Game reports are required to be filed in ArbiterSports by the center referee within 48 hours of game
time if you want to be paid for the game. Assistant Referees (AR) are always paid. Filing the report is
the center refereeʼs professional responsibility. Itʼs a contractual obligation with the leagues, so no
excuses will be accepted.
1. Fill in the correct game score. Double check for accuracy because you cannot edit a report once
you save it.
2. If one of the AR slots has not been assigned, then you will need to mark that slot as a No Show. If
an assigned official does not show up, mark that official as a No Show.
3. If you issued a yellow card and/or red card, check the box next to “USSF Misconduct Page 1”. If
you issued a red card, you will also need to check the box next to “USSF Misconduct Page 2
Supplementary Report”. The blue text is link to download the report forms in PDF format. Written
Misconduct reports must be filled out and emailed to assign@swisra.org within 24 hours. Referees
who do not file their misconduct paperwork will no longer be assigned. If you have questions, ask.
4. Where guest players are allowed by the Rules of Competition, you are required to list them in the
Comments section of the game report.
5. If you issue a yellow and/or red card, in addition to filing the written paperwork, you are required to
state the details for each card in the Comments section of the game report. Refer to the LOTG for
proper terminology. Sloppy writing is not professional.
6. If there is anything you want the league or assignor to know or you just need to vent, put it in the
Comments. League representatives read all the Comments.
7. Click “Save”.

Complete and submit this form to the appropriate competition authorities (e.g., local league, tournament
director, cup coordinator, etc.) as directed. If needed, and in addition to the U.S. Soccer Referee Report, the
U.S. Soccer Supplemental Referee Report can be used for instances of referee assault, referee abuse,
dismissal of team officials, sending-off offenses, serious injuries, game abandonment or other substantial
occurrences.

Complete and submit this form, along with a U.S. Soccer Referee Report, to the appropriate competition
authorities (e.g., local league, tournament director, cup coordinator, etc.) as directed for the dismissal of team
officials, sending-off offenses, serious injuries, game abandonment or other substantial occurrences. For
instances of referee assault or referee abuse, complete and submit this form, along with a U.S. Soccer Referee
Report, to the appropriate competition authorities (e.g., local league, tournament director, cup coordinator,
etc.), State President with jurisdiction for the competition and State Referee Administrator.

Statement:
In the 78th minute with the score tied 1-1 and BNSC
attacking, the Fusion goalkeeper came out 15 yds.
off her line. #22 for BNSC chipped the ball over
the goalkeeper's head. The ball was heading
directly into the goal when Fusion player #5, Erin Stop,
reached out and grabbed the ball with both hands
preventing the ball from entering the goal. I immediately
blew my whistle and indicated a penalty kick for
BNSC Premier. I separated Fusion player #5, Erin
Stop from the other players and shoed her the red card
for denying an obvious goal scoring opportunity by
handling the ball. Fusion player #5 left the field and
headed for the parking lot. BNSC Premier scored
from the penalty kick.
