Make sure they know how the program works, and how easy it is to get their employees
on board.

reward, eligible employees need to complete just two simple health-related activities:

Biometric screening
To participate in an onsite biometric screening event, please contact your producer or Premera
Coordinator, Melissa Ange at Wallace Insurance.

Paying attention to your health is important! That’s why Premera Blue Cross Blue Shield
of Alaska is offering you a $150 Visa debit card to participate in two simple healthinformation activities.
It’s part of Premera’s Wellness Rewards program, designed to get you thinking – and acting –
to improve your health.
How do you earn your reward? Premera will send you a $150 Visa debit card when you
complete just two simple and confidential activities:

Online health risk assessment during <<Month--201X>>

Biometric screening by <<month—date—201X>>
What’s a health risk assessment? It’s a quick way to evaluate your health status and risks by
answering questions about your lifestyle, habits, and personal and family medical history.
What’s a biometric screening? It’s a quick way to learn some important numbers about your
current health:

Blood pressure

Blood sugar

Cholesterol

BMI (body mass index)
How do you get your private and confidential screening (results are not shared with
either your employer or your health insurance company)? Four options:

Get Screened at your doctor’s office
Complete the Biometric Screening at your doctor or primary care provider’s office. Show your
Premera member card. Your doctor will have to complete the Provider Biometric Screening
Form [link to PDF of form]. Then you or your provider can fax the completed form to [TBD] in
order to qualify for a $150 Visa® debit card.

Get Screened at Walgreen’s
Find your nearest participating Walgreens by visiting Walgreens.com/findastore and checking
the “Health Tests” box, or calling 877-926-3968. Appointments are recommended, so contact
your nearest participating location to schedule one. You will need to bring a photo ID, your
Premera member card, and a printout of the Biometric Screening Voucher [link to PDF of form]
to your screening.

Complete a home test kit
Premera has partnered with Summit Health to provide a home test kit. You can complete it in
the privacy of your own home and mail in your results. The kit comes with a postage paid
addressed envelope. Order your home test kit here [link to Summit Health site]
You must order your Home Test Kit by the end of the first month after you enroll in your medical
plan. Please complete it and mail in your results by the middle of the following month to qualify
for your $150 Visa® debit card.

Onsite Screening
Your employer may offer a Biometric Screening at your work place. Check with your employer
on whether or not this option is available.
What’s the point? When you know your numbers, you get a picture of your current health.
Combined with Information from your health risk assessment, the picture gets even clearer. The
WebMD website can help you feel better, work better, and play better with tips, resources, and
information about wellness. You can also find more health tips and tools when you register on
premera.com.

First-time visitor to the site?

Use your 11-digit Premera ID number, including 01 as the last two digits, without a dash.
Example: 12345678901. For questions about correct due dates to submit your screening
results, please contact your Benefit Administrator.

Create a personal username and password, set your security questions, and provide
your email address.
Returning user – forgot your username or password?


Select “Forgot your username or password?” and follow the instructions.

