Imaging – announcement Hologic’s imaging agreements expire; GE sole
supplier for mammography and bone densitometry
Despite months of negotiating, Premier and Hologic were unable to come to
terms regarding an extension to agreements for Mammography Products and
Services (PP-IM-112) and Bone Densitometry (PP-IM-113). Like every other
current contracted supplier of major imaging equipment systems (GE, Philips,
Toshiba, Fujifilm, Swissray and Quantum), Hologic was offered the opportunity
to extend its Premier agreements for 20 months beyond their original expiration
date of September 30, 2010, but declined unless some significant changes to its
agreements’ terms and conditions were made.
The requested changes included some that were a significant enough departure
from Premier’s business model to make them impossible to accommodate.
Therefore, GE Healthcare is now Premier’s sole supplier of Mammography (PPIM-090) and Bone Densitometry (PP-IM-091) systems until May 31, 2012. Premier
will continue working with GE to offer periodic group buy opportunities to further

Recall Nursing – announcement that Hospira, Inc. issued a voluntary recall
alert regarding its Symbiq™ One-Channel (list no. 16026) and Two-Channel (list
no. 16027) Infusers. Hospira has received reports of unrestricted flow of
medication with these devices. Unrestricted flow of medication and/or other
therapy has the potential to cause life-threatening effects and/or critical patient
injury, especially in specific patient groups such as critically ill patients, patients
with congestive heart failure and/or neonates. This condition may occur if the
administration set cassette is removed improperly. Additionally, when this
condition occurs, the "Check Flow Stop" alarm, which is designed to notify
users of a free flow condition, may not function as intended and may not provide
appropriate warning to users. The U.S. Food and Drug Administration (FDA) have
classified this Urgent Device Correction as a Class 1 recall. This recall does not
require the removal or return of any Symbiq devices at this time. As part of the
next software release, Hospira will be correcting the defective “Check Flow
Stop” alarm by increasing the detection sensitivity when the cassette is removed
prior to the cassette carriage being in the fully open position.

Recall Nursing - B. Braun announced that it is initiating a recall on their 13
mm and 20 mm addEase Binary Connectors. B. Braun has received reports of
particular matter being generated upon the insertion of the B. Braun addEase
Binary Connector (N7790 and N7993) into a PAB container. During this insertion,
fragmentation of the PAB container stopper may occur, resulting in a small
amount of visible particulates in the solution.
There is a risk of potential injection of particulates which could result in lifethreatening injuries, such as pulmonary embolism, stroke, heart attack and/or
death. This is available through Premier agreement PP-IV-045.
Because of this issue, B. Braun is initiating a voluntary recall for the following

Recall – announcement that Baxter has been given two years to recall all
Colleague Infusion Pumps and to provide refunds or replacement pumps.
Certificate of Necessity (CN) must be returned to Baxter by November 2010
In a letter dated July 13, 2010, the U.S. Food and Drug Administration (FDA)
issued its final order to Baxter Healthcare Corporation regarding the recall of the
Colleague volumetric infusion pumps from the domestic market. The pumps are
being recalled due to persistent safety problems that have resulted in serious
patient injuries and more than 500 deaths.
The FDA ordered Baxter to recall and destroy all Colleague pumps currently in
use in the U.S. and provide a refund or a replacement Baxter device to all
Colleague infusion pump owners with no delivery, installation or training costs
to the customer. Additionally, all lessees may terminate their lease with Baxter or
one of its authorized agents without penalty.
Baxter has until July 14, 2012 to carry forth the actions outlined in the final order
of the Consent Decree. During this transition period, customers may continue to
use Colleague pumps but the FDA recommends replacement of Colleague
infusion pumps as soon as practicable. Customers have until mid-November
2010 to submit the
Certificate of Necessity (CN). The CN must be returned within the specified
timeframe in order for the customer to be eligible for either the refund or
replacement programs.
While the FDA has set a 24-month transition period, it is imperative that
members begin planning their process as soon as possible as there are some
rather involved steps within the evaluation, selection and implementation
phases. Premier will continue working closely with Baxter and other suppliers
offering suitable alternatives to the Colleague infusion pump so expect our
updates to be on-going. Baxter will publish an FDA-approved Transition Guide
by September 11, 2010.
For further Clinical Services support: Contact us directly or through your client service
executive to arrange for further assistance: Timothy Glennon, RN, MSN, MBA, CMRP Vice
President, Clinical Services, glennon@gnyha.org or (212) 506-5445, or Sandee Schoby, LVN,

Clinical Services
While not all-inclusive, the document will provide
guidance for minimizing disruption and patient risk, offer suitable FDA approved
or cleared alternatives to the Colleague infusion pumps and give greater detail
on the refund and replacement
programs. Until the guidance document is available, members are encouraged to
review all of the information that is currently available through the resources
below.
Recommended actions:



Thoroughly read through all available information from the FDA and Baxter
Begin taking inventory of all Colleague pumps, batteries, spare parts and
consumables. This should include required information for the CN: serial
number, model, owned/leased status, leasing company, and details of other
large volume infusion pumps currently used at the facility.
Complete the CN no later than mid-November 2010. The CN form will be
available when the Transition Guide is published no later than September 11,
2010.

Recall – announcement that Medivators Reprocessing Systems
announced that is it initiating a recall involving the use of the Medivators DSD-110HU0109 Endoscope Hookup to connect the Pentax EG-3630U Ultrasound Endoscope
to a Medivators DSD endoscope reprocessor or Medivators Scope Buddy
Endoscope Flushing Aid. These hookups are available through Premier agreement
PP-OR-517, Automated Endoscopic Reprocessors. This recall has been initiated due
to a misprinted DSD Hookup Application Guide (MG04-0001 revision M) that
recommended the use of the DSD-110-HU0109 instead of the DSD-110-HU0144 and
CLM-110HU0145 hookups.
Use of the DSD-110-HU0109 for reprocessing of the Pentax EG-3630U Ultrasound
Endoscope in conjunction with the DSD may result in inadequate high-level
disinfection of either the balloon filling channel or the elevator wire channel
depending on the method of connection.
Suggested action steps:



Immediately examine your Pentax EG-3630U Ultrasound Endoscope
hookup, and inform all staff of this notice to ensure that the DSD110-HU0109 is not used with the Pentax endoscope.
Return of the DSD-HU0109 hookup is not being requested, and
customers can continue to use the DSD-110-HU0109 hookup except
where the Pentax EG-3630U Ultrasound Endoscope is concerned.

administrators and infection control healthcare professionals of important
information regarding the regulatory status of the STERIS System 1 processor
(SS1) used in surgical and endoscopy suites for reprocessing, i.e., sterilizing or
disinfecting, medical devices.
STERIS has significantly modified the SS1 and the FDA has not approved or
cleared this modified product. Thus, the FDA has not determined whether the

medical devices. Use of a device that is promoted to sterilize or disinfect a
medical or surgical device, but that does not properly perform these functions,
poses risks to patients and users. Improperly disinfected or sterilized
instruments may transmit pathogens to patients and healthcare staff, or expose
them to hazardous chemicals. Improper sterilization or disinfection may also
adversely affect the quality and functionality of reprocessed instruments.
The FDA recommends that if a facility has an acceptable alternative to the SS1 to
meet sterilization and disinfection needs, the facility should transition to that
alternative as soon as possible to ensure continued patient safety. If an
acceptable alternative to the SS1 is not available, the facility should promptly
