A
n organization can only accomplish its
mission through the diligent help and
support of people working together. The
Medical Council of Canada (MCC) is no exception.
It would be unable to effectively evaluate physicians’
competence or maintain a registry of physicians’
qualifications without the efforts of countless
individuals.
The Council lies at the heart of the MCC, setting policy
direction for the organization. Council members come
from various backgrounds and represent different
constituencies – medical regulators, educators,
students, residents and members of the public. No
matter whom they represent, however, Council
members come together in the spirit of collaboration
and with the express intent of improving physician
assessment.
Once policy direction has been set by the Council,
MCC staff work tirelessly towards implementing it.
They review candidates’ examination applications,
work with examination sites across the country, liaise
with partner organizations in the medical community,
run the IT infrastructure that supports all of the
Council’s activities, research new trends in medical
assessment, and perform a myriad of other roles to
ensure the smooth functioning of the organization.
The test committees form another integral
constituency that supports the MCC’s operations.
Made up of physicians from across the country,
test committees do the arduous work of creating,
reviewing and testing examination content. They
perform the critical function of developing and
approving questions and cases, and consequently
ensure the validity of the Council’s examination
process.
How does the Council know whether it is testing
what needs to be tested on its examinations? Test
committee members, who are medical experts in
their respective fields, represent current medical
thinking in Canada. Their participation is an essential
part of the Council’s quality assurance process for its
examinations.
The MCC is committed to world-class medical
assessment. It provides equal opportunity for all
eligible medical graduates to demonstrate their
medical competence. The people behind the Council,
regardless of their affiliation, are passionate about
providing candidates with that opportunity.
This Annual Report strives to commend every
individual working on behalf of the MCC. Without
their contributions, the Council would be unable to
achieve its mission and objectives.

I
f I am asked to define the Medical Council of
Canada (MCC) in just a few words, I explain
that it is an assessment body that evaluates medical
students and graduates to ensure that they will be
fine physicians once out in practice in Canada. While
the word “assessment” denotes rigorous testing and
examination, it unfortunately does not do justice to the
many contributors who are so vital to our success.
This year, the theme of the Annual Report is The
people behind the Medical Council of Canada because
we wanted to highlight the individuals who play
such a integral role in the delivery of our services.
We also wanted to focus on several significant
accomplishments that will benefit our stakeholders,
including candidates, medical regulatory authorities,
medical educators and researchers.
In 2009, the MCC improved customer service
by merging two separate departments to assist
candidates with all of their inquiries. Recognizing
that physicians want more feedback as they study for
MCC examinations, we also launched an online selfevaluation for candidates preparing to take the Medical
Council of Canada Qualifying Examination Part I.
This self-evaluation is now available on our website
alongside another that had been released in 2005 for
candidates preparing to take the Medical Council of
Canada Evaluating Examination.
The Council is committed to improving fairness and
transparency in the credential recognition process. It
is with great anticipation that we look to collaborating
with the Federation of Medical Regulatory Authorities
of Canada on a Canadian medical registration system
that would allow individuals to use the same process to
apply for a medical licence, whether they are applying
in Alberta or in Nova Scotia.
At the 2009 Annual Meeting, the Council formally
approved the governance model for the National
Assessment Collaboration. This was the culmination
of several years of hard work to implement a key
recommendation from the 2004 Report of the
Canadian Task Force on Licensure of International
Medical Graduates (IMGs) - to develop a standardized
competency assessment process for IMGs, including
common screening criteria and tools for assessment,
which could be implemented locally or regionally.
The Collaboration is heavily engaged in preparing
a national clinical examination for IMG candidates
seeking postgraduate training positions. Parts of the
examination were offered in 2009 in Alberta, and
the full examination is planned to be administered in
2010 in Manitoba, Quebec and British Columbia. It is
expected that the examination will be fully operational
across the country in 2011.
Finally, a significant undertaking was announced in
2009 – the launch of the Assessment Review Task
Force. This group is responsible for charting a course
for MCC examinations into the future. We look
forward to the Task Force’s recommendations, which
are planned to be unveiled in time for the 2011 Annual
Meeting.
The MCC has a bright, bustling future to which it can
look forward. The entire MCC community – past and
present - is to thank for that, as well as for nearly a
century of excellence in physician evaluation.

ABOUT THE MCC

T
he Medical Council of Canada (MCC) provides examinations that
assess whether medical graduates have the knowledge and skills
required for the practice of medicine. Every year, it conducts more than 11,500
evaluations of medical students and graduates through its examinations, offered
in both official languages. By successfully completing the MCC examinations and
meeting all other eligibility requirements, a candidate receives the Licentiate of the
Medical Council of Canada and is included in the Canadian Medical Register.
Candidates must apply to the medical regulatory authority in the province or territory
for which they seek a medical licence. The Licentiate of the Medical Council of
Canada is one of the requirements the medical regulatory authority asks of candidates
applying for an independent practice licence. As a standard requirement across the
country, the Licentiate provides its holders with increased portability to practise
medicine across provincial and territorial borders. This national qualification also
ensures a common standard for physician competency across the country.
The MCC also plays an integral role in facilitating the transition of international
medical graduates into the Canadian health-care system. International medical
graduates can begin the process of obtaining a medical licence before their arrival in
Canada. While still abroad, they can submit copies of their credentials to the MCC’s
Physician Credentials Repository for them to be authenticated. Internaltional medical
graduates can also take the Medical Council of Canada Evaluating Examination in
one of the 73 countries where it is offered.

