Hill College Health Science Application Procedure Vocational Nursing Spring 2014 Application Deadline: November 1, 2013 The Health Science departmental application packet will be available on the departmental website www.hillcollege.edu/student/healthscience/vocational_nursing or in the Health Science Department, after July 1, 2013 Prospective students must contact the Health Science Department to make appointment to pick up or submit the Health Science Vocational Nursing application packet. Email: tpalinkas@hillcollege.edu After submission of the completed packet the applicant will be given the HESI A2 with Critical Thinking Referral form. The applicant must have the completed referral form at the time of their scheduled HESI A2 with Critical thinking test. **TESTING NOTES** **You will not be able to test on Fridays, at a Hill College testing center beginning May 13, 2013 through August 9, 2013, as the campus is closed. ** The absolute last testing session for the HESI, for this testing block, will be noon October 30, 2013. **You may only take the HESI test 2 (two) times in an application block. Documents and/or files will be purged after 1 year excluding the following: o Course evaluations o Course waivers o Transcripts Candidates who are enrolled in one or more of the pre-requisite courses and/or incomplete files will be considered for acceptance at a lower priority than candidates who have completed the application process as well as completed the pre-requisites with a “C” or better by the admission deadline date. **Note: It is the policy of Hill College, for any course re-taken, the most recent grade received will stand. ** Submission of this application packet does not guarantee admission/selection in the Vocational Nursing program. Please contact the Health Science department regarding the completion of this requirement. _____ CPR with skills – American Heart Association BLS/Health Care Provider. Must last entire nursing school year. _____ Official High School Transcript or GED Scores _____ TOEFL Policy: Students must demonstrate English proficiency as an admission requirement. This may be demonstrated by one of the following: 1. Attended four years of high school in one of the 50 United States and receiving a diploma or 2. Attended two years of high school in one of the 50 United States and passed the GED or 3. Acceptable GED scores from one of the 50 United States and acceptable scores from the Test of English as a Foreign Language Test (TOEFL) within the two years of the posted deadline date of program application. TOEFL Waiver: The student can request a TOEFL waiver form from the Health Science department if they meet the following criteria: completion with a “C” or better of 4 different college level English courses and 1college level communication course; from an accredited US college/university. TOEFL minimum Internet score of 80 or a computer-based TOEFL score of 250 or a paper/pencil test score of 600. Scores will not be considered if they are more than two years old. TOEFL Test score reports must be sent directly to the Health Science Department from the testing center. Note: You must show that you have completed the required pre-requisites: BIOL 2401 Anatomy & Physiology I and BIOL 2402 Anatomy & Physiology II and General Psychology PRIOR to the deadline date. Note: The student must show proof of 2 MMR’s, Tdap (documentation of 1 shot in lifetime), Tetanus (documentation of booster within the last 10 years), Varicella vaccine x2, flu vaccine, and Hepatitis B series. The student needs to be aware that the Hepatitis B series will take 4-6 months to complete. Revised: June 2013 Hill College Nursing Programs ANY candidate with eligibility issues related to criminal convictions seeking consideration for a Hill College Nursing Program must be aware of the following: *** Applicants with an eligibility issue must provide to the Hill College Health Science program documentation that they have completed the declaratory order process prior to the posted deadline date. Applicants with pending declaratory orders at the posted deadline date will be considered at a lower priority for selection. Texas Board of Nursing (BON) rules and regulations regarding eligibility issues can be found at http://www.bon.texas.gov/exam-eligibility.htm. Failure to disclose an eligibility issue prior to admission into the nursing program WILL result in AUTOMATIC DISMISSAL from the nursing program. Eligibility Issues Applicants who answer “YES” to any of the following questions must download and complete the Declaratory Order application using this link: http://www.bon.texas.gov/olv/pdfs/DOapp.pdf. The Board of Nursing Review of applicants with eligibility issues can take three (3) to six (6) month to complete. For any criminal offense, including those pending appeal, have you: A. been convicted of a misdemeanor? B. been convicted of a felony? C. pled nolo contendere, no contest, or guilty? D. received