announced a voluntary recall of the following consumer product. Consumers should stop using recalled products
immediately unless otherwise instructed.
Name of Product: Cranium Cadoo Board Games
Units: About 38,000
Importer: Cranium Inc., of Seattle, Wash.
Hazard: The surface paint on the die contains excessive levels of lead, violating the federal lead paint standard.
Incidents/Injuries: None reported.
Description: Only the die found in Cranium Cadoo board games with lot numbers 2007195 through 2007244 are included
in the recall. The Cranium Cadoo game is packaged in a square cardboard box with an orange background. The seven
digit lot number is printed under the plastic tray on the bottom half of the box.
Sold at: Fred Meyer, Kmart, Shopko, Wal-Mart and specialty game stores nationwide between October 2007 through
January 2008 for about $20.

Imaginarium Activity Centers Sold at Toys “R” Us Recalled
Due to Choking Hazard
WASHINGTON, D.C. - The U.S. Consumer Product Safety Commission, in cooperation with the firm named below, today
announced a voluntary recall of the following consumer product. Consumers should stop using recalled products
immediately unless otherwise instructed.
Name of Product: Imaginarium Multi-Sided Activity Centers and Jungle Activity Centers
Units: About 16,400

Manufacturer: Taizhou Orient Toys Company Ltd. (Multi-Sided Activity Centers), and Shanghai Cosmos Gift Industry

Hazard: Small parts on the toys can detach, posing a choking hazard to young children.
Incidents/Injuries: Toys “R” Us has received 12 reports of small parts detaching from activity centers. No injuries have
been reported.
Description and Models: This recall involves the Imaginarium Multi-Sided and Jungle activity centers. The multi-sided
activity center has moveable block letters, clock face, sliding shapes and gears. The jungle activity centers have peg
jumping monkeys, sliding fish, abacus beads, bead mazes and other animal-themed activities. “Imaginarium” and the
name of the activity center are printed on the packaging. Item numbers 69042 (multi-sided) and 69083 (jungle) are printed
on the back of the box, near the bar code.
Sold exclusively at: Toys “R” Us stores nationwide and www.toysrus.com from August 2007 through February 2008 for
between $20 and $50.

Kids Station Toys Recalls Little Tikes Toy Cell Phones Due to
Choking Hazard

Name of product: Little Tikes Chit ‘N Chat Toy Cell Phones
Units: About 1 million
Manufacturer: Kids Station Toys International Ltd. of Miami, Fla.
Hazard: The hinge cover on the toy cell phone can detach from the phone, posing a choking hazard to young children.
Incidents/Injuries: CPSC and the firm have received three reports of the toy breaking, including one report of a child
beginning to choke on a small part.
Description: The Little Tikes brand Chit ‘N Chat Toy Cell Phone resembles a flip-phone style cell phone and has a 10key numeric button pad and three buttons with pictures of animals. All of the buttons make a sound when pressed. The
toy phones are multi-colored and have a sticker on the back with model number KSL4010 (sold separately) or KSL8032,
KSL8033, or KSL8051 (sold with other items). “Designed and Licensed by Kids Station Toys International Ltd.” is printed
on the back of the toy phones. Toy phones with a screw visible on the hinge cover are not subject to the recall.
Sold at: Department, juvenile product, and drug stores nationwide from June 2006 through March 2008 for about $8
(individually) or about $20 (set).

Remedy: Consumers should take these toy phones away from young children immediately and contact the firm for

Playskool Voluntarily Recalls Toy Tool Benches after the
Death of Two Toddlers
WASHINGTON, D.C. - In cooperation with the U.S. Consumer Product Safety Commission (CPSC), Playskool, of
Pawtucket, R.I., is voluntarily recalling about 255,000 Team Talkin' Tool Bench toys following the deaths of two young
children.
Playskool received reports that a 19-month-old boy from Martinsburg, W.V., and a 2-year-old boy from League City,
Texas, suffocated when oversized, plastic toy nails sold with the tool bench toys became forcefully lodged in their throats.
Though the toy nails are not considered a small-part, and the toys are intended for children age 3 and older, Playskool is
voluntarily conducting a recall as a precaution to prevent additional incidents.
The Team Talkin' Tool Bench™ is a 20-inch tall plastic toy tool bench with an animated red toy saw, a yellow toy drill and
a blue toy vice. The toy talks and makes various sound effects, including tool sounds. The product also includes a toy
hammer, screwdriver, two 2¼-inch plastic screws, two 3-inch plastic nails and pieces to build a small toy plane. The red
Playskool logo is on the front of the brown surface of the tool bench.
The toy was sold at Toys R Us, Wal-Mart, Target, KB Toys stores and various other stores nationwide from October 2005
through September 2006 for about $35.
Consumers should immediately take the two toy nails away from children and contact Playskool to get information on
returning the nails for a $50 certificate for a Playskool (or its related companies') product.

