Complete all of the following items, and submit this document together with the manuscript.

□ Are you certain that this is not a duplicate submission? (in any language, including manuscripts currently in the
submission process)
□ Have you sufficiently discussed the content of the manuscript with all coauthors, and obtained their consent to
submit it?
□ When using the figure(s) and/or table(s), have you obtained permission from the copyright holder(s) before
submission.
□ When submission is made in English, have you had the manuscript checked by a native speaker of English, or
someone who is competent at writing papers in English?
□ Does the cover sheet contain: (1) the title, (2) authors’ names, (3) affiliated body’s official name, (4) corresponding
author’s name, affiliated body’s address, phone number, and e-mail address, (5) keywords (about five), and (6)
marginal heading? When submission is made in Japanese, is each item except (4) accompanied by a description in
