and can be executed
without the need to dispatch an Engineer to site. Simple MACs are
charged @ $200 per hour (during Business Hours). A 50% uplift
will apply to all work outside Business Hours.
(c)
A complex MAC is defined as any customer edge router
configuration change that is executed in greater than 2 working
hours or requires a field engineer to be dispatched to site. Adding
equipment is considered to be a complex MAC and will be charged
as per a new installation. Moving / relocating equipment to new or
existing premises is considered a complex MAC and will be charged
as per a new installation. (Optus Evolve IP VPN establishment
charges)
(d)
Relocate includes both relocations within a building and relocations
to another building.
(e)
We reserve the right to decide if a MAC is simple or complex and we
also reserve the right to treat a series of simple MACS as a complex
MAC.

OPTUS EVOLVE SSL REMOTE ACCESS PRICING SCHEDULE
4.1
Establishment charges
The Optus Evolve SSL Remote Access establishment charge involves the set
up of the customer, the setting up of reporting on the eCare web site and the
initial set up of user accounts on the radius server.

Moves, Adds, Changes
(a)
Any MACs performed by you online are non-chargeable. However any MACs performed by our personnel will attract a charge
equivalent to the establishment charge.
(b)
Where the customer upgrades/downgrades a service as part of a MAC, the applicable Optus Evolve establishment charge is
payable, in addition to any Downgrade Charges.
COPY OF BACK BILLS
You may request a copy of your bill from a previous billing period. We may charge you for the copy of the bill at the rate of $55
inclusive of GST per copy.

PAYMENT DISHONOUR CHARGES
(a)
If you elect to pay your bills by cheque we may charge you a dishonour fee of $22 if the payment is dishonoured by your
nominated financial institution.
(b)
This fee is in addition to any fees that your financial institution may charge you.
8
CANCELLATION FEES AND DOWNGRADE CHARGES
8.1
Cancellation fee
The cancellation fee is calculated in accordance with the following table:

service interface) for the remainder of the minimum term from the date
cancellation takes effect.
In addition if you receive Optus Evolve MRS service and have elected
to pay the 'establishment charge' set out in the standard pricing table
on a monthly basis, then you will also be required to pay an amount
equal to the unpaid balance of that 'establishment charge' determined
as at the date that the cancellation takes effect.
More than 12 months Cancellation during first 12 months

100% of all amounts that would have been payable in respect
of the recurring monthly charge (including any charge in
respect of the service interface) for the period between the date
cancellation takes effect and 12 months after the service start
date; and

20% of all amounts that would have been payable in respect of
the recurring monthly charge (including any charge in respect
of the service interface) for the period between 12 months after
the service start date and the expiry of the minimum term; or
Cancellation after first 12 months

8.2
20% of all amounts that would have been payable in respect of
the recurring monthly charge (including any charge in respect
of the service interface) for the period between the date
cancellation takes effect and the expiry of the minimum term.
Downgrade charges
If you downgrade:

(a)
Before the first anniversary of the service start date, the downgrade charge will be the amount notified by us which may be an
amount up to, but not exceeding, 100% of the recurring monthly charge (including any charge in respect of the service
interface) for the period from the downgrade until the first anniversary of the service start date; or
(b)
After the first anniversary of the service start date, the downgrade charge will be nil.