Lead Safety Standard
WASHINGTON, D.C. - The U.S. Consumer Product Safety Commission, in cooperation with the firm named below, today

Name of Product: “Sarge” die cast toy cars
Units: About 253,000
Importer: Mattel Inc., of El Segundo, Calif.
Hazard: Surface paints on the toys could contain lead levels in excess of federal standards. Lead is toxic if ingested by
young children and can cause adverse health effects.
Incidents/Injuries: None reported.
Description: The recall involves die cast “Sarge” 2 ½ inch toy cars. The toy looks like a military jeep and measures about
2 ½ inches long by 1 inch high by 1 inch wide. The recalled toy has the markings “7EA” and “China” on the bottom. The
“Sarge” toy car is sold alone or in a package of two, and may have the product number M1253 (for single cars) and K5925
(for cars sold as a set) printed on the packaging. The cars marked “Thailand” are not included in this recall.
Sold at: Retail stores nationwide from May 2007 through August 2007 for between $7 and $20 (depending on whether
they were sold individually or in sets).

CPSC, Fisher-Price Announce Recall of Infant and Toddler
Toys
WASHINGTON, D.C. - In cooperation with the U.S. Consumer Product Safety Commission (CPSC), Fisher-Price, of East
Aurora, N.Y., is recalling about 20,000 infant and toddler Intelli-Table toys for repair. Two red knobs on the toy can break
off, creating small parts that can pose a choking hazard for young children.
Fisher-Price has received 32 reports of the knobs breaking off the toys. A 14-month old child was found with a knob in her
mouth. Her mother removed it and no injuries have been reported.
The Intelli-Table is a round, plastic activity table with a blue, removable top that uses three interchangeable play rings to
help children learn numbers, music, and games. The toy is intended for children ages nine to 36 months. The base of the
Intelli-Table is red with three legs that are blue, yellow and teal green. The play rings are colored white, yellow and teal
green. The red knobs on the yellow ring and white ring can break off. The toy has a Fisher-Price logo on the blue
removable top; and has the model number, 77148, and the words, "Mattel, Inc." and "China," molded into the bottom of
the red base. Only models manufactured from September 25, 2000 through October 7, 2000 are being recalled. The
recalled models have a date code from 269(0) through 281(0) molded into the underside of the blue, removable top.
Toy, mass merchandise and discount stores sold these toys nationwide from October 2000 through March 2001 for about
$70.
Consumers should take the toys away from young children immediately and contact Fisher-Price toll-free at (800) 2207137 anytime to order a free repair kit with replacement knobs. The repair kit consists of two replacement knobs with new
screws. Consumers can also visit the Fisher- Price web site at http://www.service.mattel.com/us/recall.asp to determine if

CPSC, Fisher-Price Announce Recall of Little People® Animal
Sounds Farms
WASHINGTON, D.C. - In cooperation with the U.S. Consumer Product Safety Commission, (CPSC), Fisher-Price, of East
Aurora, N.Y., is voluntarily recalling about 67,000 Little People® Animal Sounds Farms manufactured from June 17, 2002
through July 31, 2002. Two small metal screws that hold the toy "stall doors" in place can come off, posing an aspiration
or choking hazard to young children.
Fisher-Price has received 33 reports of the screws coming off of the toy, including four reports where a screw was found
in the child's mouth and one report of a child that aspirated a screw into his lung. The latter case required the child to be
hospitalized and undergo emergency surgery to remove the screw from his lung.
The recalled Little People® Animal Sounds Farms are shaped like a barn and make animal sounds when the doors of the
cow or horse stall are opened. The recalled toys have a model number of 77973 or 77746 and a six-character
manufacturing date code that begins with 168 through 212, followed by the number 2 as the fourth digit. Both the model
number and date code can be found on the underside of the green ramp. The Fisher-Price logo appears at the top of the
tallest side of the barn in red and white. The recalled toys were manufactured in Mexico.
Mass merchants and toy stores nationwide sold these toys between July 2002 and December 2002 for about $30.

