Educator completes self-assessment of performance against the performance standards
(outlined in the rubric).
Educator writes draft of professional practice SMART goal with action steps and
benchmarks (based on self-assessment).
Educator reviews student performance data with colleagues and evaluator. Student learning
goal should address areas of need, and be aligned with district and school goals. Goal can
be individual or team (see below)
Educator meets with colleagues who have shared responsibility of students or common
instructional responsibilities to consider forming a PLT to develop a team student learning
goal.
Educator writes draft of (individual or team) student learning SMART goal with action
steps and benchmarks.
Educator meets with evaluator to review self-assessment and proposed goals.
Timeframe
September
Evaluator approves final Educator Plan.
By November 1
Educator collects artifacts of practice, especially related to standards III and IV. Artifacts
are shared with evaluator.
Evaluator observes educator’s practice:
-unannounced observations of any duration followed by feedback.
-for non-Professional Status educators in their first year, at least one announced
observation.
Any time during twoyear evaluation cycle.
Any time during twoyear evaluation cycle.
Educator submits artifacts of practice (if not already submitted) and reports on progress
toward attaining goals.
If requested by either the educator or the evaluator, they meet to review artifacts and
performance against Performance Standards (described in rubric) and progress on goals.
Evaluator completes formative Evaluation and gives to educator.

Summative Evaluation
Process, product
Educator submits artifacts of practice (if not already submitted) and evidence of attainment
of goals.
Evaluator analyzes evidences of performance and attainment of goals and assigns a
performance rating for each standard and an overall rating, which includes attainment of
goals.
Evaluator completes summative evaluation and gives to educator.
Timeframe
April – May of Year 2
Educator and evaluator meet to discuss summative evaluation if the educator is rated needs
improvement or unsatisfactory.
For educators receiving a rating of exemplary or proficient, they may meet, upon request
by either the educator or the evaluator, to discuss the summative evaluation.
Educator responds in writing, if desired, and signs summative evaluation by June 15.
