Champions of Supportive Housing Awards
Nomination Form
CSH is seeking nominations for its 2014 Champions of Supportive Housing Awards. Awardees will be honored at the
CSH Supportive and Integrated Housing Luncheon on January 8, 2014. Nominees may be individuals or
organizations. Agencies may only submit one nomination form per award category. Award selection is performed by
CSH.
Awards categories include:
Champion of Supportive Housing – Tenant Leadership Award
Nominees are current or former tenants of supportive housing who have exerted leadership within the supportive
housing community.
Champion of Supportive Housing – Supportive Housing Provider Award
Nominees of this award have accomplished a significant achievement or made a notable effort to achieve quality or
innovation in its service delivery that has resulted in a significant impact on addressing the needs of vulnerable
populations. This may include operating housing that is attractive, sustainable, functional, appropriate for the
surrounding community, and conducive to tenants’ stability, and where tenants are offered services that are tenantfocused, effectively address tenants’ needs, and foster tenants’ housing stability and independence.
Champion of Supportive Housing – Community Partner Award
Nominees of this award have played a significant role in advancing supportive housing funding, policy, community
partnerships, or other areas of impact. Nominees may include public or private funders, private market landlords or
owners, housing developers, and community advocates. Nominees will have dedicated time, leadership and resources
to the advancement of supportive housing.
