CHAPTER 5
RESERVE OFFICER PROGRAMS PROCESSING
SECTION 1 – APPLICATION PREPARATION
050101. OVERVIEW. This section provides specific guidance for
preparing an officer programs application for all Reserve
Officer Programs.
050102.
RESPONSIBILITIES
a. Unless specifically directed otherwise by the Officer
Programs Officer (OPO) or personnel with “by direction”
authority, the Officer Processing Lead (OPL) shall:
(1) Ensure that all officer application paperwork is
managed, monitored and completed by the applicant and the
Processing Division within the Officer Programs Department.
Unless specifically directed otherwise by the OPO, officer
recruiters (ORs) primary duties are to recruit applicants and
Officer Processors (OPs) primary duties are to process the
applications.
(2) Conduct a thorough review of each application to be
forwarded for selection consideration.
(3) Ensure at a minimum the following:
(a) All required application documents are complete,
accurate and included as listed in the appropriate checklist.
(b) Any discrepancies between documents are resolved
prior to submission to COMNAVCRUITCOM.
(c) The OR and OPO have completed their review and
made all necessary comments on the Application Processing and
Summary Record (NAVCRUIT 1131/1).
b. Prior to submitting an application to COMNAVCRUITCOM,
the OP, the OR, the OPL and the OPO or personnel with “By
direction” authority, shall thoroughly review the file for
completeness and accuracy. Each individual reviewing the
application shall make any necessary comments in the

appropriate sections on the Application Processing and Summary
Record (NAVCRUIT 1131/1), sign and date where indicated,
confirming their individual review.
c. During the course of the review, the OR shall evaluate
the information in the application, and using this information
and his/her personal knowledge of the applicant, make an initial
recommendation of the applicant's suitability for appointment.
The OR shall record in writing his/her evaluation and
recommendation on the Application Processing and Summary Record
along with all other items of information required therein.
d. When the complete application is forwarded to the OPO or
XO/CO for signature (or personnel with “by direction”
authority), it carries the OPO’s, OR’s and OPL/OP’s implicit
assurance, by virtue of the initialed application checklist and
written evaluations, that the application is in compliance with
policy and processing regulations, and that the suitability
recommendation is consistent with the factual material therein.

050104. APPLICATION PROCESSING AND SUMMARY RECORD. The
Application Processing and Summary Record (NAVCRUIT 1131/1)
shall be completed prior to submission for all applicants.

050105.
APPLICATION FOR COMMISSION
a. The Application for Commission or Warrant Rank, U. S.
Navy or Navy Reserve (NAVCRUIT 1131/2) shall be completed by all
applicants prior to submission. Instructions for completing
this form can be found in the status bar within the MS Word ®
document.

b. Completion of this form is self-explanatory; however,
dependent upon the applicant's answers to individual questions,
additional documentation may be required.
050106. MOTIVATIONAL STATEMENT. Section VIII, Motivational
Statement, of the Application for Commission (NAVCRUIT 1131/2)
shall be completed by all applicants.
050107. ELECTRONIC PERSONNEL SECURITY QUESTIONNAIRE.
shall be completed in accordance with Article 050201.

050108. FINGERPRINT CHARTS. Fingerprints shall be submitted in
accordance with Article 050201.
050109. SF86 AGENCY USE FORM. Information for completion of
the SF86 Agency Use Form is contained in the SECNAV 5510 series
instruction. See Article 050201 for further instructions.

RESUME.

050111. CURRICULUM VITAE. Applicants for all Direct Commission
and Inter-service Transfer Programs shall provide a signed, and
dated curriculum vitae (resume).
050112.
TRANSCRIPTS
a. Accredited Institutions. Officer candidates for all
programs must, prior to applying for commission, possess a
baccalaureate degree from a college or university that is listed
in the Directory of Accredited Institutions of Post Secondary
Education or the Higher Education Directory, published annually
by Higher Education Publishing Incorporated. The college or
university must be accredited by the New York State Board of
Regents or the agencies listed above. Exceptions to this will
be reviewed on a case-by-case basis by COMNAVCRUITCOM.
Completion of all work for a degree at such institutions,
together with written certification by the college/university
that the degree is to be conferred later, is also acceptable.
b.
Non-accredited Institutions
(1) Officer program applicants from colleges/
universities not accredited by one of the seven regional
accrediting agencies may be considered for Navy officer programs
if the non-accredited institution's credits are fully accepted
by at least three regionally accredited institutions. The

following example is an acceptable statement that may be signed
by the registrar, admissions officer, or any other appropriate
official of a regionally accredited institution and must bear
the school seal:
"This is to certify that (name of regionally
accredited institution) accepts all credits by transfer
from (name or non-accredited institution) in the same
manner that it accepts credits from institutions accredited
by regional associations. (Name of regionally accredited
institution) also admits graduates of (name of nonaccredited institution) to graduate study if they meet the
same standards that graduates of regionally accredited
colleges/universities meet."
(2) On an individual basis, an applicant who has
received a baccalaureate degree from a non-accredited
college/university may be processed if he/she has been accepted
by a regionally accredited institution as a candidate for a
master's or higher-level program of study.
c. Graduates of Foreign Institutions. Foreign college or
university graduate applicants must furnish an official
translated transcript indicating that he/she has been awarded a
baccalaureate degree. The degree must be certified in writing
by an international consulting firm, the NAVCRUITDIST Education
Specialist, or a COMNAVCRUITCOM Education Specialists
(COMNAVCRUITCOM N35). A written statement regarding
accreditation of the foreign institution compared to a degree
granted by an accredited college and university in the United
States is required. Only if the certification is positive will
the application be submitted. Questions concerning
international consulting firms shall be directed to the
COMNAVCRUITCOM (N35). See Article 050304 for translation of
foreign documents.
d. Official Transcripts. A transcript is considered
"official" only if issued by the original source, sent directly
from the educational institution in question, and clearly
identified as being an official document of that institution;
i.e., marked by a raised or colored seal of the institution of
higher learning. For all medical student and MSC programs, see
Article 060160.
e. All transcripts shall be official transcripts.
Transcripts stamped "issued to students" are not acceptable
unless they have been sent directly from the school to the

NAVCRUITDIST in a sealed envelope. In this case, the OR shall
annotate on the Application Processing and Summary Record
(NAVCRUIT 1131/1) the process by which the transcripts were
obtained.
f. Transcripts will be submitted for each post secondary
institution the applicant has attended.
g. Applicants for Medical programs (less students) shall
include a statement regarding their participation in continuing
education and statements from appropriate training program
officials regarding the completion of professional or
certification training.
h. All Dental Corps applicants shall provide a certified
copy of their dental school diploma.
050113.
PROFESSIONAL CERTIFICATES/CERTIFICATION
a. If an applicant has earned a professional certification
or license, a copy of the appropriate document shall be included
in the application package. Examples include:
(1) Engineer in Training (EIT)
(2) Professional Engineer (PE)
(3) Graduate Medical Education Certificates
(Internship, Residency, Fellowship)
(4) Current Medical Professional licenses (certified)
(5) Medical Specialty Certification
(6) Foreign Medical Graduate Certificate
(ECFMG/FMGEMS)
(7) Nursing License (certified) (all licenses ever
held).
(8) Certificate of Ordination
(9) National Commission on Certification of Physician
Assistant
(10) Dental Specialty: Specialty Board Certificate
(11) JAG: Proof of Good Standing with BAR
(12) MSC Clinical Psychology:

b. Where reproduction of the certification or license is
prohibited, a statement that the certification has been sighted
by the recruiter shall be included in the Application Processing
and Summary Record (NAVCRUIT 1131/1).
050114.
INTERVIEWS
a. The primary objective of the professional interview is
to determine the applicant’s potential as a naval officer. The
interview has secondary functions to include:
(1) Providing an opportunity to inform the applicant of
the details of the program for which he/she is applying;
(2) Informing the applicant about the training he/she
will receive; and
(3) Providing the applicant an opportunity to answer
questions.
b. The number of interviews required varies among programs.
The application checklist shall be reviewed to ensure the
correct procedures are followed.
c. All interviews shall be completed by an officer holding
a paygrade of O-3 or senior.
d. One of the interviewers shall be from the officer
designator of the program for which an application is being
made. In the case of MSC, one interview shall be with an
officer of the same sub-specialty.
e. Interviews may be performed by either USNR or USN
officers.
f. Personnel assigned to recruiting duty in the Officer
Programs Department are not authorized to interview reserve
officer program applicants.
g. In no case shall a reserve applicant for a reserve
officer program be interviewed by other than as indicated above.
h. In no case shall an interview appraisal form be prepared
based on input, either written or oral, from another source.
i.
Regardless of the interview method chosen, all interview

appraisals submitted with an application shall be the result of
either close association (the one informal interview allowable)
or of formal observation (face-to-face personal contact).
j.
Interview Procedures
(1) As mutually agreed upon prior to the interview,
either the senior interviewing officer or the individual officer
most closely associated with the applicant, shall ensure that
during the course of the interview, the applicant is advised of
the following:
(a) The contents of the applicable service
agreement.
(b) The duration and type of training to be received
to include the mandatory Physical Fitness Assessment (PFA)
requirements.
(c) That after processing and approval, the
application will be forwarded to COMNAVCRUITCOM for final
action.
(d) That written notification to the NAVCRUITDIST is
required for withdrawal of an application or declination of
selection, stating the reasons for the request.
(e) That marginal applicants should be prepared for
possible rejection.
(2) The interview room must be comfortable and provide
sufficient privacy.
(3) Interviewers shall have information concerning the
applicant's background, test scores and physical qualifications
prior to the interview. The application folder of each
applicant shall be furnished to the interviewers for review
prior to commencement of the interview.
k.
Recording of Interview Results
(1) Written interview results shall be made using the
Interviewer's Appraisal Sheet (NAVCRUIT 1131/5). Interview
results shall not be recorded during the course of the
interview. However, each interviewer shall evaluate carefully
and impartially the characteristics of the applicant and record

evaluations while the details of the interview are still fresh
in his/her mind.
(2) Any applicant who speaks English as a second
language shall complete a portion of the interview
telephonically to access the applicant's level of proficiency
and clarity using English as their primary means of
communication. The interviewer's comments shall include a
statement as to the applicant's ability to communicate in
English.
(3) Appraisals legibly handwritten in ink are
acceptable, but not recommended.
050115.
a.
REFERENCES - REQUEST FOR REFERENCE
A minimum of three references are required.
b. The Request for Reference (DD Form 370) shall be used to
document employment, character, peer, professional and military
reference information.
c. The purpose of gathering personal data regarding an
applicant is to provide information about the applicant's
leadership potential, character, and integrity from sources
other than the applicant. Any unauthorized disclosure of
information provided in response to requests for personal data
not only betrays the trust that an individual has placed in the
Navy, but jeopardizes the reliability of future responses. No
applicant shall be told the nature of any reply to an inquiry,
nor shall he be told he has been disapproved for reasons of an
unfavorable investigative report.
d. Employer references. Each employer listed on the
Application for Commission or Warrant Rank, U. S. Navy or Navy
Reserve (NAVCRUIT 1131/2) and the Electronic Personnel Security
Questionnaire (EPSQ), including part-time employers, for three
years immediately preceding the date of application shall be
queried using the DD Form 370. The following guidance applies:
(1) If requested by the applicant, a DD Form 370 shall
not be forwarded to the applicant's current employer (e.g., the
applicant feels it might jeopardize their job). If the current
employer is not queried, it shall be annotated on the
Application Processing and Summary Record (NAVCRUIT 1131/1).

(2) Officer recruiting processors shall complete the
front of the employment reference and mail it to the appropriate
employer.
e. Military References. The Commanding Officer of all
applicants who are currently serving on inactive duty in any
branch of the Armed Forces shall be requested to provide a
reference.
f. Peer References. Three peer references are required for
officer applicants applying for the Chaplain program. Peer
references are also required in conjunction with the Healthcare
Professional Qualification Summary (HPQS) as indicated. HPQS
peer references are used to assess the applicant’s clinical
skills.
g. Character Reference. Only when employer references are
not available will the applicant be required to submit character
references.
h. Congressional/Political References. Applicants
inquiring about such endorsements or reference should be advised
that they are not necessary and will be favorably considered by
the selection board only when the individual providing the
reference has had personal knowledge of the applicant prior to
the time of application.

HEALTH PROFESSIONAL QUALIFICATIONS SUMMARY
a. Health Care Providers deal in direct patient care and
include the following:

b. As part of the application process, recruiters verify
license/registration with state and/or national licensing boards
and forward the Health Professional Qualifications Summary
(NAVCRUIT 1131/11) as part of the application.

050117. LETTER OF GOOD STANDING. A letter of good standing
with the school/residency program is required for 2105 and 2205
applicants who are currently enrolled in a residency program.
050118. LICENSE. Certified copies of licenses are required for
officer applicants applying for any direct appointment to
Medical Programs (2105, 2205, 2305 and 2905), JAG or Chaplain
program.
050119. LICENSE VERIFICATION FOR MEDICAL PROGRAMS APPLICANTS.
The licensing of all health care provider applicants shall be
verified with the appropriate state/national licensing board
using the Licensure Verification for Medical Programs Applicants
(NAVCRUIT 1131/13). All Nurse Corps applications shall include
a report of inquiry with a licensing agency from all states in
which the applicant has ever held a license.
050120. VERIFICATION OF NURSING EDUCATION. All Nurse Corps
applications shall include a report of inquiry of their nursing
school(s) using the Verification Of NLN/CCNE Nursing Education
(NAVCRUIT 1131/17). This information can be located on the
internet at the NLN and CCNE websites.
050121. PROOF OF GOOD STANDING. JAG workforce applicants must
provide proof of good standing with their BAR.
050122. REPORT OF MEDICAL EXAMINATION. See Articles 020306,
020312, and 020313 for DD Form 2808, Report Of Medical
Examination, requirements.
050123. REPORT OF MEDICAL HISTORY. See Articles 020312 and
040801 for DD FORM 2807-1, Report of Medical History,
requirements.
050124. PRIVACY ACT - HEALTH CARE RECORDS. The Privacy Act Health Care Records, (DD Form 2005), shall be read and signed by
all officer applicants. The original will be retained in the
applicant’s residual file and a copy included in the
application.
050125. DOCUMENTATION OF REQUEST FOR ECCLESIASTICAL ENDORSEMENT
APPROVAL
a. An Ecclesiastical Endorsement (DD 2088) verifies that
the applicant is a fully qualified clergy from a recognized
religious organization; actively employed in a denominationally
approved vocation; and recommended as being spiritually,

