HUBUNGAN PELAKSANAAN VERIFIKASI READ BACK UNTUK PRESCRIBING
ERROR DENGAN KEJADIAN DISPENSING ERROR PADA PASIEN RAWAT INAP DI
RUANG ANGSOKA I, II DAN III RSUP SANGLAH DENPASAR
ABSTRACT
Introduction : Medication error is a major source of potential harm to patients. Error on
dispensing phase (dispensing error) occurs at the time of preparation until the delivery of a
medication by a pharmacist. Communication failures and misinterpretations between the
prescriber and the dispenser, also failure to identify correctly some drugs whose names sound
almost the same or similar to one another is one of the causes of dispensing errors. Read Back
Verification technique is necessary as a good way to verify the information, especially when
copying doctor's orders. Method :This research attempts to analyze the correlation between Read
Back verification to incident of Dispensing Error using descriptive-correlation design. This
research observed the implementation of Read Back verification on 30 samples of inpatient’s
KIO and then matched them with data of Dispensing Error incidents that obtained through
Patient Safety Monthly Report. Result : Results obtained when analyzed with chi-square test is p
= 0.000 (p <0.005) which means that there is a correlation of Read Back verification with
Dispensing Error incident in hospitalized patients. Discuccion :So that, supervision of
implemented Read Back verification is necessary to do as an effort to prevent Dispensing Error
incident occurred.