Fisher-Price Recalls "Laugh and Learn" Bunny Toys Due to
Choking Hazard
WASHINGTON, D.C. - The U.S. Consumer Product Safety Commission, in cooperation with the firm named below, today
announced a voluntary recall of the following consumer product. Consumers should stop using recalled products
immediately unless otherwise instructed.
Name of product: "Laugh and Learn" Learning Bunny Toys
Units: About 500,000 (an additional 700,000 were sold worldwide)
Importer: Fisher-Price, of East Aurora, N.Y.
Hazard: The pink pompom nose can detach, posing a choking hazard to young children.
Incidents/Injuries: None reported.
Description: This recall involves the Laugh and Learn Learning Bunny that measures about 10-inches tall. The yellow
bunny with one green and one orange ear has musical and counting sound effects. The words, "Laugh and Learn" are
printed on the bunny's shirt. Product numbers involved in the recall are: K0468, K2960, K2961, K2962, K2963, K2964,
K2965, K3440, K6898, K7884, L0327, and K5862. The product numbers are located on the fabric tag sewn to the body of
the bunny. Only bunnies with three dimensional pompom noses are included in this recall. Bunnies with flat or
embroidered noses are not subject to this recall.
Sold at: Discount department stores and toy stores nationwide May 2006 through December 2006 for about $15.

Remedy: Consumers should immediately take these recalled toys away from children and contact Fisher-Price to arrange
for the return of the bunny to receive a voucher for a replacement toy of the customer's choice.

Recall of Big ActionTM Construction Toys
The Big Action Construction toy is a large plastic construction set that features a rotating
crane that sits on top of a construction site. The crane has a bucket that is attached by a 25inch strap. The bucket can be raised and lowered by using a turning crank on the crane.
Children can wrap the 25-inch strap around their necks, posing a strangulation hazard.
Fisher-Price has received 16 reports of children wrapping the strap around their necks. Seven
children suffered minor injuries.
The recalled Big Action Construction toys are model number 72028. The model number is on
the underside of the toy. The toy has a brown base with a yellow and silver crane and a blue
drawbridge.
"Fisher- Price" is written on the crane. The toy has two construction vehicles and a number of
boulders. Only models with a 25-inch strap attached to the crane are being recalled.
Toy, mass merchandise and discount stores sold the Big Action Construction toys nationwide
from January 1997 through June 2000 for about $60.
Consumers should cut the 25-inch strap off of the toy immediately and throw the strap away.
Fisher-Price will provide a free replacement crane boom and bucket with a 12-inch strap so
that children cannot wrap it around their necks.

Fisher-Price Recalls Infant Musical Toy Chair Posing
Strangulation Hazard

Name of Product: Fisher-Price® Laugh & Learn™ Musical Learning Chair™
Units: About 614,000
Manufacturer: Fisher-Price, of East Aurora, N.Y.
Hazard: A child can become lodged between the seatback and side table of the chair, possibly leading to an entrapment
of the neck. This can pose a strangulation hazard to young children.
Incidents/Injuries: Fisher-Price has received three reports of young children getting their necks lodged between the
seatback and the side table of the toy, including one report of a child receiving a welt on the neck.
Description: The recalled Laugh & Learn Musical Learning Chair is a plastic infant toy that measures about 17 inches
high. It plays music and teaches children numbers and letters. It also features a blue chair with four green plastic legs and
a side table with a purple base and white top. The table holds a clock, book, and a lamp. Smiley faces are displayed on
the lamp and seat back. "Fisher-Price" is written on the front of the chair. The packaging states that the toy is intended for
children 12 months through 36 months of age. The model numbers are H4609, H7167 through H7173, H8157, H8998,
and J0272 through J0275 can be found on the underside of the seat of the chair.
Sold by: Discount department and toy stores nationwide from May 2005 through January 2006 for about $27.

Remedy: Consumers should take the recalled musical chair away from children immediately and contact Fisher-Price to
receive a free repair kit that contains a plastic piece that connects in between the seatback and side table, preventing the
possibility of entrapment.

