Transferees to Academic Programs for Full-time Students 1 Definition: A transferee to the academic programs for full-time students is classified as a Day Program transfer applicant. The Cumulative Grade Point Average (CGPA) of subjects taken in tertiary level institutions attended is a control variable. [Please be informed that an applicant from a non-La Salle tertiary school must have a satisfactory CGPA. Failure to meet the aforementioned CGPA will automatically disqualify the applicant from applying to the Day Program. [(Graduates of the Philippine School of Interior Design (PSID) are given a reconsideration for application, but only to the BS-Interior Design Program.)] Pre-computation of CGPA should use the CSB grading system: 4.0 (97-100%) 2.0 (80-84%) 3.5 (93-96%) 1.5 (75-79%) 3.0 (89-92%) 1.0 (70-74%) 2.5 (85-88%) 0.0 Below 70% Grades of Withdrawal-Failed (WF), Unofficially Dropped (UD), Unauthorized Withdrawal (UW), Absence Without Official Leave (AWOL), Failure due to Absence (FA) are considered failures, hence will be given a grade of 0.0. A grade of Incomplete (INC) is treated as a Withdrawal with Permission (WP) and is therefore not included in the computation of the Cumulative Grade Point Average (CGPA).] 2 Procedure 2.1 Students who have enrolled in other tertiary institutions here or abroad may not apply as freshmen. They may inquire at the Admissions Center at the start of the sixth week of each trimester (first week of July and third week of October), but not later than the ninth week, regarding acceptance to the College for the second and third trimester of the current school year. For enrollment in the first trimester of the next school year, inquire on the sixth week of the third trimester of the current school year (February) but not later than the ninth week. 2.2 The applicant must submit a certified true copy of his/her transcript of records, taken from other institutions. 2.3 The applicant should inquire at the Admissions Center after completing the aforementioned activity regarding his/her eligibility to apply. After completing the first stage, the pre-application assessment (pre-application forms are available at the Admissions Center) of the applicant’s documents by the Admissions Director, the Admissions Center staff will issue to the first-stage qualified applicants an application payment slip for the nonrefundable processing fee, to be paid at the Accounting Office. The applicant must then present the official receipt to the Admission staff and obtain an Application Form. After submission of the completed Application Form with all the required documents to the CSB Admissions Center, the applicant will be issued an entrance examination permit by the Admissions staff. He/She may then take the entrance examination on the scheduled date. 2.4 Admission is based on past academic performance, College entrance test results, personality attributes, pre-test and/or post-test interviews with the Academic Deans/Chairpersons/Counselors and/or Admissions Director, and relative level of maturity. The Executive Vice President reserves the right to disapprove an applicant’s request for admission into any program. 2.5 The applicant should inquire about the status of his/her application at the Admissions Center on the date specified in the Guidelines on Application of Transferees. 2.6 If accepted, the respective Academic Deans, in coordination with the Registrar, determine the credits granted for units or subjects taken in the applicant’s previous school(s). The Transfer Credential document [a Certified Updated Unofficial Transcript of Records (TOR) and the Certificate of Transfer Credentials (Honorable Dismissal)] from the last school attended is required of accepted applicants. (Note: Should the grades in the TOR and records and/or statements previously submitted and/or signed by the student differ from the Official Transcript which is to be forwarded to the DLS-CSB Registrar by the school, the former shall automatically nullify the student’s application and/or dismiss him/her from the College.) 3 Requirements 3.1 Certified unofficial transcript of records (for evaluation purposes) 3.2 For applicants who had no more than one year of tertiary schooling: 3.2.1. Photocopy of first year to fourth year high school report cards, duly certified by the high school registrar. (Refer also to 3.3 under freshman requirements, if applicable) 3.2.2Two CSB recommendation forms (Refer to 3.4 under freshman requirements) 3.3 Certification from the Dean or Registrar of the tertiary school attended that the applicant was never involved in any case/offense which called for disciplinary sanction(s). 3.4 Certification from the Dean or Registrar of the tertiary schools attended that the applicant was not dismissed from the school for disciplinary reason(s). (Please note that certification is only for applicants who changed schools while in college.) 3.5 Accomplished “Request for Course Credit for Transferees Form” (RCCTF) (Attach the Flowchart and the course syllabus of subjects taken). 3.6 Three (3) recent identical 2”x 2” photographs (taken within the last six months) 3.7 Photocopy of birth certificate 3.8 For foreign students and for Filipinos who graduated or studied in schools overseas: a photocopy of an authentication of records from the Philippine Embassy in the relevant countries of schooling (present original for verification) 3.9 Requirements for Non-Filipino permanent residents, Dual Citizenship, or Tourist/ Balikbayan Visa holders: Please refer to the additional documents/information listed in the Foreign Students section, page 7 A student visa is required of accepted foreign applicants as a pre-requisite for confirming their enrollment. Working Students applying to any of the Night Programs 1 Definition A Night Program transfer applicant is a high school graduate who may or may not have enrolled in any tertiary institution here or abroad; is working at least part time; and is at least 19 years old (age waivers maybe given by the CDP chairperson for meritorious cases.) The Cumulative Grade Point Average (CGPA) is not a control variable. 