morally, intellectually and emotionally qualified to represent
the applicant’s religious body in the Chaplaincy of the Armed
Forces. An ecclesiastical approval attests that a seminary
student is considered a potential candidate for ordination. It
is the prerogative of the individual church or religious body to
establish its own requirements. Each denomination has specific
application procedures for endorsement consistent with the
policy of that particular church body.
b. The Issuing of an Endorsement. In connection with an
application for appointment in the Chaplain Corps/Theological
Student Program, a copy of the Ecclesiastical Endorsement (DD
2088) or Documentation of Request for Ecclesiastical Endorsement
(NAVCRUIT 1131/10) shall accompany all applications forwarded by
the NAVCRUITDIST. In many instances, the application for the
Navy Chaplain Corps Program, and the request for Ecclesiastical
endorsement, does not occur simultaneously. Districts shall
forward applications if the applicant provides a completed
Documentation of Request for Ecclesiastical Endorsement
(NAVCRUIT 1131/10) indicating the date of request for
ecclesiastical endorsement and the name of the endorsing
agent/office contacted. An application will not be considered
by the selection board until the Ecclesiastical Endorsement is
received in the Office of the Chief of Chaplains. Forwarding an
application without ecclesiastical approval can extend the time
before an applicant is notified of the final selection decision.
c. Requesting Endorsement or Approval. The point of
contact in the endorsement system between individual religious
bodies and the Department of Defense is the ecclesiastical
endorsing agency. The endorsing agent is a descriptive term
used by all three services to identify the committee,
commission, board, or diocesan office authorized by each
organization and formally recognized by the Department of
Defense to issue endorsements or approvals. For information
regarding ecclesiastical endorsements, contact the Chaplain
Programs Manager, COMNAVCRUITCOM (N342).
050126. APPLICATION FOR ACCESSION FINANCIAL INCENTIVES (SPECIAL
PAY, LOAN REPAYMENT AND STIPEND) LETTER. The Application for
Accession Financial Incentives Letter is the written agreement
provided by CNRFC Incentive Shop to all eligible applicants
during affiliation.

eligible critical wartime subspecialties.
050127. MAXIMUM AGE STATEMENT OF UNDERSTANDING. All Medical
Corps (MC) and Dental Corps (DC) officers who will be beyond

their 40th birthday, and all other officers who will be beyond
their 35th birthday on the day of commissioning, are required to
acknowledge the potential limitations on their career created by
their age by reading and signing the Maximum Age Statement of
Understanding contained within the respective Service Agreement.
050128. PRIOR SERVICE RECORD. Applicants with prior or active
military service shall submit the #4 copy of their DD Form 214,
all fitness reports/evaluations, page 4’s, page 5’s, page 9’s
and any derogatory information with their officer application.
050129. CERTIFICATE OF RELEASE OR DISCHARGE FROM ACTIVE DUTY/
PRIOR SERVICE RECORD RELEASE REQUEST
a. Certificates of Release or Discharge from Active Duty
(DD Form 214)/Prior Service Record Release Request (NAVCRUIT
1070/1) are required whenever an applicant has indicated prior
active or inactive duty in another service (including State
National Guard and U.S. Public Health Service).
b. The Prior Service Record Release Request (NAVCRUIT
1070/1) shall be completed by the processor and forwarded to the
following address:

c. Copies of Certificates of Release or Discharge from
Active Duty (DD Form 214, #4 Copy) or Statement of Service from
the United States Public Health Service (USPHS), as applicable,
shall be forwarded to COMNAVCRUITCOM as directed.
050130. CONDITIONAL RELEASE. Members currently serving in
another service reserve component may be considered, provided
the individual submits a Conditional Release (DD Form 368) with
the application. Navy recruiting personnel shall not initiate
such requests. However, Navy recruiting personnel shall verify
the applicant has obtained the authorizing signature from an
authorized official.
050131. DISCLOSURE OF SOCIAL SECURITY NUMBER - PRIVACY ACT
STATEMENT. The Disclosure of Social Security Number – Privacy
Act Statement (NAVCRUIT 5211/1) shall be read and signed by all
officer applicants. The original will be retained in the

applicant’s residual file and a copy included in the
application.

050132.
VERIFICATION OF BIRTH/BIRTH CERTIFICATE
a. All applicants for officer programs shall furnish a
birth certificate or provide official verification of birth.
(1) U.S. Birth Certificate. Enclose “Certified to be a
true copy” signed and dated by the person verifying authenticity
in the application.
(2) Non-U.S. Birth Certificate. Enclose “Certified to
be a true copy” signed and dated by the person verifying
authenticity in the application along with an English
translation of birth certificate. The translation shall be
certified by the translator and shall include the translator’s
printed name, signature and date. This translation cannot be
completed by the applicant.
b. If the birth certificate cannot be produced, the Request
for Verification of Birth (DD Form 372) can be used as primary
evidence by completing the form per the instructions contained
thereon and forwarding it to one of the agencies listed below.
A copy of the DD Form 372 shall be retained in the applicant's
residual file pending response. Upon receipt of the agency
verified DD Form 372, replace the unverified copy with the
agency verified DD Form 372 in the residual file and enclose the
original in the application.
(1) State Bureau of Vital Statistics or State Health
Department.
(2) County Department of Public Health, County Clerk's
Office, or County Registrar's Office.
(3) City Department of Public Health, City Clerk's
Office, or City Registrar's Office.
(4) In the event the agency states that no birth record
exists, a combination of secondary evidence, such as a U.S.
passport (or one in which the individual was included),
baptismal certificate, certificate of circumcision, hospital
record, separation documents of prior service personnel,
affidavits of persons having personal knowledge of the facts of
birth, primary school records, family Bible entries, early
census, newspaper files or insurance papers, any of which must
have been created as close to the time of birth as possible, and
which may be used to verify the applicant's date and place of

birth shall be reviewed by the NAVCRUITDIST Commanding Officer,
or an officer designated in writing by the Commanding Officer,
for determination of enlistment eligibility. Upon verification,
the officer shall prepare a DD Form 372 and indicate in Section
II the circumstances of how the verification was made (e.g.,
"Determination based on review of hospital records and DD Form
372 by
." Such entry shall be signed and dated
by that officer in the "Verified by" block.
c. The DD Form 372 is not to be used to obtain birth record
information from the U.S. Citizenship and Immigration Services
(USCIS), formerly the Immigration and Naturalization Service
(INS).
050133.
EVIDENCE OF CITIZENSHIP
a. Applicants for all officer programs shall be citizens of
the United States and shall submit positive proof of citizenship
at the time of application.
b. Native born citizens. Applicants born in the
geographical United States (50 states, Guam, Puerto Rico, Virgin
Islands and the Northern Mariana Islands) are considered native
born citizens.
c. Applicants born outside the United States of U.S.
citizen parents. One of the following verification forms can be
sighted and a DD Form 372 completed citing circumstances of how
the verification was made.
(1) Certificate of citizenship issued by USCIS, formerly
INS. The Evidence of Citizenship (NAVCRUIT 1100/25) shall be
completed.
(2) Report of Birth Abroad to a Citizen of the United
States of America (FS 240).
(3) Certification of Birth issued by a U.S. Consulate of
the Department of State (FS 545 or DS 1350).
(4) U.S. Passport (or one in which the applicant was
included).
(5) For applicants not born in the Panama Canal Zone who
cannot provide any of the above documentation, prepare DD Form
372 including as much information as is known about the
applicant and a signed Authority for Release of Information and

Records (DD 2221). Mail both documents with a self-addressed
return envelope to:

(6) Applicants born in the Panama Canal Zone:
(a) To a U.S. citizen parent, provide a certificate
of birth issued by the Panama Canal Zone government indicating
U.S. citizenship and verified by the Panama Canal Zone
government will be accepted.
(b) Who cannot provide any of the above
documentation, the Panamanian Birth Certificate must be verified
with the Panama Canal Commission utilizing a specially modified
DD Form 372. Applicants shall not be enlisted until the Panama
Canal Commission returns the DD Form 372 indicating that either
the applicant's natural mother or father was a U.S. citizen at
the time of the applicant's birth. This DD Form 372 will be

