What is an Aging & Disability Resource Center?
Aging and Disability Resource Centers (ADRCs) are the first place to go to get
accurate, unbiased information on all aspects of life related to aging or living with a
disability. ADRCs are friendly, welcoming places where anyone -- individuals,
concerned families or friends, or professionals working with issues related to aging or
disabilities -- can go for information specifically tailored to their situation. The ADRC
provides information on broad range of programs and services, helps people
understand the various long term care options available to them, helps people apply for
programs and benefits, and serves as the access point for publicly-funded long term
care. These services can be provided at the ADRC, via telephone, or through a home
visit, whichever is more convenient to the individual seeking help.

Resource Directory is a helpful guide providing information regarding a wide variety of
services available in the area.
We hope you find this directory to be a useful tool. Please SAVE this directory and
refer to it when looking for local resources. The resources listed are just a snapshot of
over 8,000 local, state, and national resources available at the Aging & Disability
Resource Center serving Adams, Green Lake, Marquette, and Waushara Counties.

ADRC Governing Board
The role of the ADRC Governing Board is to:
 Assist in developing a mission statement for the ADRC Consortium that is
consistent with the goals of the State-wide redesigned long-term care system.
 Help determine the structure, policies, and procedures of the ADRC Consortium
within State guidelines and local government structure.
 Ensure that the ADRC Consortium has a viable plan for implementation and
operation.
 Oversee the operation of the ADRC Consortium.
 Identify unmet needs and prepare plans to meet them.
 Assure input from consumers, service providers, and local constituents in general
policies, practices, and goals for the ADRC Consortium.
 Provide financial oversight for the ADRC Consortium including assisting in the
development of a budget and monitoring expenditures.
 Ensure that the terms for the State contract are fulfilled.

Access: Your Key to Health, Nutrition & Other Programs
ACCESS is a fast, easy-to-use internet tool that can help you or someone you know to:
 Find out which low or no-cost health, nutrition and other programs they might be
eligible for.
 Apply online for FoodShare, Medicaid/BadgerCare and the Family Planning
Waiver Program.
 Get up-to-date information about the status of their FoodShare,
Medicaid/BadgerCare, Family Planning Waiver Program, SeniorCare or
Caretaker Supplement benefits.
 Report a job change online.
 Get answers to common questions about benefits.
How to use ACCESS
To use ACCESS, open a web browser and go to http://access.wisconsin.gov. Click on
the button for the tool you would like to use and follow the instructions on the screen.
Keep in mind that all of your information will stay private and secure.
ACCESS gives you four tools for getting and managing health and nutrition benefits:
Am I Eligible? will ask some basic questions about your family, your money and your
bills. Based on your answers, it will tell you if you might be able to get help with the
following health, nutrition and other programs such as: FoodShare, Medicaid,
BadgerCare, SeniorCare, and other prescription drug assistance programs,
Free/Reduced-Price School Meals, Summer Food Service Program, The Emergency
Food Assistance Program (TEFAP), Special Supplemental Nutrition Program for
Women, Infants and Children (WIC), and Special tax credits
Apply For Benefits is an online application for FoodShare, Medicaid/BadgerCare
and/or the Family Planning Waiver Program. It will ask detailed questions about your
family, your money and your bills. You may need to talk with someone at your local
agency after you submit an application.
Check My Benefits lets you get up-to-date information about your FoodShare,
Medicaid/BadgerCare, Family Planning Waiver Program, SeniorCare and Caretaker
Supplement benefits.
Report My Changes You can use ACCESS to report job changes to your worker.

