SUMMER STUDY ABROAD – PRAGUE

Check-Out: Sat, June 25th, 2011 by noon (housing/luggage storage not available after)
Registration and Required Forms
Your space in the Tisch Study Abroad Program can not be confirmed until you’ve submitted your deposit to Admissions at
the Office of Special Programs.
Required Forms:
1) Confirmation Form: due at date give by Admissions office
2) Immunization Forms: due April 30th – IMMUNIZATION RECORDS ON FILE AT NYU
3) Tisch online Supplemental Forms – due April 30th via the application system to which you applied to the
program. Use the same Username and Password you used to create your application.
4) NYU Traveler – due April 30th required by public safety – link on Blackboard - You DO NOT need to mail
in forms – it is all done online – ignore the glitch that asks you to mail in any forms
Visas
No Visa is needed for the summer for U.S. citizens. Non-U.S. citizens will need to contact the Czech Consulate to research
whether or not a visa is required and will need to obtain a visa on their own. If you plan to travel outside the Czech
Republic after your program ends, you should call the consulate of the country or countries you plan to visit to ask if they
require a visa.

Travel and Arrival
You must book your own flight to Prague arriving no later than the check-in day. Student travel discounts: STA Travel or
StudentUniverse.com. You should check-in directly to your residence. In the past, students have had taken a taxi or public
transportation to get to their residence (see Blackboard). SUGGESTED: Airport Shuttle Bus near Namesti Republik, Leaves
every 30-60min; 120Kc+ for 30min ride

Health and Wellness
While you’re studying abroad, it’s important to maintain your physical and mental health. If you need regular care from a
medical or mental health professional, the Student Services office can work with you to get referrals to providers in the city
where you will be studying. You should start this process as early as possible.
Be aware certain prescription medications cannot be mailed internationally due to customs restrictions and may not be
available overseas. Prescriptions written in the U.S. are not honored overseas, so you should either secure enough
medication for the entire duration of your stay, or plan to use local mental health or medical professionals. It is important
that you discuss these issues with a health professional before going abroad.
As part of your overseas program, you have been enrolled in a global health insurance plan called HTH Worldwide, at no
additional cost, which will provide you with easier access to medical and mental health services and protection in the event
you become ill or injured while abroad with NYU, or require ongoing health or mental health care during your time away.
This new health benefit will work in conjunction with your primary health insurance plan to supplement your coverage
overseas. Please check Blackboard for information regarding HTH Insurance and other Health and Wellness tips.