Examinations

T
he MCC offers three different examinations that evaluate a medical
graduates’ clinical skills and competency. Graduates who have not
obtained their medical education from a recognized accredited program in
North America must first pass the Medical Council of Canada Evaluating
Examination (MCCEE) prior to being eligible to take the two Qualifying
Examinations. Passing the MCCEE demonstrates that the international
medical graduate has equivalent medical knowledge to a Canadian graduate
entering residency training. The MCCEE consists of 175 multiple-choice
questions that focus on child, maternal, adult and mental health, as well as
population health and ethics. In 2009, 2,861 candidates took the computerbased MCCEE in more than 35 countries.
The Medical Council of Canada Qualifying Examination Part I (MCCQE
Part I) is divided into two parts. The first half of the examination uses
computer technology to deliver multiple-choice questions that adapt during
the examination to suit the candidate’s skill level. The second half evaluates
clinical decision-making by presenting the candidate with roughly 60
medical cases. In 2009, 5,079 candidates took the MCCQE Part I.
The Medical Council of Canada Qualifying Examination Part II (MCCQE
Part II) is an objective structured clinical examination (OSCE), which means
candidates are tested at stations by interacting with standardized patients,
individuals who are trained to simulate patients with illnesses or problems.
The MCCQE Part II is offered in 16 different sites across Canada. In 2009,
3,679 physicians attempted the MCCQE Part II examination.

he MCC’s Physician Credentials Repository authenticates (through
source verification) credentials and offers lifetime storage of
medical documents for international medical graduates who need to have
foreign credentials recognized in Canada. In 2009, the MCC verified 11,680
documents.
The physician can choose to share his or her credentials electronically
with stakeholders in the Canadian medical community including medical
regulatory authorities, and in the near future, with health authorities and
hospitals. This will greatly facilitate a physician’s application process for
medical licences and for positions within the medical profession. While the
service is currently only available to international medical graduates, it will
eventually be offered to all Canadian physicians.
As of December 31, 2009, the repository had 9,208 international medical
graduate users with credentials issued from organizations spanning 148
countries.

Research and Development

I
n order to remain at the forefront of medical assessment, the
MCC created a separate Research and Development department
in 2004 with an expanded mandate that includes designing new assessment
and evaluation tools. Researchers at the MCC conduct studies in a variety
of areas related to the examinations including validity, response time and
scoring methodologies.
The department provides examination feedback to the Canadian faculties of
medicine in the form of Deans’ reports, which allow faculties to determine
how their students fared in comparison to those at other faculties.
The Research and Development department provides grants to external
researchers in the field of medical education and assessment to fund research
in areas that will help improve medical education in Canada. Grants are also
provided to medical schools to fund postdoctoral fellow positions in medical
education.
All grant applications are peer reviewed by a committee of experts, which
allocates the funds. In 2009, the MCC awarded $377,634 in new grants to
researchers.

Council reaffirmed the fees for 2010 that were proposed at the 2008 Annual Meeting. As
such, the 2010 fee for the MCCQE Part I is C$720; the fee for the MCCQE Part II is
C$1,850; the fee for the MCCEE is C$1,500 for a first-time applicant and C$1,300 for a
repeat applicant.
It was moved by Dr. Dennis Kendel and seconded by Dr. Theresa Farrell that the 2011
MCCQE Part I fee of $780 be approved.
It was moved by Dr. Dennis Kendel and seconded by Dr. Morris VanAndel that the 2011
MCCQE Part II fee of $1,950 be approved.

Section 5.04
A notice in writing stating the day, hour and place of each Annual Meeting or Special
General Meeting and the general nature of the business to be transacted shall be served
either personally or by transmitting such notice to each member of the Council at least
fourteen days (exclusive of the day of service but inclusive of the day for which notice is
given) before the date of every meeting directed to such address as appears on the books of
the Council or, if no address be given therein then to the last address of the member known
to the Executive Director, provided always that a meeting of the members of Council may
be held for any purpose at any time and at any place, including by teleconference or other
electronic means, without notice if all members of the Council are present in person or if
absent members have signified their consent in writing to the meeting being held. Notice
of any meeting or any irregularity of any meeting or in the notice thereof may be waived in
writing by any member. The accidental omission to give notice of any meeting or the nonreceipt of the notice by any member or members shall not invalidate any resolution passed
or any action or proceedings taken at the meeting.
Section 6.02
The President shall act as Chair of the Executive Board and the Executive Director, or
designate, shall act as Secretary to the Executive Board.
Section 6.06
Each member of the Executive Board shall be entitled to exercise one vote on each issue
which comes before each meeting of the Executive Board. Unless otherwise specifically
provided by the Canada Corporations Act or the by-laws of Council, at all meetings of
the Executive Board, every question shall be determined by a majority of votes cast at the
meeting. A declaration by the Chair of the meeting that a resolution has been carried or not
and an entry to that effect in the minutes shall be prima facie proof of the fact without proof
of the number or proportion of the votes recorded in favour of or against such resolution.