Elders at Risk
Definitions of Elder Abuse:
 Physical abuse is the willful infliction of physical pain, injury or unreasonable confinement. It
includes, but is not limited to, beating, choking or burning, inappropriate medication or tying or
locking a person up. It also includes sexual abuse, which occurs when a person has been forced,
tricked, threatened or otherwise coerced into sexual contact against one’s will.
 Material abuse, sometimes called financial exploitation, is the misuse of an elder's money or
property. It includes deception, diverting income, mismanagement of funds and taking money or
possessions against a person’s will.
 Neglect occurs when a caregiver's failure to provide adequate food, shelter, clothing, medical or
dental care results in significant danger to the physical or mental health of an older person in
his/her care.
 Self-neglect means a significant danger to an elder person's physical or mental health because the
elder person is unable or fails to provide him/herself with adequate food, shelter, clothing, medical
or dental care.
Reporting suspected elder abuse: When reporting any type of elder abuse, one should call the nearest
Aging & Disability Resource Center. Adult Protective Services is the only public service authorized and
empowered by state laws to respond to and investigate allegations of abuse, neglect and exploitation of
elderly and vulnerable adults.
The caller’s confidentiality is protected by law: No person may be held civilly or criminally liable or be
found guilty of unprofessional conduct for reporting in good faith. If possible, callers should be prepared to
report what happened, when and where it happened, and who the suspected abuser might be. When
reporting any type of abuse in Adams, Green Lake, Marquette, or Waushara County, one should call the
nearest Aging & Disability Resource Center.
Services available: The principal purpose of an elder abuse investigation is to determine the validity of
the report and to determine if the person is in need of protective services. Every report is checked
promptly because of personal safety. Every effort will be made by the lead elder abuse agency to offer
services which will maintain the older person in his or her home. If qualified, some services which may be
offered are medical services, day or respite care, home-related services (meal deliveries, home repairs,
friendly visitors, etc.), transportation, dietary assistance, counseling, support groups, referral to sources of
financial assistance or legal assistance, mental health services and criminal justice interventions.

Long Term Care Services
LONG TERM SUPPORT PROGRAMS IN GREEN LAKE, MARQUETTE, AND WAUSHARA COUNTY:
What is a Managed Care Organization? When a person becomes eligible and decides to enroll in
Family Care or IRIS, they become a member of a managed care organization (MCO) which manages and
delivers their benefit. This benefit combines funding and services from a variety of existing programs into
one flexible long-term care benefit tailored to each individual’s needs, circumstances and preferences.
Who is eligible? MCOs serve people in three primary target groups who have long-term care conditions
expected to last for more than 90 days and are on medical assistance. The three target groups are:
1.
Frail older adults (65 years and older)
2.
People with physical disabilities (17 years, 9 months or older)
3.
People with developmental disabilities (17 years, 9 months or older)
What is Family Care: Family Care is a program that provides a full range of long-term care services, all
through on flexible benefit program. Long-term is any service or support that a person may need as a
result of a disability, getting older, or having a chronic illness that limits the ability to do the things that
people need to do throughout the course of the day. There are a variety of services and supports
available in Family Care that can help people do these things independently or with the support of
someone else.
How does Family Care work? When you are a participant in a Family Care program, a team of people
come together to help you identify what sort of assistance you might need and work with you to arrange
your long-term care services. You are an active participant on the team that also includes, at a minimum,
a care manager and a registered nurse. You may choose to include family members or loved ones on
your team.
 People participate in determining the service they receive
 People choose service providers from a comprehensive network.
 People receive the services they need through one benefit.
 People receive services that best achieve the results they desire.
What is IRIS? IRIS stands for Include, Respect, I Self-Direct. It is a Wisconsin program where you are
able to self-direct your publicly funded, community-based, long-term care supports and services.
How does IRIS work? An individual budget is established based on your needs. With that budget in
mind, you will develop your individual support and service plan. Friends, family, or others may help you
manage your IRIS budget and plan. The Independent Consultant Agency supports you to develop your
plan and will provide detailed information about IRIS. You make your own purchases and hire services
providers directly, or with the help of an employment agency. You authorize payments to your providers.
The Financial Services Agency pays the bills promptly and does all of the employee-related paperwork.

