Requirements for Students on Academic Probation

SPRING 2015

NOW – Register for one of the following Academic Recovery Orientation workshops by January 5, 2015 at www.moreheadstate.edu/probation . All workshops will be held in Crager Room in ADUC. *For online and regional campus students, the workshop will be accessible via Blackboard on January 19 – February 2, 2015. The workshop must be viewed and the quiz completed by no later than midnight on February 2, 2015.

Make sure to attend your scheduled Academic Recovery Orientation workshop. Failure to attend will adversely affect your participation in the Academic Recovery Program and/or MSU 099 – Learning for Success.

During the spring 2015 semester, you MUST:

Be enrolled in *MSU 099 – Learning for Success IF THIS IS YOUR FIRST SEMESTER ON PROBATION. Students who are on continued probation or who have completed MSU 099 previously will not be placed in the course but must participate in the Academic

Recovery Program as required by the probation guidelines for scholastic standing. * Our office will automatically enroll you by the beginning of the spring 2015 semester. You will only receive credit for this course once during participation in the Academic Recovery Program.

The following requirements are the components of the MSU 099 course and/or the Academic Recovery Program:

1.

Complete the Action Plan and bring with you to the workshop .

2.

Complete the Midterm Success Workshop (Blackboard module) between March 2 — March 13.

3.

Meet at least two times with your academic advisor and professors throughout the semester as outlined below. Your advisor will ensure you are making progress and provide guidance. Your professors will provide grade and attendance updates on your

Progress Form during the dates outlined below. Take the Progress Form to your meetings with your advisor and professors to sign and make comments to document each visit. Turn in completed Progress Forms to the Office of Academic Advising and

Retention, 321 Allie Young.

Schedule first meeting between FEBRUARY 2 – FEBRUARY 20; PROGRESS FORM I – DUE February 20, 2015.

Schedule second meeting between MARCH 30 – APRIL 17; PROGRESS FORM II – DUE April 17, 2015.

Meet at least three times per semester with assigned Academic Peer Coach . First appointment will be scheduled during the

Academic Recovery Workshop. More information regarding this requirement will be shared in the Academic Recovery

Workshop.

Students are strongly encouraged to develop a structured study schedule. Below are the hours of operation for the various study/tutoring locations across campus.

Tutoring and Learning Center in Allie Young Hall 220; Hours: M-F 8:00am – 4:30pm

 Tutoring and Learning Center in the Library;

Hours : Mon.-Thurs. 7:30am - 11:00pm; Fri. 7:00am – 6:00pm; Sat. 11:00am - 7:00pm; Sun. 1:00pm – 11:00pm

Math Tutoring Lab in Lappin Hall 108; Hours: 8:00am – 8:00pm