Section 7.01
Section 9.02.02 (a)
Fee changes
The officers of the Council shall be the Immediate Past-President, the President, VicePresident, the Chair of the Finance Committee, and the Executive Director. The members
of Council may elect such additional officers as Council may from time to time determine,
and may assign to them such duties as it sees fit. The term of elected officers shall be for
one year or until their successors are elected, renewable by election for one additional term,
except for the Chair of the Finance Committee, who may be elected for more than one
additional term.
Finance Committee
The Finance Committee shall consist of the Chair of the Finance Committee, the President,
and three other members who shall be elected by the members of Council at an Annual
Meeting from the general membership, one of whom shall be a member at large.
Section 10.03
The Examination Board shall consist of test committees for each domain or area in which
candidates for the Part I and Part II examinations are to be assessed, the Central Examination
Committee, and the Planning Committee.
Section 10.06.01
Prepare or select from materials available to it, test items including clinical problems that
are pertinent to:
(h) self-directed learning and maintenance of competence.
Section 10.12
The Central Examination Committee shall be responsible for:
10.12.02
determining whether denial of Standing shall be given to a candidate who has committed or
is suspected of having committed infractions of the Rules for the Conduct of Examinations
and whether such candidates shall be denied entry to future examinations, including such
terms and conditions precedent as may apply to any entry to future examinations;
10.12.03
determining whether an award of No Standing shall be given to a candidate where
procedural irregularities in the examination process may have affected the performance of
the candidate and may have prevented a reliable assessment of the candidate’s demonstrated
knowledge and abilities;
Section 10.13
The Central Examination Committee shall:
10.13.01
for those candidates to be given Standing, award to each candidate at the latest administration
of the MCCQE Part I a Pass or Fail Standing on the results of the MCCQE Part I as a whole.
10.13.02
for those candidates to be given Standing, award to each candidate at the latest administration
of the MCCQE Part II a Pass or Fail Standing on the results of the MCCQE Part II as a
whole.

10.13.03
in the event a candidate is prevented from finishing the latest administration of the MCCQE
Part I or the MCCQE Part II due to circumstances beyond the candidate’s control, decide
whether to award a Pass, Fail or No Standing on the basis of the examination or part thereof
completed.
Section 10.14
The decisions of the Central Examination Committee in regard to the Pass or Fail Standing
awarded to each candidate on the examinations shall be final, subject only to reconsideration
by the Central Examination Committee.
Section 10.15
A candidate may appeal a decision by the Central Examination Committee to the Appeals
Committee of the Council on the following grounds:
(a) where the candidate has been denied Standing;
(b) where the candidate has been denied entry to future examinations, with or
without terms and conditions precedent for the entry to future examinations;
(c) where the candidate has received an award of No Standing;
(d) where the candidate considers that the Standing awarded on the results of
an examination have been prejudiced by the conduct, but not the content, of
the examination.
Section 11.06
To be eligible for the MCCQE Part I a candidate must:
11.06.01
be a graduate, or be fully expected to complete all requirements to graduate before such
date as prescribed for each administration of the MCCQE Part I, from a medical school
accredited by the Committee on Accreditation of Canadian Medical Schools/Liaison
Committee on Medical Education (“CACMS/LCME”); or
11.06.03
be fully expected to complete all requirements to graduate from a medical school not
accredited by CACMS/LCME but listed in one or more directories of medical schools
approved from time to time by resolution of the Executive Board and be a medical school
acceptable to the Executive Director before such date as prescribed for each administration
of the MCCQE Part I, and hold a valid Pass Standing on the MCCEE or an examination
determined by the Executive Board to be an acceptable substitute; or
11.06.05
be fully expected to complete all requirements to graduate from a United States School
of Osteopathic Medicine accredited by the American Osteopathic Association before such
date as prescribed for each administration of the MCCQE Part I and hold a valid Pass
Standing on the MCCEE or other examination as determined by the Executive Board to be
an acceptable substitute.

Section 11.08
When applying for admission to take the MCCQE Part I or Part II or any re-examination, a
candidate shall submit to the office of the MCC a completed application as well as the fees,
requisite information and credential/training documents as may be prescribed from time to
time by MCC.
Section 11.09
Each accepted candidate shall receive from the Executive Director documents pertaining
to the MCCQE Part I or Part II, or any re-examination to which the candidate is accepted
including the appropriate documentation which will be required as a condition for admission
to the assigned examination date and center.
A new section was also added to the bylaws:
Section 12.02
The members of Council may prescribe regulations for the development and effective
conduct of examinations for the purpose of assessing the competency of International
Medical Graduates, all matters incident thereto.
Section 13.01
Candidates who have been denied entry to future licentiate or other examinations may,
on the basis of new information, petition the Executive Board to be granted permission to
again be allowed to have access to future examinations.
Article 15
All references to “Physician Credentials Registry of Canada” (PCRC) were changed to
“MCC Physician Credentials Repository” (PCR).
Section 16.05
After considering the notice of appeal, the evidence adduced at the hearing and the
submissions made, the appeal panel may decide to:
Section 16.05.02 allow the appeal and order:
(b) that the denial of Standing or the award of No Standing be set aside and the candidate
be granted a Pass or Fail Standing by the Central Examination Committee or by the
National Assessment Central Coordinating Committee (NAC3) based on the results of
the examination in issue;
(c) that the denial of entry to future examinations, including the terms and conditions
precedent to any re-entry to future examinations as determined by the Central
Examination Committee or NAC3 be set aside, reduced or altered.
The Council approved the inclusion of a new article:
Article 26
Any Notice (which term includes any communication or document, electronic or
otherwise) to be given, sent, delivered or served pursuant to the By-laws or otherwise,
in writing or otherwise, to a member of Council, officer, member of a Committee or
candidate, shall be sufficiently given if delivered personally to the person to whom
it is to be given, or if delivered, mailed, sent by means of recorded communication
or, with the written consent of the recipient, by electronic means including e-mail, to
the person’s recorded address. A Notice so delivered shall be deemed to have been
given when it is delivered personally or to the person’s last known address. A Notice so
mailed shall be deemed to have been given when sent.