(c) Are U.S. citizens only if either the natural
mother or father was a U.S. citizen at time of the applicant's
birth (this may also apply to applicants born in New Cristobal
(Colon, Republic of Panama)). Applicants who were born in the
Republic of Panama of U.S. citizen parent(s) claiming U.S.
citizenship shall have citizenship verified in the same manner
as any other applicant born abroad of U.S. citizen parent(s).
(7) Applicants unable to produce one of the above, or
other documentation indicating naturalized U.S. citizenship,
shall not be processed.
d.
Derived citizenship or naturalized citizens
(1) Applicants who have either completed naturalization
proceedings on their own behalf or claim to derive citizenship,
though alien-born, from the citizenship/naturalization of their

parent(s), must present a Certificate of Citizenship, a
Certificate of Naturalization (both of which are issued by
USCIS, formerly INS, or a U.S. passport, or one in which the
individual was included, as proof of citizenship. The
processing office will sight one of these forms and complete the
Evidence of Citizenship (NAVCRUIT 1100/25) form and return the
certificate to the applicant.
(2) In the case of those applicants who claim to derive
citizenship from the status of their parent(s), and who can
offer no proof other than documentary evidence of their
parent(s)' status, the following can be used:
(a) Document Verification Request (INS G-845) can be
used by the applicant to request verification of naturalization
or certificate of citizenship, or
(b) A United States passport
050134. CHANGE OF NAME STATEMENT. When an applicant does not
use the name listed on his/her birth certificate or Evidence of
Citizenship form (see Article 050134), the following additional
information shall be submitted (not required for nicknames):
a.
Record of court procedure legally changing name, or
b.
Change of Name Statement (NAVCRUIT 1131/14)
050135. FAMILY CARE PLAN CERTIFICATE. Any applicant that is a
single parent with physical custody of dependent children or any
applicant with a military spouse and dependents shall complete a
Family Care Plan Arrangements (NAVPERS 1740/7) for review by the
NAVCRUITDIST Commanding/ Executive Officer. The NAVCRUITDIST
CO/XO shall consider the 1740/7 and, based on their review,
endorse a Family Care Plan Certificate (NAVPERS 1740/6). Such
applicants shall be advised by the OPO that all individuals
accepted for naval service are expected to maintain a high
degree of commitment to his/her professional responsibilities,
and that no exemptions from policies or preferential treatment,
to include duty assignments, are extended on the basis of
dependency status.
050136.
POLICE RECORD CHECKS
a. Police Record Checks (PRC) (DD Form 369) are required on
all applicants who self-admit to criminal or juvenile offenses.

lessons learned is required for any offense (see Article
020105).
b. The application may be forwarded prior to completion of
the PRCs (DD Form 369). However, PRCs must be completed prior
to enlistment/appointment. If, during the investigation, a PRC
reveals any police or juvenile involvement not listed by the
applicant, a statement from the applicant regarding the offense,
and the reason for its omission, shall be included in the
application.
c. PRCs shall not be forwarded to law enforcement agencies
of foreign countries. If the applicant has indicated that a
foreign record exists for any offense other than minor traffic
violations, a request for a foreign police check shall be made
to the Director of Naval Intelligence (DNI) for action. Where
there is clear indication that rejection will ultimately occur,
based on locally available information, rejection shall be
effected without requesting a foreign police check from DNI.
The results of foreign police checks shall be forwarded directly
to COMNAVCRUITCOM by DNI.
d. All recruiting activities that originate out of town
PRCs are authorized to utilize law enforcement telecopy/
teletype facilities where access is feasible and permitted in
place of the DD Form 369. The hard copy of the request and
response message are acceptable for verification of completion
of the record check.
050137. INTERSERVICE TRANSFER OFFICERS. Interservice transfer
officers shall submit an Interservice Transfer Request Letter
(Exhibit 0501-01) requesting transfer between Reserve components
with a signed contingent resignation. See the Interservice
Transfer Checklist (NAVCRUIT 1131/41).
a. Inter-service applications for 1315 and 1325 designators
shall be forwarded directly to PERS 4911C at the address below.
b. All other application shall be forwarded to
COMNAVCRUITCOM for review and evaluation. COMNAVCRUITCOM’s
recommendations are then forwarded to Naval Personnel Command
(Pers-4911C).
c. If CNP disapproves, the inter-service transfer request,
they will notify the applicant and parent service by letter and
file the application without further action.

d. If CNP approves, the officer will be transferred.
Naval Personnel Command (PERS-4911C) will obtain appropriate
commissioning documents from Commander, Navy Personnel Command
(PERS-4803) and coordinate the transfer with the parent service.
e. If Naval Personnel Command (PERS-4911C) desires a waiver
of this instruction the application will be sent to the
Secretary of the Navy with supporting justification, and proceed
as previously stated above based on the Secretary’s decision.
f. Officers approved for inter-service transfer are also
required to submit a Ready Reserve Service Agreement (NAVPERS
1200/1), to remain in the Ready Reserve for an indefinite
period. See Section 0505.

050301.
OVERVIEW
a. This section provides specific guidance for submitting
an application for Reserve Officer Programs.
b. All Reserve officer applications are completed in one
stage. Unless otherwise indicated, all documents shall be
submitted to COMNAVCRUITCOM as electronic copies of originals
(via email or fax) to expedite the selection process and
minimize costs related to standard mail.
050302. COMNAVCRUITCOM INTEGRATED RECRUITING INFORMATION
MANAGEMENT SYSTEM
a. Upon submission of an application for any program other
than the Inter-Service Transfer Pilot (1315) and NFO (1325)
applicants, the OPL/OP shall:
(1) Record the applicant into COMNAVCRUITCOM Integrated
Recruiting Information Management System (CIRIMS) as a DCO or
IST. Reference should be made to the CIRIMS USER'S GUIDE for
detailed instructions on the use of this system.
(2) Upon completion of a new entry, review, print and
staple a CIRIMS DAILY REPORT in the applicant's NAVCRUITDIST
file.
b. Upon receipt of the application at COMNAVCRUITCOM, all
information concerning the status of the applicant will be
conveyed via CIRIMS and email. It is essential that all data
elements contained in a new record are complete and accurate,
and that transactions entered at all levels concerning
selection, appointment, declination, discharge, etc., can be
substantiated and are entered into CIRIMS in a timely manner.
c. Under no circumstances shall an applicant be notified
that he/she has been final selected until it appears in CIRIMS
as a "final select".

documents are not to be reproduced photographically:
a. Driver’s license and automobile titles in certain

TRANSLATION OF FOREIGN DOCUMENTS
a. All foreign documents submitted must be a line-by-line
translation into English prior to COMNAVCRUITCOM submission; a
document summary is unacceptable. See Article 050112 for
foreign transcripts.
b. The documents are not to be translated by the applicant,
applicant’s spouse or any family member.
c. Individuals conducting the translation shall verify, by
signature, correctness and accuracy of the document. The
following statement shall precede the required signature:
“I verify the translation on this document as being
correct and accurate to the best of my knowledge.”

APPLICATION FORMS AND CHECKLISTS.
a. The OR and OP shall review the kit using the Reserve
Officer Application Checklist for Non-Medical Programs (NAVCRUIT
1131/21) or the Reserve Medical Officer Application Checklist
(NAVCRUIT 1131/20). Prior to submitting the application, the OR
and OP (or personnel with “by direction” authority) shall
initial and date the checklist.
b. For all programs, completed checklists shall be
maintained with the applicant's residual file at the
NAVCRUITDIST. A copy of the checklist shall not be submitted to
COMNAVCRUITCOM.

