Description: Works with Lions Club International (related agency is Northwest Lions Foundation For
Sight and Hearing, agency #1745). Offers new glasses to low-income person. Frames are chosen
from a select group which have been gifted to this program by the parent company. Eligibility:
Requires letter or voucher from a non-profit agency which has a tax ID number or Public Health Seattle & King County. Also needs a eyeglass prescription. You can call any LensCrafters location
in King County check phone listings for numbers.

Open Thursday to anyone over 18 with no medical insurance. ID is required. Please call first day of
month at 9:00 am to schedule an appointment. Clients are encouraged to walk-in for stand-by line or
wait list starting at 7:30 am, may arrive as early as 6:00 am. Thee are only about 8 appointments per
Thursday. Clinic closed July and August. Please call for additional information.

Provides services to anyone with severe vision loss. Programs include counseling, instruction in
safe movement and travel, independent living skills, low vision evaluation and training, adaptive aids
store and volunteer support. Some services are offered at no charge to clients.

Services provided through VISION USA include a comprehensive exam & diagnosis of eye health
and vision problems. Eligibility: Cannot have vision insurance, no eye exam in last two years and
must have a job or live in a household where there is one working household member. Must call for
an application.