D
r. John McCrea, a member of Council since 2002, said
working with the Medical Council of Canada (MCC) has
allowed him to pursue his interest in medical policy and
governance. Since medicine is a self-regulating profession, he said it is
important that all physicians can participate in the governance process.
“Regular members of the profession have access to all levels of governance
of the profession. I’ve tried to learn how governance works really in the
hopes of making it work better.”
Dr. McCrea is a family physician and emergency room doctor, representing
the College of Physicians and Surgeons of New Brunswick (CPSNB) on
Council. Before joining the Council, Dr. McCrea started off working with
small committees at the CPSNB. He said he was primarily interested in
manpower management and physician recruitment, and then became
involved with policy and governance.
The reason that self-regulation works for medicine, said Dr. McCrea, is that
the operations of national organizations in charge, such as the Council, aren’t
remote. “When you look at the reach of Council to all the constituencies
and population, it really is very effective,” Dr. McCrea said.
Dr. McCrea said the MCC has recognized that excellence in health care
is about the people in the profession and developing the human capital to
support it.
“Medicine is a profession that attracts some talented people. The leadership
at the MCC has built an administration for the medical profession that has
drawn some really bright, positive people,” Dr. McCrea said. “It’s all about
trying to bring the best out of the candidates.”

Dr. Frédérick D’Aragon
Resident
Member of Council
A
s a resident member on Council, Dr. Frédérick D’Aragon
is the most recent Council member to have received his
Licentiate of the Medical Council of Canada (LMCC),
which he was awarded in 2008. Having just been through the examination
process, Dr. D’Aragon said he understands the importance of the Licentiate
and the Qualifying Examinations.
“The LMCC represents proof of my competence to practise medicine. The
qualification is recognized across the whole country, and all around the
world as a high standard of medical practice. With this qualification, I will
be allowed to move to another country, like Australia, for my fellowship.”
Dr. D’Aragon completed his MD at Université de Sherbrooke where
passing the Medical Council of Canada Qualifying Examination Part I is
a requirement for graduation. He said being on Council has taught him a
lot about medical education. “Collaboration is the most important tool to
develop and innovate. The Medical Council of Canada (MCC) is more than
just an examination body!”
He said he is honoured to represent medical residents and be a part of the
MCC. “I have had the opportunity to meet and work with the principal
medical leaders in Canada. My involvement with the Council allows me to
participate in maintaining and improving high standards of medical practice
in Canada.”
As a personal goal with Council, Dr. D’Aragon said he would like to
continue to be involved with the Finance Committee. “I would like to
review the cost effectiveness of the exam process without compromising
the quality of assessment.”

I
n the 10 years that Dr. Ramses Wassef has been
involved with the Medical Council of Canada test
committees, he said he has seen significant improvement in
the assessment process.
“In the last few years, we realized that we have been
examining candidates more on their knowledge, rather than
skills. But we realized that the public expects doctors to
behave in a certain way. Professionalism is just as important as medical knowledge,” said
Dr. Wassef, now Chair of the Central Examination Committee (CEC).
Despite being told that it wouldn’t be possible to assess knowledge of professionalism with
a written, multiple-choice examination, the CEC wanted to take up the challenge, Dr. Wassef
said. “We charged the test committees to find ways to assess professional attributes.”
The first bank of test items was developed and proved successful, he said. “The questions did
differentiate very well between the candidates who could tell which response was associated
with professionalism and those who could not.”
The CEC oversees all of the examination process, figuring out how to put together the exam,
administer it and interpret it. Then the CEC is left with a range of results, and ultimately
determines who passes. “The CEC makes very important decisions,” Dr. Wassef said.
“These are high-stakes exams for the candidates, but also for the public who will have these
doctors as their physicians.”
As CEC Chair, Dr. Wassef said he looks forward to giving the CEC report at the Annual
Meeting each year and being able to highlight how smooth the examination administrations
have become. “They run so smoothly because the staff are so well trained and managers
know every detail of the process.”