050306. FORWARDING OF APPLICATIONS. The OPL/OP shall make a
CIRIMS entry when the application is first forwarded to
COMNAVCRUITCOM.
a. All correspondence shall be sent electronically to:
email to OfficerApplications@cnrc.navy.mil or faxed to (901)
874-9201.
b. When forwarding additional information to
COMNAVCRUITCOM, include NAVCRUITDIST name, POC, phone number and
email address to ensure proper and timely delivery.
c. All documents shall be forwarded in the order as listed
on the checklist.
d. Upon forwarding information to COMNAVCRUITCOM, ensure
the original documents are placed in the master application file
retained at the NAVCRUITDIST.
050307.
SUBMISSION PROCEDURES
a. After initial contact with an OR, and indication by the
applicant that they desire to pursue an opportunity in the Navy
Reserve, it is acceptable for a prospect to download forms or
receive forms by email or fax. These forms shall be completed
and forward to the NAVCRUITDIST (by standard mail, email or in
person). Multiple faxing of required documents is discouraged
as re-faxing typically deteriorates documents beyond legibility.
Utilization of overnight delivery services by the OR is not
authorized. If a document received by COMNAVCRUITCOM is not
legible, an error report will be generated requiring the
District to provide a legible copy. Processing shall stop until
the necessary document is provided.
b. Once the application is received at COMNAVCRUITCOM
(N36), the COMNAVCRUITCOM Processor shall:
(1) Review the application for completion and accuracy.
(2) Enter a Received date in the applicant’s CIRIMS
record.
(3) If the application is determined to be incomplete,
notify the NAVCRUITDIST via email with a list of missing items
and/or required actions.

c. If CIRIMS is not updated within 48 hours after an
application has been forwarded to COMNAVCRUITCOM, the
NAVCRUITDIST should call COMNAVCRUITCOM (N36) for verification
of receipt.
d.
Incomplete Application Packages
(1) The NAVCRUITDIST shall submit the necessary
information for complete and accurate applications within 14
days from notification that the application is incomplete.
(2) If the application remains incomplete after 14 days,
the COMNAVCRUITCOM processor shall enter a PRO “X” in CIRIMS
"Notes" and the electronic record will be deleted from
COMNAVCRUITCOM records the following day. Applicants with a
CIRIMS entry of PRO “X” are not qualified.
e. Not Basic Eligibility Requirements (BERs). When
applications are determined not to meet BERs, the COMNAVCRUITCOM
(N36) processor shall enter a PRO “X” in CIRIMS “Notes” and the
electronic record will be deleted from COMNAVCRUITCOM records
the following day. Records with a CIRIMS entry of PRO “X” are
not qualified.
f. Medical Review by COMNAVCRUITCOM (00M). After receipt
of an application by the COMNAVCRUITCOM Processing Division
(N36), medical examination documents are forwarded to the
COMNAVCRUITCOM Medical Advisor (00M) for a determination of
physical qualification. The determinations will be as follows:
(1) Physically Qualified (PQ)
(2) Not Physically Qualified (NPQ)
(3) Temporarily Not Physically Qualified (TNPQ). For
TNPQ applicants, the processor will notify the NAVCRUITDIST and
the OR of requested documentation by COMNAVCRUITCOM (00M). The
NAVCRUITDIST has 30 days to submit the requested information.
After 30 days, the record shall be forwarded to the appropriate
COMNAVCRUITCOM division officer recommending non-selection.
g. Armed Forces Institute of Pathology (AFIP). Healthcare
provider’ applications are forwarded to AFIP for verification of
credentials by the COMNAVCRUITCOM Program Manager. All medical
programs applicants must sign and date a Verification Conditions
and Release of Information (NAVCRUIT 1131/47) form to be

forwarded with the application. See the Reserve Officer
Affiliation Checklist (NAVCRUIT 1131/42).
h.
Professional Review Board
(1) For non-medical applicants, applications will be
forwarded to the Professional Review Board after the Medical
Advisor has determined the applicant to be PQ.
(2) For medical programs applicants, applications will
be forwarded to the Professional Review Board once application
and AFIP review (if required) are complete.
i.
Incomplete Applications
(1) After a PQ, if an application is incomplete, the
NAVCRUITDIST has 30 days in which to submit the necessary
information for a complete and accurate application.
(2) Applications incomplete after 30 days may be granted
two extensions, not to exceed 90 days.
(3) Any extension requests shall be submitted in writing
to the respective COMNAVCRUITCOM (N36) processor.
(4) Applications still incomplete after 90 days shall be
forwarded to the appropriate COMNAVCRUITCOM division officer
recommending non-selection.
j. Final Selection/Non-Selection. Receipt of professional
recommendation does not guarantee final selection. Information
included in the application could reveal characteristics about
an applicant that are inconsistent with final selection. These
include applicants who are:
(1) Not physically qualified (NPQ) for commissioning
(2) Not best qualified (NBQ) for commissioning
(3) In receipt of final adjudication of Security
Clearance in accordance with COMNAVCRUITCOMNOTE 5510
(4) Not Eligible for age waiver
k. Commissioning Documents. Commissioning documents will be
requested after final selection determination by COMNAVCRUITCOM
via PERS-4911 (if required).

050308.
RECONSIDERATION REQUESTS
a. Any officer program applicant who has been non-selected
may request reconsideration of their application by submitting
such request in writing to COMNAVCRUITCOM (N36).
b. If the request for reconsideration is submitted within
six months of official notification of non-selection, the
following information must be included:
(1) A new Application Processing and Summary Record
(NAVCRUIT 1131/1), with the “Reconsideration” block marked
appropriately.
(2) Applicant’s request for reconsideration, including a
letter from the applicant stating the reason he/she should be
reconsidered for selection. The reconsideration letter must be
signed and dated by the applicant.
(3) Any new or updated information including, but not
limited to transcripts, interviews and references.
(4) The field OPL/OP shall make changes to the original
CIRIMS record. A CIRIMS Action Request shall be completed by
the field OPL/OP and forwarded to COMNAVCRUITCOM (N36) when
requesting any changes in designator, recruiter’s SSN, FY, etc.
c. Applications will not be reconsidered for the same
program within six (6) months of non-selection unless new
information is submitted that indicates the applicant is better
qualified than when previously non-selected or unless changes
occur within the requested program’s status (i.e., requested
program was previously closed and re-opens).
050309. RECORDS MAINTENANCE. This Article is in accordance
with SECNAVINST 5212.5 series direction. The OPO shall ensure
compliance.
a. Recruiting Operations Records. These are files related
to the recruiting of officer and enlisted personnel to include
documents on local administration of recruiting programs,
recruiting reports not specifically authorized for other
disposition, and correspondence with civilian communities or
organizations or with other activities or offices. Destroy when
2 years old.