Long-Term Support Programs in Adams County
Community Options Program (COP)
Community Options helps people who need long term care to stay in their own homes and communities.
Its purpose is to provide cost-effective alternatives to expensive care in institutions and nursing homes.
Elderly people and people with serious long-term disabilities receive funds and assistance to find services
they are not able to get through other programs. Community Options care managers know what services
are available in the community, and learn what families and friends are able to do. A care manager will
talk to you, or your family member about how to arrange the services and supports you need to avoid
going to a nursing home.
Community Integration Program (CIP)
The Community Integration Program is a Medicaid Home and Community-Based Waiver for adults with
developmental disabilities.
In general, people who have a developmental disability are able to lead very productive lives in their
community. People are able to access integrated employment, live in settings such as supported
apartments, utilize the local transportation system, and access local doctors, clinics and hospitals. In order
to access the county-operated Community Integration Program Waiver (CIP Waiver), the person has to
meet the Federal Definition of Developmental Disability.
The CIP Waiver helps people with developmental disabilities to stay out of institutions, or to relocate from
state centers and nursing homes back to their communities. In many circumstances, CIP can help
prevent someone from having to leave his/her community. CIP is funded through the federal Medicaid
Program (MA). It is known as an "MA waiver" because the federal government has waived certain
regulations, allowing Wisconsin to use the dollars to follow people to the community. The money goes
from the state to county waiver agencies who administer the program.
What is the application process? The Aging & Disability Resource Center (ADRC) will be able to assist
applicants with determining their eligibility for all programs. Please call your local office to speak with an

Home Care-Skilled vs Non-Skilled Services
Our directory has a section for Home Health Agencies that provide either skilled, non-skilled or both
services. Below is a brief description explaining the differences:
Skilled Nursing:
These services provides medically necessary skilled nursing that is provided by a medical professional in
your or your loved one’s home when prescribed by a physician. This includes nursing, physical therapy,
occupational therapy and speech-language therapy. If individuals are eligible for skilled services, they can
also receive part-time assistance with personal care needs from a home health aide. You or your loved
one are required to be “homebound” as a condition for these services. Home health is covered under the
Part A Medicare or Medical Assistance.
These services may include but is not limited to:
 Initial assessment and evaluations
 Pre- and post-operative care
 Skilled nursing assessment with obtaining and monitoring vitals
 Catheter care/change
 Diabetic care
 Infusion therapies
 Lab draws/injections/IV therapies
 Wound care/dressing change
 Wound vac competent care
 Tube feeding, G-tube care and teaching
 Orthopedics
 Pulmonary care
 Medication management and administration
 Central line and port maintenance
Non-skilled services:
Non-skilled services include homemakers, companions and chore persons to assist individuals who need
help with things such as meal preparation, shopping, escort to doctor visits and other errands. They also
can provide help with laundry, light housekeeping, and companionship. You can obtain services through
agencies by privately paying. Please contact the agencies directly for details about the services they offer
and the costs involved.

The Living Well with Chronic Conditions program, developed by
Stanford University, will help you develop the skills needed to take
control of your health condition and live a happier and healthier life.
Participants will learn how to:

Better communication with health providers

Falling is NOT a normal part of Aging!
Falls can be devastating, cause injury, fear, lifestyle changes, and loss of independence. Falls
are NOT a normal part of aging, and the GOOD NEWS is…they can be prevented! The Aging
and Disability Resource Center offers 3 programs designed to prevent falls, decrease the fear of
falling, and/or increase activity levels.