S
ince joining the MCC in 1993 as a chief examiner at the
University of British Columbia for the Medical Council of
Canada Qualifying Examination Part II (MCCQE Part II),
Dr. Joan Fraser has become an expert on the objective structured clinical
examination (OSCE). Dr. Fraser, now Chair of the OSCE test committee,
said working with the MCC has given her exciting opportunities.
“I was invited to Indonesia in October 2008 as a clinical skills and OSCE
expert. The MCC has given me experience I would not have otherwise
been able to gain,” Dr. Fraser said. “Faculties in Indonesia were happy to
hear about the OSCE program and I was proud to be able to represent the
MCC.”
As a pediatrician, Dr. Fraser said working with the MCC has allowed her
to explore the field of medical education.
“I’ve certainly learned a lot more with respect to psychometric testing.
The MCC has done a lot to further my interest in medical education,” Dr.
Fraser said.
Dr. Fraser said she wants to continue working with the MCC to strengthen
the MCCQE Part II.
“My goal, while working with the committee, is to put as much effort as
possible into improving the exam and making it the best product in the
world. Although, locally and internationally, it is already one of the best
in the world.”
“As a British Columbia resident, I have to travel a long way to come to
Ottawa but I really enjoy working with the MCC team,” she said. “It’s a
great group of people.”

rish McKeagan believes that assisting examination
candidates is one of the best parts of her job.
She has worked at the Medical Council of Canada (MCC) in credentials and
registration for more than five years. For Ms. McKeagan, “work at MCC
is meaningful and serves a purpose. Helping candidates through source
verification processes provides a tremendous sense of accomplishment.
Even on a stressful day, you have that one candidate that you’ve helped,
and it brings it all back into perspective.”
MCC service agents are responsible for ensuring that candidates have
access to the information and resources they need while preparing for their
examinations, which can be a stressful time for them, Ms. McKeagan said.
“When a candidate calls, they might be stressed or in need of reassurance.
They want guidance. We’re here to help them through the process and
determine what we can do to facilitate their application process.”
Ms. McKeagan said her department works really well together as a team,
which adds to the positive atmosphere at work.
“When I think of our department, I think of a quote I once heard: ‘A group
becomes a team when each member is sure enough of himself and his
contribution to praise the skills of others.’ This certainly rings true of our
department – every player puts the team ahead of personal glory.”

Award
Outstanding Achievement Award
in the Evaluation of Clinical Competence
Dr. André Jacques
T
he Outstanding Achievement Award is an
internationally peer reviewed award bestowed on
individuals who have made a significant and vital
achievement in the field of assessment and evaluation of clinical
and professional competence in the health professions.
The 2009 Outstanding Achievement Award was presented to Dr.
André Jacques.
His career began as a family doctor in 1975. From the start, Dr.
Jacques was involved in continuing medical education (CME) at a
regional, provincial, national and international level. He was also
an Associate Professor in Family Medicine at the University of
Montreal.
In 1993, he accepted a position at the Collège des médecins du
Québec and has been Director of the CME Division and, since 1997,
Director of the Practice Enhancement Division. In 1986, he joined
the Council on CME of Québec where he is currently appointed as
Secretary.
Dr. Jacques is an elected member of the executive committee of
the Federation of Medical Regulatory Authorities of Canada since
1998. He has also served as president in 2002.

Distinguished Service Award

T
he Dr. Louis Levasseur Award is named for the former Chair of the Medical Council’s
Central Examination Committee, Dr. Louis Levasseur, who for 26 years personified
an unwavering commitment to the objectives of the Council. The Dr. Louis Levasseur
Award is presented to a Council, staff or committee member, past or present, or to a person whose
collaboration has contributed in an extraordinary manner to the mission and vision of the Medical
Council of Canada. The 2009 recipient of the Dr. Louis Levasseur Award is Dr. Tom Maguire.
Tom Maguire attended the University of Alberta for undergraduate degrees in Physics and Education
and a Master’s degree in Educational Psychology. After completing his doctorate in psychometrics
and program evaluation at the University of Illinois, he became a member of the Departments of
Educational Psychology at the University of Alberta and at the University of Victoria where he taught
courses in applied statistics and psychometrics. In 1994, he was appointed to the J. Alan Gilbert Chair
in Medical Education at the University of Alberta and in 1997, he was named Professor Emeritus.
In 1987, the Central Examination Committee decided to change how the pass/fail standing was
established for the Medical Council of Canada Qualifying Examination (MCCQE) Part I. Dr. Maguire
was asked to explore implementing a criterion-referenced approach. His recommended process, which
over time has been enhanced, is still used for the multiple-choice portion of the examination.
In 1997, he was invited as a psychometric consultant to assist in developing a computerized MCCQE
Part I. He advocated for the inclusion of computer-adaptive testing to provide each candidate with a
unique assessment while retaining content validity. Computer-adaptive testing was later incorporated
into the computerized MCCQE Part I.
For the past 10 years, he has acted as an informal sounding board to individuals in the Evaluation
Bureau and the Research and Development departments. He was also a member of the Examination
Development Advisory Committee.

National Assessment
Collaboration
With the approval of the National Assessment
Collaboration (NAC) governance model in
October 2009, the Canadian health-care system
is one step closer to having a streamlined
process
for
assessing
foreign-trained
physicians, or international medical graduates.
The NAC, which includes partners from across
the country, agreed to focus its efforts on a
clinical examination to assess international
medical graduates seeking entry into residency
programs. The NAC’s administrative body
reports to the Medical Council of Canada’s
(MCC) Council.
The clinical examination, which is centrally coordinated, will be offered regionally through
the provincial international medical graduate
programs. In 2009, the Alberta International
Medical Graduate Program incorporated
elements of the NAC clinical examination in its
own assessment. British Columbia, Quebec and
Manitoba plan to offer the examination in 2010.