b.
Officer Recruiting Records
(1) Control Card Records and Summary Record Forms. These
files show status of applications and their depositions. Retain
these records until the applicant is commissioned or 5 years
after applicant is rejected, whichever is earlier. Note: The
Application Control Card and Summary Record have been combined
into the Application Control and Summary Record (NAVCRUIT
1131/1).
(2) Withdrawn or Rejected Applications For Commission.
These files include those of individuals found disqualified by
the local recruiting officer, and those forwarded to the
COMNAVCRUITCOM for final action and rejection, and related
correspondence and papers.
(a) Return to Applicants: Birth certificates,
transcripts of college credit, letters of endorsement, and other
material of value to the applicant. Note: Letters of
endorsement are distinctly different from letters of reference.
Letters of endorsement are generally offered to the applicant
for inclusion in their application. They are not required to be
submitted directly to the Navy nor do non-disclosure rights
apply, unlike letters of reference.
(b) Destroy all other documents after final
determination is made by COMNAVCRUITCOM.
c. Correspondence with Individual Applicants for
Commissions, Prospective Applicants, and Other Interested
Individuals (filed alphabetically by name). Destroy when 2
years old.
d. Officer Recruitment Status and Production Reports.
Destroy when 6 months old. [Note: Historical production data may
need to be retained longer in order to develop the Marketing
Operations Plan.]
e. Requests for Information. Any requests for information
held in the residual file by anyone other than COMNAVCRUITCOM
shall be forwarded to COMNAVCRUITCOM (N36) for review and
action.
050310. AUTHORIZATION AND GUIDANCE ON RECORDS DESTRUCTION. All
records shall be approved and initialed by the OPO prior to
destruction. Destruction of files shall be by burning or
shredding. The Reserve Officer Residual Checklist (NAVCRUIT

1131/51) shall be utilized when preparing records and the
original maintained in a Dead File.
a. CIRIMS and OAMS Records. Status of applications and
their dispositions shall be maintained in the CIRIMS and the
Officer Accessions Management Systems (OAMS).
b. Third Party Information. Third party information, (i.e.,
any written, personal opinions expressed about the applicant, to
include references and police checks) shall be destroyed at time
of withdrawal, rejection, enlistment/ commissioning,
declination, or at the six-month mark from the date of the
applicant's first resubmission. Summaries, interviews, FITREPs
and EVALs are not third party information. Summaries and
interviews, however, shall not be shared with applicants.

SECTION 4 - COMMISSIONING OF NAVY RESERVE OFFICERS
050401. GENERAL. This section contains guidelines for
commissioning of Navy Reserve officers. The authority to
appoint a selected applicant for an officer program is inherent
in the appointment documents provided by PERS-4802 to the
NAVCRUITDIST for delivery, contingent upon compliance with any
attendant restrictions.
050402.
PREPARATION FOR COMMISSIONING
a. Prior to an applicant's commissioning, the NAVCRUITDIST
(OPO) or CO's designee shall:
(1) Ensure that a review of the applicant's file is
completed to ascertain that no additional information of a
derogatory nature has been received subsequent to the time the
application was forwarded to COMNAVCRUITCOM headquarters.
Attention shall be paid to any character and employer references
or police record checks that may not have been forwarded to
COMNAVCRUITCOM prior to the time of selection.
(2) If unreported derogatory information is discovered,
ensure the commissioning is not effected and immediately notify
the NAVCRUITDIST CO and appropriate COMNAVCRUITCOM Officer
Programs Director (N31, N33, N34).
b. Prior to commissioning an applicant, the Officer
Processor (OP)/Officer Processor Lead (OPL) shall:
(1) Ensure the physical examination has not expired
(over 24 months old at time of commissioning). If the physical
has expired, the member shall not be commissioned until he/she
has received a new physical and has been found physically
qualified for commission. HIV test results must be less than
365 days old.
(2) In the case of selectees who are members of the Navy
Reserve or are members of another service, review the

and conditions are met prior to commissioning. If the release
date from the Navy Reserve or other service has expired, obtain
a new conditional release with a new release date. Under no
circumstances will the applicant be commissioned prior to
obtaining a signed release.

(3) Assemble the necessary and required commissioning
documentation using the Distribution of Commissioning Documents
Checklist (NAVCRUIT 1131/35).
(4) Review the BUPERS List of Restrictions, forwarded
with the individual's commissioning documents, to ensure that
all restrictions are either complete or scheduled for completion
prior to effecting the commissioning. Special attention shall
be paid to security restrictions (also see COMNAVCRUITCOMNOTE
5510).
c. If not already complied with, applicants drawing or
claiming a pension, retired or retainer pay, or disability
compensation from the United States Government by virtue of
prior military service, shall, by time of commissioning:
(1) Voluntarily agree to waive the pension, retired or
retainer pay, or disability compensation being received, or
(2) Voluntarily agree to waive claim for active duty pay
and allowances.

PREBRIEFING THE SELECTEE
a. The OP/OPL shall ensure the selectee completes the
following commissioning documents prior to effecting the

b. The officer recruiter (OR) and OP/OPL shall review the

shall notify the appropriate COMNAVCRUITCOM Division Officer

(N31, N33 or N34) and NAVCRUITDIST CO immediately should changes
or new information preclude an appointment. Particular
attention shall be paid to the following:
(1) Change of Status. Any positive answer on the Change
of Status Questionnaire (NAVCRUIT 1131/33) that is inconsistent
with previously furnished information. If inconsistencies are
found, or new information is discovered, the applicant shall not
be commissioned.
(2) Physical Condition. If the selectee's physical
condition appears to have changed since the examination
administered at the time of application; or information
indicates that a change may have occurred; or the selectee
indicates that his physical condition has changed since the
latest examination, the applicant shall not be commissioned.

(3) Lawsuits
(a) No selectee who has a lawsuit pending against
them shall be commissioned without prior approval of the
appropriate COMNAVCRUITCOM program manager.
(b) In order to assist COMNAVCRUITCOM in making a
determination in such cases, a description of the nature of the
case shall be forwarded along with a statement from the
applicant's lawyer as to when the case will be tried and whether
or not the applicant's presence in court at the time of trial
will be required.
(4) Active Duty Recall. All selectees shall be advised
by the OPO/OR that they may be recalled to active duty during
their service in the Navy Reserve. If a selectee has not
previously been enlisted into the Navy and/or received bonuses
or stipends from the Navy and communicates his/her unwillingness
or objections to serve on active duty, the selectee shall not be
commissioned.

EFFECTING THE APPOINTMENT
a. Any commissioned officer of any component of a U.S.
Armed Force, whether or not on active duty, may administer the
commissioning oath of office.

b. The official administering the oath of office, the OPL
and OP shall ensure that all restrictions appended to the
appointment documents have been met prior to the candidate being
administered the oath. In the event there is a question
regarding the restrictions involved, administration of the oath
shall be delayed until all questions have been resolved.
Alterations of names, grades or dates of rank, shall not be made
on appointment papers before or after execution. Pen and ink
changes of birth dates and social security numbers may be made
only upon receipt of authority from PERS-4802. Appointment
papers will be reissued by PERS-4802 upon notification of an
error.
c. All remaining commissioning documents shall be completed
by the OPL or OP prior to the selectee's commissioning.
d. After the oath has been administered, the appointee and
the officer administering the oath shall sign all copies of the
Oath and Acceptance of Office (NAVPERS 1000/20).
e.
The OPL/OP shall ensure:
(1) All signatures are original. Use of carbon is not
authorized except for typing in the date and name/rank of the
official who administered the oath.
(2) Appointees serving in enlisted status, and
appointees serving in temporary commissioned or warrant grade,
whose permanent status is enlisted have been honorably
discharged from their enlisted status by reason of "selected
changes in service obligation to accept permanent appointment to
officer grade" per MILPERSMAN 1910-102.
f.
The Appointing Officer shall:
(1) Honorably discharge appointees from their enlisted
status by reason of "selected changes in service obligation to
accept permanent appointment to officer grade" per MILPERSMAN
1910-102 as of the day immediately preceding the date of
acceptance of the permanent appointment as appropriate.
(2) If not the custodian of the enlisted service record,
notify the service record custodian regarding the type of
appointment accepted, the date it was accepted, and provide a

pay and non-pay billets within Navy Reserve units. The process
includes verification of officers' status, completion of

billet, approval from PERS-4911 and COMNAVRESFOR, execution of
orders, attainment in CIRIMS, and obtaining drill verification.