2 Procedure An applicant to the Night Program should follow the same admission procedure as an applicant to the Day Program. In addition, the applicant will be scheduled for a post-test interview with the Career Development Program (CDP) guidance counselor and/or Department Chairperson where necessary. 3 Requirements Requirements listed for full-time students, as well as a Certification of Employment must be submitted to the Admissions Staff. Further proof or evidence of actual employment may be required (e.g. Tax Identification No., SSS or GSIS no., copy of the latest Income Tax Return). Second Degree Applicants Applicants seeking a second degree may apply for screening and present the following: 1 Letter of application stating the reason(s) for seeking admission to the Program 2 Photocopy of the official transcript of records (original must be presented to the Admissions staff for verification) If the applicant is allowed to pursue application, the procedure and requirements outlined in the Transferee section on page 5 applies. Foreign Students* The documents/information listed below are required, and are to be submitted to the DLS-CSB Admissions Center (2544 Taft Avenue, Manila, Philippines) upon application. FOREIGNERS WITH VISAS OTHER THAN TOURIST/BALIKBAYAN VISAS: 1. Photocopy of photo, data and stamp of latest arrival page of the passport of the student. 2. Photocopy of the updated Alien Certificate of Registration (or a recognition and ID for Non-Native Dual Citizens only). 3. Photocopy of updated SRRV ID. 4. Photocopy of Immigrant Certificate of Residence (ICR). (All original documents of #s 1 to 4 should also be presented for verification.) 5. One (1) original Transcript of Records authenticated by the Philippine Foreign Service Post. If these records are written in a language other than English, complete and official English translations must be submitted together with the original records. The transcript should indicate all subjects taken and grades earned. The grading system should be clearly stated in the transcript. 6. Certificate of Good Moral Character from the Principal or school head of the last school attended. TOURIST/BALIKBAYAN VISA HOLDERS 1. Photocopy of the photo, data and stamp of the latest arrival pages of the applicant’s passport. The passport should be presented for verification. 2. Two (2) original Transcripts of Records authenticated by the Philippine Foreign Service Post. (One will be used for the school files for securing the student visa, and the latter is to be submitted later, prior to confirmation of enrollment, if accepted.) If these records are written in a language other than English, complete and official English translations must be submitted together with the records. The transcript should indicate all subjects taken and grades earned. The grading system should be clearly stated in the transcript. 3. Certificate of Good Moral Character from the Principal or school head of last school attended. To facilitate the application for a Student Visa, please be informed that the following are to be submitted by accepted applicants to the CSB Foreign Liaison Officer (Office of the Registrar, 7/F CSB International Conference Center) prior to confirmation of enrollment: 4. Two (2) copies of the Personal History Statement (PHS) forms duly accomplished by the applicants. (These forms must be filled out in the applicant’s handwriting.) 5. A notarized proof of adequate financial support to cover expenses for the student’s accommodation and subsistence, as well as school dues and other incidental expenses. 6. Police Clearance Certificate issued by the national police authorities in the student’s country of origin or residence, duly authenticated by the Philippine Foreign Service Post having consular jurisdiction over the place of origin for a student who has been residing in the Philippines for less than 59 days. However, a student applicant who has been residing in the Philippines for more than 59 days at the time he applies for the said change/conversion of his admission status to that of a student shall be required to submit the National Bureau of Investigation (NBI) Clearance. 7. Quarantine Medical Clearance Certificate by the Philippine National Quarantine Office. Please note that Tourist/Balikbayan Visa Holder applicants, IF ACCEPTED, may choose to apply for their student visa through the Department of Foreign Affairs (DFA) or the Bureau of Immigration and Deportation (BID). Should an applicant opt to personally attend to his/her application for student visa, he/she should inform the College Registrar of his/her decision in writing. He/She will then be issued a letter of endorsement to the government agency concerned (DFA/BID). a. Department of Foreign Affairs Processing and Issuance will be at the student’s country of origin. Processing Fee: US $100 Processing Period: 2 to 3 weeks b. Bureau of Immigration and Deportation Processing and Issuance will be at the BID Manila Office. Processing Fee: US $300 Processing Period: 2 weeks DLS - College of Saint Benilde observes a trimestral academic calendar: the first trimester starts in late May, the second in early September, and the third in early January. Please note that the schedule of the aforementioned agencies for submission of applications for student visas is as follows: First Trimester - January to March Second and third trimesters - June to August Important Reminder: For each succeeding term enrollment, a photocopy of annual updates of foreign required documents is to be submitted to the CSB Foreign Liaison Officer. *Students who present any of the following documents upon admission shall not be considered foreign students, and therefore shall not be required to submit the aforementioned requirements: 1.
