Terms and Conditions
1. A copy of your wireless service agreement (contract) and a copy of the
Costco receipt must be included with each rebate coupon mailed to Stuart
Lee Rebates, the rebate vendor. Please keep a copy of your rebate form for
your records.
2. All rebate forms must be post-marked no later than the 30th day from the
date of purchase as indicated on your Costco receipt.
3. Eligibility of this rebate is contingent on your new 2 year service agreement (contract). If your contract is cancelled within 12-14 weeks following
activation, your rebate will become ineligible and will not be honored.
4. For eligible rebates, your check will be mailed to the address provided
approximately 12-14 weeks after Stuart Lee receives your rebate request.
5. All rebates require a 2 year new agreement.
6. Activation rebates are valid for new customers only. Upgrades do not
qualify.
7. All rebate requests become the property of Wireless Advocates and will
not be returned. Use of multiple PO Boxes or addresses to obtain rebates will
be considered fraudulent and will not be honored.
8. Neither Wireless Advocates nor Stuart Lee will be responsible for any
rebate requests that are stolen or lost in transit, require postage upon
delivery, or fail to be delivered to the rebate address. If the rebate form
arrives late, damaged, illegible or incomplete, it will not be honored.
9. Offer valid for up to 5 phones per Mail-in-Rebate Form.
10. Limit one rebate offer per Wireless Number and ESN/IMEI.
11. Do not send rebate requests via certified mail, UPS or FedEx. Only United
States Postal Service deliveries will be accepted.