ELIGIBILITY
a. An officer's eligibility for assignment to a Navy
Reserve unit, including non-pay assignment, shall be determined
before executing Inactive Duty Training (IDT) orders. Navy
Reserve members may be assigned to four sub-categories:
(1) Ready Reserve (USNR-R)
(2) Standby Reserve-Active (USNR-S1)
(3) Standby Reserve-Inactive (USNR-S2)
(4) Retired Reserve (USNR-Retired)
b. Members shall be in the Ready Reserve for assignment to
a SELRES unit with pay. Members must be in the USNR-R or USNRS1 for non-pay assignment to a unit. Officers in the USNR-S2
are not eligible for unit assignment (BUPERSINST 1001.39 Chapter
1 Section 103). They shall request Naval Personnel Command (NPC
4911) to transfer them back to the Ready Reserve (USNR-R) in
order to drill. USNR-RET members may participate on a
voluntarily basis only per Chapter 10 section 1008 BUPERSINST

RECRUITER RESPONSIBILITIES
a. The Officer recruiter shall complete the following steps
in the order listed. Each step shall be completed prior to
beginning the next step.
(1) During the initial interviewing process, the
recruiter shall have the applicant read, sign, and date the
Privacy Act Statement (NAVCRUIT 5211/1) and Voter Registration
Information (DD Form 2645).
(2) Verification of officer’s status with PERS-4911.
Before processing a Navy veteran officer for affiliation, it is
necessary to verify, by telephone conversation with a

(3) Complete the affiliation application paperwork in

fax cover sheet.
(5) For NAVET medical designators (2105, 2205, 2305 and
2905), obtain COMNAVCRUITCOM Program Manager and BUMED Community
Manager approval.
(6) The Affiliation Processing and Summary Record
(NAVCRUIT 1131/43) along with copy of current license and
license verification shall be forwarded to COMNAVCRUITCOM (N36)
for review by the respective Program Manager. All blocks on the
Affiliation Processing and Summary Record (NAVCRUIT 1131/43)
shall be completed.
(7) This information will also be forwarded to the BUMED
Community Manager by the COMNAVCRUITCOM Program Manager for
review.
(8) COMNAVCRUITCOM Program Managers will obtain quota
number and financial incentive written agreement from CNRFC
(N112) for eligible medical sub-specialties (2105, 2205, 2305
and 2905). If approved, CNRFC (N112) will complete the
financial incentive written agreement and email a copy to the
COMNAVCRUITCOM PM and the recruiter.
(9) If an applicant wishes to decline eligibility for a
financial incentive program, the Page 13: Statement Of
Declination of Financial Incentives (NAVCRUIT 1131/212) shall be
completed and included in the applicants kit.
(10) Approval from the COMNAVCRUITCOM Program Manager is
required prior to entering the applicant into CIRIMS. A
COMNAVCRUITCOM processor (N36) will email the recruiter with the
COMNAVCRUITCOM PM’s approval or disapproval and financial
incentive quota # if applicable. This email shall be kept as
part of the residual file for all NAVET medical program
applicants.
(11) The OPL or individual designated in writing by the
CO shall enter the financial incentive quota into CIRIMS prior
to forwarding to PERS-4911.

(12) Once the applicant is approved in CIRIMS, the
recruiter shall have the applicant execute the Financial
Incentive Agreement and submit it to the Navy Reserve Activity

(j) For all other designators, billet information
shall be obtained from the Regional Reserve Readiness Command
(REDCOM).
(2) Submit required documents to PERS-4911 for
affiliation via fax. Telephone and fax numbers are listed in
Article 050505.
(3) Immediately following fax transmission, the
recruiter shall enter the applicant’s information into CIRIMS
and forward to PERS-4911.

c. Recruiters are responsible for tracking the progress of
their applicants in CIRIMS from time of entry through final
approval by CNRFC, attainment and drill verification entry.
d. For Inter-Service Transfer Officers, include a Ready
Reserve Agreement (NAVPERS 1200/1) and Oath of Office (NAVPERS
1000/20).
e. For Direct Commission Officers (DCO) and Limited Duty
Officers/Chief Warrant Officers (LDO/CWO), include the Ready
Reserve Agreement (NAVPERS 1200/1), Oath of Office (NAVPERS
1000/20), all enlisted Page 5's (History of Assignments) for
current enlistment, and most current enlistment/immediate
reenlistment DD Form 4.
f. For USNR officers who have recently separated from
active duty, submit Ready Reserve Agreement (NAVPERS 1200/1),
Oath of Office (NAVPERS 1000/20) (if USNR Oath of Office was
required upon separation from active duty, discharge or
resignation), a copy of Certificate of Release or Discharge from
Active Duty (DD Form 214, long form), and separation orders.
g. For Individual Ready Reserve (IRR) personnel, submit
Ready Reserve Agreement (NAVPERS 1200/1). A status check with
PERS-4911 will determine if other documents are required.
h. For S-2 Personnel, submit Ready Reserve Agreement
(NAVPERS 1200/1). The recruiter shall ensure these members have
a current physical examination. Do not send physical
examinations to PERS-4911 unless requested to do so.
i. The NAVCRUITDIST OPOs shall ensure entries in CIRIMS are
made correctly and the application is forwarded to PERS-4911
within 48 hours from time of entry.
j. PERS-4911 shall indicate approval/disapproval in CIRIMS.
If approved, the applicant's record will be forwarded to
Surface/Air CNRFC in CIRIMS. If approved by Surface/Air CNRFC
in CIRIMS, the recruiter shall enter attainment date in CIRIMS.
k. The OPL shall print the CIRIMS approval report and
forward, with the Officer Application/Orders for Inactive Duty
Training (NAVRES 1321/1), to the Reserve Center Commanding
Officer and Regional Reserve Readiness Commander for execution
of all designators except Civil Engineer Corps (5105). For 5105
officers, orders are executed by CNRFC CEC Program Manager at
(202) 685-9014/1954, fax (202) 685-1465.

l. The OR shall maintain a copy of CIRIMS approval report
in the residual file.
m. Credentialing of Medical Programs Officers. The HSOJAX
FAX Transmittal (NAVCRUIT 1131/49) shall be faxed by the
recruiter to CCPD once the applicant is attained in CIRIMS. A
signed and witnessed Health Care Credentialing Refresher
Acknowledgement for NAVET (NAVCRUIT 1131/50) form shall
accompany the HSOJAX FAX Transmittal (NAVCRUIT 1131/49) for all
NAVET applicants. The COMNAVCRUITCOM Program Manager will also
forward the applicant’s information to CCPD.
n. Mail Initial Assignment for Inactive Duty Navy Reserve
Officer Letter (Exhibit 0505-01) and Welcome Aboard Letter
(Exhibit 0505-02) to the applicant along with a copy of their
processing record.
o. Obtain the Drill Verification Muster Sheet from the
gaining command after the applicant drills and place it in
applicant’s residual file. Fax a copy of the drill verification
to the NAVCRUITDIST OPO and enter the drill date into CIRIMS.
This entry must be made within two months from the month of
attainment.
050512. TOUCH AND GO RECRUITING. Touch and Go Recruiting
allows two NAVCRUITDISTs to receive credit for affiliating a
USNR-R officer who is relocating after separation from active
duty.
a. The TOUCH Recruiter is the recruiter who meets a Navy
veteran (NAVET) officer released or to be released from active
duty within 90 days. The TOUCH Recruiter shall:
(1) Be responsible for completion of the affiliation
package including obtaining the DD-214 and separation physical.
(2) Forward the complete affiliation package to the
Hometown Officer Recruiter (HTOR).
(3) The HTOR shall:
(a) Review of Physical status
(b) Obtain approval by COMNAVCRUITCOM Program
Manager for medical program NAVETS.