Task Force
The MCC would have been very different today
had it not followed the recommendations from
the 1987 Future Directions Task Force. As a
result of those recommendations, the MCC built
its own in-house examination centre, published
detailed objectives on what MCC evaluations
would cover, and launched the first ever national
clinical examination – the Medical Council of
Canada Qualifying Examination Part II.
Now, in 2009, the Council announced that it
was embarking on a similar review exercise
with the Assessment Review Task Force. Its
mandate includes exploring new assessment
techniques and looking at MCC examinations
within the context of the current, continually
evolving medical environment. The Task Force
is expected to publish its recommendations in
October 2011.

Two MCC
departments merge
After much planning and preparation, the
MCC’s Physician Credentials Repository
(PCRC) and the Credentials and Registration
department merged in late 2009. These two
client-facing departments amalgamated to
improve their ability to help candidates with
all of their inquiries – whether related to
examination applications, eligibility or the
status of their credentials. The new department
is called the Repository and Registration centre.
Work will continue in 2010 and beyond to merge
the credentials repository and examination
application systems to provide even more
seamless service to candidates.
New practice
examination
In response to popular acclaim for the online
practice examination that helps candidates
prepare for the Medical Council of Canada
Evaluating Examination, in November 2009
the MCC launched the Self Administered
Qualifying Examination Part I (SAE – QEI) to
assist candidates in preparing for the Medical
Council of Canada Qualifying Examination
Part I (MCCQE Part I).
The SAE – QEI is offered over the Internet and
consists of 96 multiple-choice questions. Upon
completion of the SAE – QEI, participants
instantly receive email feedback consisting of
the number of questions answered correctly
and a comparison of the participant’s results to
those of other MCCQE Part I participants. The
MCC is exploring options to enhance the SAE –
QEI by including short menu questions similar
to those featured in the clinical decision-making
component of the MCCQE Part I.

efore being selected by McGill University
as the Medical Council of Canada’s (MCC)
external postdoctoral fellow, Kevin Chin’s research focus
was primarily human rights education. Dr. Chin said his
knowledge of medical education research was quite limited
before he started his fellowship, but his interest in the field
has grown.
“I greatly appreciate the opportunities provided to me by the MCC to be involved in the growing
and vibrant field of medical education research. Through its postdoctoral fellowships, the MCC is
acknowledging the critical role that research plays in advancing the field of medical education,” Dr.
Chin said.
Since beginning his two-year fellowship in January 2009, Dr. Chin has been appointed the principal
investigator on a project funded by the Royal College of Physicians and Surgeons of Canada that
explores narrative skills in teaching and clinical settings. He has also been involved with research
related to understanding faculty leadership and mentorship roles, improving learning environments
for residents in obstetrics-gynecology, and fostering self-regulated learning processes with medical
students.
Dr. Chin said working with the MCC has also given him the opportunity to attend academic
conferences and “connect to the field through attending presentations and networking.” In May
2009, he participated in his first Canadian Conference on Medical Education. Dr. Chin said he is
looking forward to being able to attend more conferences in 2010.
“Events such as these have provided me with fora to share research ideas, to learn about the work
of other researchers and to meet with people who are dedicated and enthusiastic about medical
education.”

We have audited the statement of financial position of the Medical Council
of Canada as at December 31, 2009 and the statements of operations,
changes in net assets and cash flows for the year then ended. These
financial statements are the responsibility of the Council’s management.
Our responsibility is to express an opinion on these financial statements
based on our audit.
We conducted our audit in accordance with Canadian generally accepted
auditing standards. Those standards require that we plan and perform an
audit to obtain reasonable assurance whether the financial statements are
free of material misstatement. An audit includes examining, on a test
basis, evidence supporting the amounts and disclosures in the financial
statements. An audit also includes assessing the accounting principles
used and significant estimates made by management, as well as evaluating
the overall financial statement presentation.
In our opinion, these financial statements present fairly, in all material
respects, the financial position of the Council as at December 31, 2009
and the results of its operations and its cash flows for the year then ended
in accordance with Canadian generally accepted accounting principles. As

1. Purpose of the Organization:
The Medical Council of Canada was established in 1912 under the Canada Medical
Act. The Council was subsequently incorporated without share capital under the
Canada Corporations Act as the Medical Council of Canada (the “Council”). The
Council is a registered charity under subsection 149(1) of the Income Tax Act.
The Council’s original purpose was to establish and promote a qualification in
medicine, known as the Licentiate of the Medical Council of Canada, such that
holders thereof are acceptable to provincial licensing authorities for the issuance
of a licence to practise medicine. At the time of incorporation, its role expanded
to include the initiation and promotion of research and development in the
evaluation of medical knowledge and competence of undergraduate and graduate
medical students and practitioners of medicine and of other health sciences.
Its Vision: Striving for the highest level of medical care for Canadians
through excellence in evaluation of physicians.
Its Mission: With the key stakeholders, the Council:
• Develops, validates and implements tools and strategies to evaluate
physicians’ competence; and
• Maintains a national registry of physicians and their qualifications
throughout their professional careers.
2. Significant accounting policies:
These financial statements have been prepared in accordance with Canadian
generally accepted accounting principles and include the following significant
accounting policies:
(a) Basis of presentation:
These financial statements reflect the operations of the Council. These
financial statements do not include the revenue, expenses, assets and liabilities
of the Council’s related entities: The Medical Identification Number for
Canada Inc. and Carleton Condominium Corporation No. 517. Related parties
are disclosed in note 8.

