Elizabeth Hertel, Director of Health Policy and Innovation with
the Michigan Department of Health and Human Services, will
be the Consortium’s guest speaker at the Friday, October 30,
quarterly Board and membership meeting.
She will provide a detailed update on the State Innovation
Model (SIM).
Guests are welcome to attend this meeting which begins at 9
a.m. in the MPHI Interactive Learning Center (2436 Woodlake
Circle, Okemos) or tune into the webcast of Elizabeth’s
presentation beginning at approximately 10:30 a.m.

The Consortium is working collaboratively with several
organizations to conduct a PCMH Symposium. This symposium's focus is Advancing the Patient Centered Medical
Home - focusing on team-based care and integration of
pharmacists, behavioral medicine, and care managers in
the primary care setting. Several take-away models will be
provided, and the educational experience is planned to
benefit all members of the primary care team.

RE-SET YOUR CLOCKS: DAYLIGHT SAVINGS
TIME
At 2 a.m. on Sunday, November 1, be sure to reset your
clocks back one hour to 1 a.m.
In addition, to routine clocks, don't forget your vehicles,
various appliance clocks, and timers which may regulate machinery and devices in your residence and work
site.
Remember your batteries in smoke alarms as well.

EDUCATION TO PRACTICE (E2P)
Education to Practice is a statewide initiative encouraging health
educators and professionals to work together to increase the
number of high-functioning health care teams capable of caring
for the whole patient. Using existing staff and students, with very
little additional cost, clinics can adopt an E2P model to improve
access and quality of patient care.
This fall, E2P plans to visit more sites utilizing interprofessional
best practices in order to develop resources and collect data that
will help healthcare professionals achieve the Triple Aim of