Medicare Part D, Prescription Drug Options

Help… What plan is best for me?
Whether you have Medicare A, B, D, supplemental insurance, a Medicare Advantage plan (with or without Part D coverage),
SeniorCare, Employer/Retiree Group plans, Veteran's Benefits, Federal Employer Health Benefits, you are being presented with
choices. These options generate the question: "Which coverage or combination of coverages work for me?"
The big question is whether or not your current coverage works for you and whether or not it will change for the coming year. You
certainly want medical coverage and should seriously consider if you need prescription coverage. Do you have a drug plan now?
Does it work well for you? Can you afford it? Will it work for you next year? If you feel the need to review your options, these are
some things to consider:
If your employer/retiree group plan includes prescription coverage, it may be as good as or better than the Part D drug plan,
which means you do NOT need to enroll into the Medicare drug plan. Compare your out-of-pocket costs for your drugs from one
plan to the other and be sure to ask, “What will happen with the rest of my medical coverage if I decide to make a switch?”
Medicare Advantage plans are another way to get your Medicare coverage (they replace original Medicare and the supplement). The
co-pays and maximum out-of-pocket costs vary from plan to plan, so compare the plans’ costs and premiums. (Note: These plans
can change year to year. They may or may not include Part D coverage.) Annual enrollment in 2011 will be from October 15 thru
December 7, 2011. There is also an "Open Enrollment Period" from January 1 thru February 28 where you may make a "like" change.
You can only make ONE election during these periods, so be sure you understand your options prior to agreeing to enroll into any
plan.
If your Part D Prescription Plan is changing or no longer meeting your needs, you have until December 31st to review and elect the
Part D drug plan that meets your needs for the coming year. Or you may compare your options with SeniorCare (if eligible) or any
other "creditable" drug coverage you may have (i.e., VA benefits or employer-sponsored coverage).
Please be sure to base your decision for your health care coverage on your own needs and preferences, not on what works for
someone else. These are just a few questions people should consider when deciding which coverage works well for them. If you

Plan Ahead for Emergencies

Considerations for Older Adults
Get Ready Now:
 Make sure you have basic supplies for survival for at least three days in your emergency supply kit.
 Prepare an emergency medical kit to include a 2-week supply of medication. medical equipment such
as hearing aid batteries, written treatment information such as an up-to-date list of all medications
you’re taking and the doses, as well as a list of any medical problems you have and how they’re being
treated.
 Update your kit at least twice a year – use Daylight Savings time changes each spring and fall so you
don’t forget.
 Make a plan for every detail of your daily routine; then make an alternative plan.
 Share your plan with family, friends, and neighbors.
 Learn about likely disaster in your area so you know what to prepare for.
Special Considerations for Caregivers:
 Name a substitute caregiver in case you are unable to provide care.
 Show others how to operate necessary medical equipment (such as oxygen). Keep written directions
with the equipment.
 Keep a “daily planner” of routines.
Important Questions to Consider:







Is my emergency supply kit updated and easily accessible?
If I have to evacuate my home, where do I go? How do I get there if I don’t drive?
Does the facility I live at have a plan for emergencies?
Do I rely on electricity for dialysis, oxygen, etc.?
What if I am unable to call or signal for help?
What if I don’t have any friends or family to help me?
What happens to my pets if I have to evacuate my home?

Your Benefits - Elderly

An Elderly Benefit Specialist is a person trained to help people ages 60 or older who are having a problem with
their private or government benefits. They help older persons figure out what benefits they are entitled to and
tell them what they must do to receive them.
Elderly Benefit Specialists receive ongoing training and are monitored by attorneys knowledgeable in elder
law. The attorneys are also available to assist older persons in need of legal representation on benefit matters.
Who should seek help from an Elderly Benefit Specialist?

Older persons who just want to know more about any private or public benefit.
Older persons who need help in organizing the paperwork to apply for benefits.
Older persons who have been denied a benefit that they think they are entitle to receive.
What can Elderly Benefit Specialists do?

Provide accurate and current information on your benefits.
Suggest alternative actions that you can take to secure benefits or appeal denials of benefits.
Advocate on your behalf with other parties.
Explain what legal action or other possible solution is required.

Your Benefits - Disabled

The Disability Benefit Specialist (DBS) works closely with information and assistance, options counseling, and
other resource center services. The DBS provides services to people ages 18 to 59 with physical disabilities,
developmental disabilities, mental illness, and substance use disorders.
How can a Disability Benefit Specialist help you?






Provide accurate information on public and private benefits and programs, including assistance with
application and appeal procedures;
Provide assistance in determining which benefits and programs you may be entitled to;
Help you to fill out complicated applications for benefits, programs and services;
Talk with you about your choices and the programs or services that might best meet your needs;
Provide referrals, assistance and support on other matters including disability rights and the impact of
earnings on financial eligibility for public benefits;
In some cases, you may also receive representation in formal appeals related to denials of eligibility,
termination of benefits or overpayment of public and private benefits.