(b) Financial instruments:
(i) Investments:
Investments are designated as available for sale and are recorded at fair
value. Realized gains and losses and investment income calculated using
the effective interest rate method is recorded in the statement of operations.
Realized gains and losses on sale of investments are the difference between
the proceeds and the average cost of the investments. Unrealized gains
or losses from the change in fair value are recorded in the statement of
changes in net assets. Fair value is determined at quoted market prices.
Sales and purchases of investments are recorded on the settlement date.
Transaction costs related to the acquisition of investments are expensed.
(ii) Other financial instruments:
The Council has classified accounts receivable as loans and receivables and
accounts payable and accrued liabilities as other liabilities.
Upon initial recognition, these financial assets and liabilities are measured
at fair value. Subsequent to initial recognition, these financial assets and
liabilities are measured at amortized cost using the effective interest
method of amortization.
(c) Capital assets:
Capital assets are initially recorded at cost and are amortized on a straight-line
basis over their estimated useful lives as follows:

(d) Employee future benefits
The Council participates in a defined benefit pension plan (the Canadian
Medical Association Pension Plan) and also provides supplemental pension
arrangements to certain employees. Pension benefits are based on length of
service and final average earnings.
The cost of pension benefits earned by employees is actuarially determined
using the projected benefit method pro-rated on service and management’s
best estimate of expected return on plan assets, salary escalation and retirement
ages of employees.
For the purpose of calculating the expected return on plan assets, those assets
are recorded at fair market value.
The excess of the net actuarial gain (loss) over 10% of the greater of the
benefit obligation and the fair value of the plan assets is amortized over the
average remaining service period of active employees of the plan (13 years).
The Council amortizes the transitional asset, which occurred when new
accounting policies were adopted, on a straight-line basis over the average
remaining service period of active employees expected to receive benefits
under the plan.
The cumulative excess of pension fund contributions over the amount recorded
as expense is recorded as a pension asset on the statement of financial position.
The cumulative excess of the amount recorded as expense over supplemental
benefits paid is recorded as a pension liability.
(e) Expenses:
In the statement of operations, the Council presents its expenses by function
with the exception of amortization of capital assets, which is presented
separately.
Expenses are recognized in the year incurred and are recorded in the function
to which they are directly related. The Council does not allocate expenses
between functions after initial recognition.
Expenses are disclosed by object for each function in the schedule of expenses.

(f) Net assets:
Net assets are internally restricted to promote research and development as
authorized by Council at the 1992 annual general meeting. On an annual
basis, an amount equivalent to the revenue from evaluating examination fees
forfeited is internally restricted.
Net assets are also internally restricted to develop new and/or alternative
methods of assessment as authorized by the Finance Committee in 1996, for
the centennial project to provide for the 100th year anniversary of the Council,
set to occur in 2012, for an insurance reserve to provide for self-insurance
in the event of a loss and, for the appeals reserve to provide for a legal court
appeal.
(g) Revenue recognition:
The Council follows the deferral method of accounting for revenue. Restricted
contributions are deferred and are recognized as revenue in the year in which
related expenses are incurred. Examination fees are recognized as revenue
when the examination takes place. All other revenue is recognized as revenue
when received or receivable if the amount to be received can be reasonably
estimated and collection is reasonably assured.
Evaluating examination fees which are inactive for a period of four years are
forfeited by candidates and are recorded as revenue.
(h) Cash and cash equivalents:
Cash and cash equivalents include deposits with financial institutions that can
be withdrawn without prior notice or penalty and short-term deposits with an
original maturity of ninety days or less.
(i) Use of estimates:
The preparation of financial statements in conformity with Canadian generally
accepted accounting principles requires management to make estimates and
assumptions that affect the reported amounts of assets and liabilities and
disclosure of contingent assets and liabilities at the date of the financial
statements and the reported amounts of revenue and expenses during the year.
Actual results could differ from those estimates. These estimates are reviewed
periodically, and, as adjustments become necessary, they are reported in
earnings in the year in which they become known.

