The Williamsport office does not have regular hours as it is the home base for Options Counselors and Case Managers who are often out of the office working with clients. Comprehensive Service Description: Information and Referral Services daily through the Lafayette office for persons with aging or disability-related questions or needs. We maintain a database of community resources in Fountain and Warren counties and other areas to assist you as well as listings of resources. All screenings and referrals for Area IV funded services are done through the Lafayette office; Home delivered meals – depending on your location; and hot noon meals or frozen meals through the senior centers; Pre-admission screening to enter nursing facilities – must contact the Lafayette office for a referral to this service. Must meet physical or cognitive disability requirements – an assessment will be done. Home Health Services are designed to help people remain in their homes rather than enter a nursing home. These services are funded through Federal and State monies and require disability and financial assessments to determine eligibility. o Funding sources include: Aged and Disabled Medicaid Waiver (any age with many disabilities), must meet income and asset requirements; CHOICE funding for persons of any age with many disabilities, may have a co-pay if income is over 150% poverty level; Social Services Block Grant/TXX for persons over 60 or those under age 60 with disability, income under 300%; Title III of Older American’s Act for persons over age 60, no income restrictions. o This assessment is originally done through the Lafayette office. However, the Options Counselor who comes to the home to assess you and have you sign an application for services is based out of the Williamsport office once a referral is made from the Lafayette office. And the long-term care Case Manager who will continue to help you with services once you are approved is also based out of the Williamsport office. Public Transportation – (no age limit or disability required) is offered through the REACH OUT Community Van for the towns of Hillsboro, Newtown, Wallace & Mellott and surrounding areas by calling 765-798-2815 Janet Keeling; an public transportation (no age limit or disability required) is offered through Boswell Area Transit Van (BAT VAN) by calling 765-869-4350 (Senior Center) 3 or 765-869-5184 to serve the towns of Boswell, Oxford, Ambia, Dunnington, Chase, Tab, & Talbot; Special Transportation for Benton County residents 60 years & older, with mobility impairments, trips are for medical purposes only through “HOPE” Transportation Program by calling 765-404-1174 Kim Bowley and leaving a message, she checks her messages daily. Please give at least 48 hours notice; The Family Caregiver Program assists caregivers of older and/or disabled persons by offering information, education, support groups, and respite and supplemental services to lessen the stress on a caregiver; SHIP (Senior Health Insurance Program – Indiana) counseling for Medicare; Private Pay Care Coordinator to assist persons age 55 and older who are able to pay for someone to assist them with finding and coordinating the services they need to remain independent; Wellness Classes and Activities: a) EnhanceFitness – a 6 week course for older persons. Call to see if it is being offered in your area; b) Matter of Balance classes to help prevent falls – call to see if it is being offered in your area; c) Chronic Disease Self Management – a 6-week course designed to help persons with chronic diseases; d) Senior Games – held in Tippecanoe County – but you are welcome to participate. Held in June every year; e) Wacky Wednesdays at the Movies – for $1 you can attend a movie and receive a snack – held in Lafayette movie theater. SERVICES AVAILABLE: Care Coordination is free. HIV related case management for HIV positive individuals residing in service area. Offers referrals for medical care, housing, support, medications, and other needs. Special Population Support Program offers free HIV prevention education in community-call to schedule. Also offers free HIV testing for those who are at increased risk due to substance abuse. Referrals provided to those who are identified as HIV positive. Free HIV case management, referrals: medical services, housing, support, education. Also offer free HIV testing and education for at-risk individuals involved in substance abuse. Call to schedule an appointment, must provide proof of HIV positive status. Service areas are Benton, Carroll, Clinton, Fountain, Montgomery, Tippecanoe, Warren, White, Jasper, Newton counties.
