Trucking Operating Accidents of the week of January 20, 2008
An extensive review was conducted on January 28, 2008 regarding a total of four
highway accidents involving our company delivery trucks. The accidents occurred during
a one-week period starting on January 20 and ending on January 25, 2008. The
company’s mandatory Accident Review meeting was held on February 1, 2008. The

Accident Summary
Four operating trucks from Spangler Trucking, Inc. headed out on their daily delivery
routines on January 20, 2008. Each truck traveled to different parts of Texas which were;
North, West, South, and East. Beginning on Monday, January 20, 2008, each truck began
to have accidents, a different day of the week, which then resulted in a loss of
merchandise and damage to the truck. Only one of the accidents involved another
vehicle. Only one of the accidents involved injury to a company driver. After a thorough
investigation our Maintenance Division traced all four of the accidents to faulty tires.
These tires apparently were all purchased at the same time, Noon on Friday, January 17,
2008, and same place which was Speed’s Tire Manufacturer. They also appeared to be all
the same brand, Speedy’s Tires. Ultimately, each truck resulted in damage totaling an
amount of $200,000 and the total amount of cargo damaged is $1,000,000.

Accident Details
1. Company truck 123 was traveling northbound about 25 miles from Dalhart on
January 20, 2008 when the tires began to skid. This was a result of the icy road
conditions. The truck collided with a light pole causing the back lift to open and
cargo falling out and being damaged. Seven beams of steel collapsed and the
surfaces were scratched badly. Before the light pole could fall and completely
crush the truck, the driver was able to rush out with no serious injuries.
2. Company truck 456 was traveling westbound about 10 miles from Dallas on
January 21, 2008 when the upper left tire blew out causing the truck to swerve
and hit a stranded parked car on the side of the highway. The 2004 Toyota Camry
belongs to owner Hillary Clinton. A large portion of cargo was damaged which
most of it was not possible to save. Major damages were made, both, to the
company truck and parked car.
3. Company truck 789 was traveling southbound about 20 miles from San Antonio
as it suddenly turned off the road just before attempting to take Exit 724. Again, a
large portion of cargo was damaged. Two of the five steel beams were slightly
bent, which is major to our company, and a big loss of revenue. The only damages
to the company truck were all four damaged tires.
4. Company truck 012 was traveling eastbound about 30 miles from El Paso when
the rear right side tire blew out causing the company truck to swerve into a deep
ditch. This caused the truck to flip on its right side and crushing most of the cargo.
The truck was completely crushed on its right side. The truck driver, John
McCain, was also injured with a broken neck, partially broken left leg and bruised
body.
To prevent a recurrence of such an accident, the insurance company, Acme Underwriters,
has brought suit against the tire manufacturer, Speed’s Tire Manufacturer.


Speed’s Tire Manufacturer has now replaced faulty tires on all of the trucks.
Since the lawsuit, Speed’s Tire Manufacturer has cooperated with providing us
with an income of $200,000 from lost business while the four company trucks
were being repaired.
