Overview of Teaching for previous three years
Summarize the candidate’s teaching efforts, including the numbers of students taught, amount of time spent in teaching related activities and the various types of teaching activities, i.e., bed side teaching, laboratory mentoring, CME, GME, etc. This summarization should be pulled from the candidate’s teaching activities summary to be found in the candidate’s personal statement from Sections 1 - 3 of the education database.
Indicate if the candidate is the course director for any programs.
For reappointment, a personal statement is not required and Section 3 does not have to be attached.
Section 1, the candidate’s cv will be included as part of the entire reappointment dossier.
Example:
Dr. X’s teaching activities over the last 3 years are extensive and will be summarized. He is the course director of PSYC 709-Clinical Seminar in Community Psychology and Mental Health Services
Research which serves as a course in the Doctoral Program in Clinical Psychology, Graduate School of Arts & Science taught to 5 students. He is also course director of PUBH 504-Behavioral and Social
Sciences in Public Health. Masters in Public Health Program, School of Medicine taught to 15 students. In addition, he serves as course director of BIOE 590 - Ethical Issues in HIV: Prevention,
Treatment, and Research taught to 15 students in the masters program in bioethics. he has also taught or guest lectured in a variety of psychiatry resident courses, workshops and seminars. In addition, he has taught in a variety of medical student courses such as Frontiers in HIV Medicine (FR502) and has been responsible for the supervision and individualized teaching of graduate students, residents, and post doctoral fellows. He is also active in teaching during the very popular ID electives at HUP in which fully a third of medical student have enrolled.
Dr. X has lectured more than 30 times primarily on the topic of mental illness in patients with HIV at other academic institutions, governmental agencies, and scholarly organizations.
He has had extensive responsibilities in supervising graduate students including 7 research projects or independent work by graduate students or medical students and 3 PhD. thesis candidate and coauthored over 38 original articles with graduate students and medical students.
Review of administrative teaching activities
Document membership on departmental and school-wide committees related to education and participation in medical school or BGS advising programs as well as any leadership roles in administering educational programs. This data can be found in Section 4 of the educational database.

Analyze internal evaluation data (HAMSTER, Biomedical Graduate Studies data, other University of

Analyze quantitative and qualitative data reports. Include any teaching awards or other significant distinctions won by the candidate. Good as well as bad data should be explained. The evaluation data can be found in Section 5B of the educational database.
For reappointment, attach the summary page from HAMSTER and include any other quantitative data that may be available.
Formal evaluations from students in the many courses in which he teaches are consistently at the high end of the grading scale indicating a superior performance as a teacher. His summary assessments for the medical student courses are above those for the course lecturers as a whole. In reviewing many dozens of course evaluation forms, one can find only one negative comment about him or low-rated evaluations of his teaching efforts.
Analyze external evaluation data (CME data)
Analyze quantitative and qualitative data reports for activities not reported on via HAMSTER. The evaluation data can be found in Section 5B of the educational database.
For reappointment, include any quantitative data that may be available.
Some of the comments from undergraduates included the following:” I thoroughly enjoyed Dr. X’s lecture, he engaged us at an intellectual level” and “Dr. X was an excellent speaker. He was articulate and energetic and was able to present his work in a clear and concise manner.”
Summary
Provide a final summary documenting the candidate’s teaching efforts and the quality of those teaching efforts.
The documentation and evaluation of Dr. X’s teaching efforts demonstrate that he teaches in many venues, including medical students and undergraduate students. His teaching efforts are well received by the trainees and he is above average for his evaluations when compared to department data.