(j) Budget figures:
Budget figures have been provided for information purposes and are derived
from the Council’s budget approved by the Members of Council. Budget
figures have not been audited.
3. Adoption of new accounting standards:
Amendment to Section 1000, Financial Statement Concepts:
Effective January 1, 2009, the Council adopted the revised Canadian Institute of
Chartered Accountants (“CICA”) Handbook Section 1000, Financial Statement
Concepts which amended the definition of an asset. Upon adoption of these
standards, the Council determined that its existing assets continued to meet the
revised definition of an asset. However, the Council has reclassified amounts
previously recorded as prepaid expenses and deferred charges to examination
content development costs included in capital assets. These reclassifications had
the following effect on the comparative figures:

3. Adoption of new accounting standards (continued):
Amendments to Accounting Standards that Apply Only to Not-forProfit Organizations:
Effective January 1, 2009, the Council adopted the amendments to the CICA
Handbook 4400 series of standards, which only apply to not-for-profit
organizations. These amendments revised the financial statement presentation
and disclosure requirements for not-for-profit organizations. The adoption of
these amendments had no significant effect on the Council’s financial statements.
Disclosure of Allocated Expenses by Not-for-Profit Organizations:
Effective January 1, 2009, the Council adopted the CICA Handbook Section
4470, Disclosure of Allocated Expenses by Not-for-Profit Organizations which
establishes disclosure requirements for not-for-profit organizations that report
their expenses by function and allocate expenses between functions. Adoption
of these recommendations had no effect on the financial statements for the year
ended, December 31, 2009, except for the additional note disclosure in note 2(e).

4. Investments (continued):
Investments are managed by investment managers in accordance with an
investment policy approved by the Members of Council. The Council’s
investment policy limits investments to fixed income securities with a rating of
BBB or better, short-term investments with a rating of R-1 or better and equity
investments that are reasonably liquid. Except for federal and provincial bonds,
not more than 10% of the manager’s portfolio can be invested in bonds or shares
of a single issuer. Furthermore, no more than 15% of the bond portfolio can be
invested in bonds rated BBB. The Council’s bonds have effective interest rates of
4.0% to 6.70% and maturity dates ranging from 2011 to 2022.

The Council regularly reviews investments to determine whether unrealized
losses represent temporary changes in fair value or are a result of other than
temporary impairments. The consideration of whether an investment is other
than temporarily impaired is based on a number of factors which include, but are
not limited to, the financial condition of the issuer and the length and magnitude
of the unrealized loss and specific credit events. The Council also considers its
intent and ability to hold an investment for a sufficient period of time for the
value of the unrealized loss to recover. Based on the evaluation as of December
31, 2009, unrealized losses are considered to be temporary.

and $4,092,945, respectively.
In the year, the Council disposed of capital assets for $Nil consideration resulting
in a $15,300 loss.
6. Capital disclosures:
The Council considers its capital to consist of its net assets. The Council’s overall
objective with its capital is to fund capital assets, future projects and ongoing
operations. The Council manages its capital by establishing restricted funds
and appropriating amounts to the restricted funds for anticipated future projects,
contingencies and other capital requirements. These allocations are disclosed in
the statement of changes in net assets.
7. Employee future benefits:
The Council is a participating employer in the Canadian Medical Association
(CMA) Pension Plan. The CMA Pension Plan is funded by contributions from
participating employers and from plan members.

The Supplemental Executive Retirement Plans (SERPs) are unfunded
arrangements, sponsored by the Council.
The CMA Pension Plan and SERPs are defined benefit pension plans.
The Council has adopted an annual measurement date for December 31 for
estimating the accounting surplus or deficit and establishing benefit costs for the
fiscal year.
The latest triennial actuarial valuation for accounting purposes was performed as
at January 1, 2008.

The Council is related to The Medical Identification Number for Canada
Corporation and Carleton Condominium Corporation No. 517.
(a) The Medical Identification Number for Canada Corporation
(“MINC”):
The Council is related to MINC by virtue of its ability to appoint 50% of their
Board of Directors. The Executive Director of the Council sits on the Board
of Directors of MINC as a Director. MINC is incorporated under the Canada
Corporations Act and is a non-profit organization under subsection 149(1)(l)
of the Income Tax Act. MINC was created to establish and maintain a unique
physician identifier for every individual in the Canadian medical education and
practice systems. The Council has a $90,000 (2008 - $Nil) liability to MINC at
year-end.

(b) Carleton Condominium Corporation No. 517:
The Council is related to Carleton Condominium Corporation No. 517 (the
“Corporation”) by virtue of its ability to appoint the majority of the Board of
Directors. The Corporation is a non-profit organization under subsection 149 (1)
(l) of the Income Tax Act, incorporated under the Condominium Act of Ontario.
The Corporation maintains the common elements of the building in which the
Council operates.
The Corporation has not been consolidated in the Council’s financial statements.

8. Related party transactions (continued):
The Corporation, as required by the Condominium Act, has established a reserve
fund for financing future major repairs and replacements of the common elements.
Contributions to the reserve fund must be at least 10% of the amount required for
contributions to the common expenses exclusive of the reserve fund. Only major
repairs and replacements are charged directly to this fund.
Included in the Council’s statement of operations are condominium fees of
$331,578 (2008 - $237,465) charged by and paid to the Corporation.
9. Financial instruments:
The Council’s financial instruments consist of cash, accounts receivable,
investments and accounts payable and accrued liabilities. The carrying values
of these instruments approximate their fair values due to their immediate or short
terms to maturity. Investments are carried at fair value, as disclosed in note 4.
Management believes that the Council is not exposed to significant interest rate,
currency or credit risks from its financial instruments.

Certain 2008 comparative figures have been reclassified to conform with the
financial statement presentation adopted for the current year.
