Thank you for choosing Stop and Go Driving School. We look forward to working with you. Attached you will find our
Registration form, and participation policy, if you would, please fill these out and bring them to class with you, this will
help expedite the check in process.
You will also find information on the steps to take to obtain your permit/license, AZ laws pertaining to permit and driver
license.
You can get your application needed to obtain your permit and
license at
http://mvd.azdot.gov/mvd/formsandpub/mvd.asp . You may print this and fill it our prior to visiting MVD for your
permit or license. (EXCLUDING SIGNATURE , this must be done in front of MVD personnel when at the MVD office.
A copy of the MVD driver’s manual can be found at http://www.azdot.gov/mvd/custsvcguide.asp should you wish to
print this to begin overview or for reference later on. One will be provided for use at class but will remain at the school.
Once you have taken your certificate to MVD and obtained your instruction permit or if you already have a valid Arizona
permit you may schedule your Behind the Wheel sessions at http://www.Stopandgo1.com/driveSched-form1a.asp .
You will need your permit information, phone number and email address given at the time of registration. If you have
any questions prior to class or your driving session please feel free to contact our office and we will be happy to assist
you. Our office hours are 08:00am-5pm Monday – Friday. 08:00am-4:00pm Saturday and Sundays.

Traffic Safety Education
Participation Policy
The goals of this class are threefold: First, to earn a Drivers Education certificate; Second, to develop defensive
driving attitudes and practices; Third, and perhaps most important, to develop a positive, mature, and responsible
attitude concerning interaction with the highway transportation system.
Responsible behavior and punctual attendance are your ways to demonstrate interest in and commitment to
achieving these goals. Please discuss with the instructor if you will be late for a class. We understand that
emergencies may happen and we will work with the student to help them reschedule the sessions. To that end, the
following policy has been established for all Driver Education programs with our school.
CLASSROOM:
1) It will be expected that students will be in their seats and ready for class at the designated starting
times (I. E.) Morning and afternoon breaks and after lunch.
2) Each student is responsible for having appropriate paperwork completed and available at the time of
class.
BEHIND-THE-WHEEL
1) Driving schedules will be given when you call the Stop and Go office, 480-837-9606. It will be the
responsibility of the student to resolve any schedule conflict that may arise with their individual on-street
instruction. The office must receive a minimum of 48 hours notice prior to canceling or rescheduling any driving
sessions or a $50 fee will be issued.
2) Each student must bring their valid Arizona Instructional Permit to each driving session.
BEHAVIOR:
1) All students are expected to exhibit positive and mature behavior. If a student exhibits inappropriate
behavior, instructors will take proper disciplinary measures. Repeated violations of behavior standards will
result in removal from the program.

INSTRUCTIONS FOR OBTAINING DRIVER LICENSE PERMIT
Below you will find a checklist of items you will need to take to the Motor Vehicle
Division (MVD), in order to receive your Temporary Driver License Instruction Permit.
Arizona state law requires all applicants submit proof of identification. You must
present two forms of identification – one must have your photo (or three forms of
identification if no photo is available). At least one document must be from the Primary
list, located on pg. 13 in your Customer Service Guide. All must be originals or certified
copies made by the issuing agency. All must be in English.

Driver Education Certificate of Completion (COC)-signed by instructor
Completed Driver License application. This must be filled out in black ink only.(Do not pre-sign
application-Signatures must be witnessed by a MVD agent or Notary Public)

Your 2 pieces of ID (3 if no picture, see MVD Customer Service Guide)

Applicable fees (see MVD Customer Service Guide)

You will be given a vision examination after your application has been reviewed and accepted. If your
COC is altered in any way MVD will not accept it. In the event you lose your COC, you must contact
the issuing school so another one can be issued to you. A $10.00 fee will be charged for replacement
of any lost COC.
Please keep in mind students with permits only, must drive with a licensed driver over the age of 21
years and seating in the seat next to them..
