How to get your Cooper Tire rebate:
1. Complete the mail-in rebate form below. This offer is not transferable, so the name on the rebate form must match the
name on the sales receipt. Please print clearly in large capital letters. This is the name that will appear on your VISA® prepaid rebate
card (or check if purchased in Canada).
2. Include the sales receipt, which must show the purchase date, qualifying Cooper Tire, purchase price and tire size. Circle the tire
purchased, which must be one of the Cooper Tires listed above. Substitutions are not eligible. Please do not use staples when
sending your information. Keep copies of all documents for your records.

COOPER TIRE OFFICIAL REBATE TERMS AND CONDITIONS:
1. This offer is open to legal residents of the 50 United States, the District of Columbia and Canada (excluding Quebec), making purchases in those
locations. The rebate offer is not transferable and is valid only for the individual who purchased the product for personal household use. Void in
Quebec and where restricted or prohibited by law.
2. US: Approved U.S. claims will receive the rebate in US dollars in the form of a Visa® prepaid rebate card, to be mailed to the address provided on
this Official Mail-in Rebate Form. Rebate card is issued by JP Morgan Chase pursuant to a license from Visa® U.S.A. Inc. The card requires activation
by the recipient and can then be used at any location that accepts Visa® cards. The card cannot be used for cash withdrawal at an ATM. Bank fees
on the card are waived for the first 90 days following date of issue. After that time, a $3.95 monthly bank fee will be applied by JPMorgan Chase
against the remaining funds on the card. The card will arrive attached to a card mailer. Read carefully all information provided on the mailer
regarding activation and use of the card. This card is not a line of credit and will expire either when all funds have been utilized or 12 months from
the month of issue. Failure to use your Cooper Tire Visa® prepaid rebate card will void the offer and relieve Cooper Tire of any further obligation. Lost
cards will not be replaced after the expiration date.
3. CANADA: Approved Canadian claims will receive the rebate in Canadian dollars in the form of a Rebate Check, to be mailed to the address
provided on this Official Mail-in Rebate Form.
4. This offer is for the purchase of four (4) qualifying new Cooper Tires (in a single transaction) between June 20, 2011 and July 31, 2011. Rebate
requests must be postmarked no later than August 31, 2011 and received at the rebate fulfillment center no later than September 14, 2011. Late
submissions will not be accepted. Please allow 6-8 weeks after Sponsor receives the rebate request for the delivery of your Visa® prepaid rebate card
or check.
5. Offer amount is based on qualifying tires purchased and the availability of eligible new tires at the time of purchase. $50 rebate is based on the
following qualifying tires purchased: the Cooper CS4 Touring, Cooper Discoverer H/T and Cooper Discoverer LSX Tires (available at select retailers).
$75 rebate is based on the following qualifying tires purchased: Cooper Discoverer CTS, Cooper Zeon RS3A, and Cooper Zeon RS3S Tires.
Substitutions are not eligible.
6. Limit of one (1) rebate per four (4) qualifying tires purchased in a single transaction. Purchases must be for household use and is limited to two (2)
rebate claims per person, family, household or address.
7. Requests from groups, organizations or businesses will not be accepted. Offer is not available to the affiliated dealers of Cooper Tire & Rubber
Company (“Sponsor”), their immediate family members or employees. Offer not valid for business or fleet vehicles. Purchaser is responsible for all
applicable taxes and other fees. This offer may not be combined with any other Cooper Tire & Rubber Company offer.
8. To apply, complete the mail-in rebate form. This offer is not transferable, so the name on the rebate form must match the name on the sales receipt.

9. Keep copies of all documents for your records. Disputes will require copies of all submitted materials for verification. Claims of “lost materials” will
not be honored unless accompanied by proof of receipt such as certified mail. All claims of “nonpayment” must be received by the rebate fulfillment
center no later than November 9, 2011. Keep a record of the date your rebate request was mailed. Please allow 6-8 weeks after Sponsor receives the
rebate request for the delivery of your Visa® prepaid rebate card or check. If rebate is not received within 8 weeks, contact a Cooper Tire representative
via email at Coopertirerebate@fahlgren.com or call 1-800-426-2637.
10. Incomplete rebate requests will delay processing. The original documents will be returned along with a letter explaining the reason for
non-payment. Corrected rebate requests must be resubmitted, along with the letter from the fulfillment center, within 10 days of receipt.
11. U.S. mail fraud statutes and the Criminal Code of Canada prohibit acts devised to defraud or obtain money or property by means of false or
fraudulent pretenses if the postal system is involved. This would include, among other things, the use of fictitious or assumed names or addresses in
order to receive multiple offers. Tampering with, altering or falsifying purchase information constitutes fraud. All decisions made by the Sponsor (or
its authorized representatives) relating to the validity of any submissions are final and binding. Sponsor reserves the right to disqualify any individual
who fails to comply with these terms and conditions or who disrupts or otherwise attempts to affect the administration, security, fairness, integrity, or
proper conduct of this promotion. Sponsor reserves the right to confirm the identity of any rebate recipient and is not responsible for delayed,