(c) Obtain a vacant billet
(d) Obtain approval by PERS 4911 and CNRFC via
CIRIMS, execution of orders and attainment in CIRIMS.
(e) Obtain drill verification.
(4) Attainment credit is received after the member
affiliates and performs four drill periods at their new Reserve
activity.
b. Officer recruiters shall electronically submit all Navy
Reserve Officer accession requests for pay billets via CIRIMS to
PERS 4911 and CNRFC.
c. Billet approval shall be based on the date the
Surface/Air Reserve Force approves the accession request record
via CIRIMS. Officer recruiters and NAVCRUITDIST OPOs shall
print out and retain the Daily Approved Report for two years
plus current per SSIC guidelines.
d. The Officer Application/Orders for Inactive Duty for
Training (NAVRES 1321/1) must be signed within ten working days
of the CNRFC approval date by the cognizant Echelon IV/V
authority.
e. Officer Recruiters shall receive credit for affiliations
when attainments are made in CIRIMS. A copy of the drill
verification shall be included in the officer recruiter's
residual files. Attrites shall be recorded on the fifth working
day of each month. “Attrite” is defined as a member claimed for
attainment credit that fails to drill in the two months
following the month of attainment credit. For example, if a
member affiliates in January, attainment shall be credited in
January. The member must drill by 31 March and drill
verification must be posted into the system prior to 31 March.
If the drill verification is not properly posted, an attrite
shall be recorded on 5 April.
f. NAVCRUITDIST OPOs shall closely monitor approvals,
attainment designators, and subspecialty codes; transaction
codes, and drill verifications. Awards boards at the end of the
fiscal year shall utilize this information to help determine
award recipients.
050513. RESERVE RECRUITING RESIDUAL REQUIREMENTS. Affiliation
and direct appointment residual files shall be maintained for

two years from the effective date of orders for all personnel
affiliated. The Affiliation Package Checklist provided to the
gaining command is the same as the Reserve Officer Residual
Checklist (NAVCRUIT 1131/51). The documents shall be arranged
in the order that they are listed.
050514. RESERVE INCENTIVE PROGRAMS. Requests for information
regarding financial incentives for Reserve officer programs
shall be addressed by CNRFC (N112), 4400 Dauphine Street, New
Orleans, LA 70146, (504) 678-6649/4461.
a. Once an applicant has been approved by the
COMNAVCRUITCOM Program Manager, the Program Manager will send an
email requesting an incentive quota number.
b. The financial incentive assigned quota number will be
emailed to the recruiter and the COMNAVCRUITCOM Program Manager.
c. The recruiter shall have the applicant sign a written
incentive agreement and forward with gain package to the Navy
Reserve Activity. The Navy Reserve Activity will complete a
request for eligibility establishment within 30 days of
affiliation, including a copy of the written incentive agreement
and forward to CNRFC (N112) (address above) for eligibility
establishment. Note: The only written incentive agreements
that will be accepted are those that were provided by CNRFC via
email to the COMNAVCRUITCOM Program Manager and recruiter.

SECTION 6 – NAVET APPOINTMENT
050601. AUTHORIZATION. MILPERSMAN 1131-040, Appointment of
Officers in the Navy Reserve, outlines policy for Regular Navy
officers separating from active duty, and former Regular Navy or
Navy Reserve officers considered qualified by Navy Personnel
Command (NAVPERSCOM).
050602. REGULAR NAVY OFFICERS BEING PROCESSED FOR HONORABLE
SEPARATION
a. Regular Navy officers who request separation from the
Navy and who have satisfied their military service obligation
(MSO) may request appointment in the Navy Reserve before
separation. Regular Navy officers who submit a voluntary
resignation and who have not satisfied their MSO will be
automatically considered for appointment in the Navy Reserve.
However, it is imperative that all officers desiring a Reserve
Appointment execute a Reserve Oath of Office and return to PERS4911.
b. Favorable action on resignation requests from officers
who have not satisfied their MSO will normally be contingent
upon their acceptance of a Reserve appointment, if tendered.
c. Separation orders shall specify whether the member has
or has not been approved for a Reserve appointment, and whether
separation is contingent upon acceptance of a Reserve
appointment.
d. The appointment shall be forwarded to the
member by NAVPERSCOM (PERS 4803) and is to be effected
no earlier than the day after separation and within 6
months of release.
e. The Reserve appointment shall be in the same grade, with
the same date of rank, and normally in the same general category
of designator, as that held in the Regular Navy.

SECTION 7 – NAVET REAPPOINTMENT
050701.
FORMER OFFICER REQUESTING REAPPOINTMENT WITHIN 3 YEARS
a. Former Regular Navy and Navy Reserve officers who were
honorably separated and did not retain an appointment may
request a Reserve appointment from NAVPERSCOM (PERS-491) within
3 years following the date of their separation.
b. The Reserve appointment will normally be in the same
grade and designator previously held. NAVPERSCOM (PERS-491)
will adjust the member’s pay entry base date. If the period of
separation is in excess of 6 months, NAVPERSCOM (PERS-486) will
adjust the member’s date of rank.
c. Member shall request a Reserve appointment in writing to
NAVPERSCOM (PERS-491), via a Navy Reserve officer recruiter.
Requests shall be endorsed by the recruiter and must include:
(1) Name, social security number, designator at time of
separation, address, and phone number;
(2) A statement of the reason for not requesting,
receiving, or retaining a Reserve commission at the time of
separation; a copy of the member’s DD-214, Certificate of
Release or Discharge from Active Duty;
(3) A copy of the member’s separation orders if
separated from active duty;
(4) A copy of an updated Electronic Personnel Security
Questionnaire (EPSQ) for National Security Positions if period
of separation is more than 1 year; and
(5) A copy of DD Form 2807-1 report of Medical History
completed within last 90 days.
050702. FORMER OFFICERS REQUESTING REAPPOINTMENT AFTER 3 YEARS.
After 3 years from the date of separation, former naval officers
may only request appointment in the Navy Reserve via an
authorized direct appointment program. Applicants must meet all
of the eligibility requirements of the direct appointment
program, per Chapter 2.

050802. PROCEDURES FOR APPLYING. Per MILPERSMAN 1320-150,
reserve officers on inactive duty who desire to voluntary
extended active duty shall submit an Application for Recall to
Extended Active Duty (NAVPERS 1331/5), directly to Naval
Personnel Command (NAVPERSCOM) (PERS-480C).
050803. PHYSICAL STANDARDS. Each applicant selected for recall
to extended active duty shall meet physical standards
appropriate to their grade as established by Chief, Bureau of
Medicine and Surgery at the time ordered to active duty.
050804. DESIGNATOR CHANGE. Recall applications from officers
who are requesting a change of designator must include the
statement: "recall contingent upon approval of change of
designator."
050805. RECALL TO ACTIVE DUTY. Officers without prior active
naval service may apply for recall to active duty per MILPERSMAN
Article 1320-150.
050806. AUGMENTATION. Officers requesting to augment may do so
per MILPERSMAN 1131-020, Appointment in the Regular Navy of
Reserve and Temporary Officers.

050807. SELECTION. The normal processing time for applications
is approximately six weeks. In addition to service needs, basic
criterion for selection is whether, in the opinion of the staff,
the candidate is fit by reason of age, education, previous naval
and civilian experience, past performance, and suitability for
assignments, etc., to compete successfully on a career basis
with their contemporaries, both Regular and Reserve. Officers
will receive official notification of the final decision on
their recall request.