deferred adjudication? E. been placed on community supervision or court-ordered probation, whether or not adjudicated guilty? F. been sentenced to serve jail or prison time? court-ordered confinement? G. been granted pre-trial diversion? H. been arrested or have any pending criminal charges? I. been cited or charged with any violation of the law? J. been subject of a court-martial; Article 15 violation; or received any form of military judgment/punishment/action? (You may only exclude Class C misdemeanor traffic violations.) NOTE: Expunged and Sealed Offenses: While expunged or sealed offenses, arrests, tickets, or citations need not be disclosed, it is your responsibility to ensure the offense, arrest, ticket or citation has, in fact, been expunged or sealed. It is recommended that you submit a copy of the Court Order expunging or sealing the record in question to our office with your application. Failure to reveal an offense, arrest, ticket, or citation that is not in fact expunged or sealed, will at a minimum, subject your license to a disciplinary fine. Non-disclosure of relevant offenses raises questions related to truthfulness and character. NOTE: Orders of Non-Disclosure: Pursuant to Tex. Gov’t Code § 552.142(b), if you have criminal matters that are the subject of an order of non-disclosure you are not required to reveal those criminal matters on this form. However, a criminal matter that is the subject of an order of non-disclosure may become a character and fitness issue. Pursuant to other sections of the Gov’t Code chapter 411, the Texas Nursing Board is entitled to access criminal history record information that is the subject of an order of non-disclosure. If the Board discovers a criminal matter that is the subject of an order of non-disclosure, even if you properly did not reveal that matter, the Board may require you to provide information about any conduct that raises issues of character. Are you currently the target or subject of a grand jury or governmental agency investigation? Has any licensing authority refused to issue you a license or ever revoked, annulled, cancelled, accepted surrender of, suspended, placed on probation, refused to renew a license, certificate or multi-state privilege held by you now or previously, or ever fined, censured, reprimanded or otherwise disciplined you? *Within the past five (5) years have you been addicted to and/or treated for the use of alcohol or any other drug? *Within the past five (5) years have you been diagnosed with, treated, or hospitalized for schizophrenia and/or psychotic disorder, bipolar disorder, paranoid personality disorder, antisocial personality disorder, or borderline personality disorder? Hill College Health Science Department Criminal background requirements The Hill College Vocational Nursing Program has implemented the following change to the application process related to the background check process. All applicants seeking entry in the Vocational Nursing program must complete the finger printing process through the MorphoTrust USA (formerly L1 Solutions). The cost of the fingerprinting is $41.45, non-refundable. The new process will be as follows: 1. Submit application 2. Once a month during application timeline, a roster with a list of all applicants will be sent to the Board of Nursing 3. Approximately 10 days after submission of the roster, applicants will be sent an email with Fast Pass application to make an appointment for fingerprints. 4. Fingerprints must be done with receipts submitted to the nursing department before deadline date of November 1, 2013 Please note that following Vocational Nursing application submission dates: *July 1- July 28, 2013 *August 1 - August 29, 2013 *September 1 - September 30, 2013 *October 1 – October 17, 2013 *Example: John Doe submitted his application postmarked August 1, 2013 Jane Doe submitted her application postmarked August 20, 2013. Approximately 10 business days after August 29, 2013 Hill College Health Science will send email confirmation to John and Jane Doe that their application was received; the email will include the MorphoTrust USA (formerly L1 Solutions) Fast Pass fingerprint application. The student must begin the fingerprinting process as soon as they receive the Fast Pass. October 17 - October 31, 2013 Applicants who submit their application October 17-October 31 will be considered for selection but at a lower priority than those who have the L1 process is in progress. >>>Early submission of applications is not a guarantee of selection in to the program. All required application documents must in file by the November 1, 2013 deadline date. >>>Applicants who try to complete the MorphoTrust USA (formerly L1 Solutions) without the Hill College Vocational Nursing MorphoTrust USA (formerly L1 Solutions) FastPass email approval will risk having their background check process delayed and/or application denied by the Health Science Vocational Nursing program.