LeapFrog Recalls to Repair Children's Activity Centers Due to
Arm Entrapment Hazard

Hazard: A child's arm can become caught in the activity center's plastic tube, posing a risk of injury to children.
Incidents/Injuries: LeapFrog has received 145 reports of children's arms becoming caught in the plastic tube, resulting in
54 reports of minor scratches and bruises.
Description and Models: The Learn-Around Playground is a multi-colored, plastic musical activity center for young
children ages 6 to 36 months. It includes a Slide and Sing alphabet panel and a ball drop on the left side, a Number Rock
musical keyboard in the center, and a Flying Rhymes airplane, pop-up LittleLeap, and "smart" book on the right side.
"Leap Frog Baby" is printed on the activity center. Item number 10200 is written underneath the activity table below "Made
in China."

CPSC, Playskool Announce Recall of Magic Start Crawl 'n
Stand Toys Sold Only at Wal-Mart
WASHINGTON, D.C. - In cooperation with the U.S. Consumer Product Safety Commission (CPSC), Playskool, of
Pawtucket, R.I., is voluntarily recalling about 300,000 Magic Start Crawl 'n Stand toys. The toy, which is designed to
encourage crawling babies to pull themselves up and stand, can tip over during use and strike falling babies in the head,
face or neck, resulting in injuries.
Playskool has received 44 reports of the toys tipping over during use. There have been 26 reports of babies suffering
injuries, including one minor concussion, and various bumps, bruises, black eyes and cut lips.
The Playskool® Magic Start Crawl 'n Stand is a multicolor plastic toy with a round base and three arched legs that support
an electronic activity center. When crawling babies touch the frame of the toy, music, lights and spinning balls are
activated. Babies can pull themselves up on the legs of the toy to play at the activity center. The Playskool logo is
imprinted into the plastic base and on top of the activity center. Writing under the activity center includes "ITEM # 06952"
and "Made in China."
Wal-Mart stores nationwide sold these toys from November 2002 through April 2003 for about $14.
Consumers should take this toy away from children immediately and contact Playskool to get information on how to return

Serious Injuries Prompt Recall of Mattel's Polly Pocket
Magnetic Play Sets
WASHINGTON, D.C. - The U.S. Consumer Product Safety Commission, in cooperation with the firm named below, today
announced a voluntary recall of the following consumer product. Consumers should stop using recalled products
immediately unless otherwise instructed.
Name of Products: Polly Pocket dolls and accessories with magnets
Units: About 2.4 million play sets (an additional 2 million play sets were sold worldwide)
Importer: Mattel Inc., of El Segundo, Calif.
Hazard: Tiny magnets inside the dolls and accessories can fall out undetected by parents and caregivers. The magnets
can be swallowed, aspirated by young children or placed by a child in their nose or ears. When more than one magnet is
swallowed, the magnets can attract each other and cause intestinal perforation, infection or blockage, which can be fatal.
Aspiration to the lungs requires immediate surgery. Magnets placed in the nose or ears can cause swelling and be difficult
to remove.
Incidents/Injuries: CPSC is aware of 170 reports of the small magnets coming out of these recalled toys. There were
three reports of serious injuries to children who swallowed more than one magnet. All three suffered intestinal perforations
that required surgery. A 2-year-old child was hospitalized for seven days and a 7-year-old child was hospitalized for 12
days. An 8-year-old child was also hospitalized.
Description: The recalled Polly Pocket play sets contain plastic dolls and accessories that have small magnets. The
magnets measure 1/8 inch in diameter and are imbedded in the hands and feet of some dolls, and in the plastic clothing,
hair pieces and other accessories to help the pieces attach to the doll or to the doll's house. The model number is printed
on the bottom of the largest pieces on some of the play sets. Contact Mattel if you cannot find a model number on your
product to determine if it is part of the recall. Polly Pocket magnetic play sets currently on store shelves are not included in
this recall.

Fisher-Price Recalls Licensed Character Toys Due To Lead
Poisoning Hazard
WASHINGTON, D.C. - The U.S. Consumer Product Safety Commission, in cooperation with the firms named below,
today announced a voluntary recall of the following consumer product. Consumers should stop using recalled products

Name of Product: Sesame Street, Dora the Explorer, and other children's toys
Units: About 967,000
Importer: Fisher-Price Inc., of East Aurora, N.Y.
Hazard: Surface paints on the toys could contain excessive levels of lead. Lead is toxic if ingested by young children and
can cause adverse health effects.
Incidents/Injuries: None reported.
Description: The recalled involves various figures and toys that were manufactured between April 19, 2007 and July 6,
2007 and were sold alone or as part of sets. The model names and product numbers for the recalled toys, which are all
marked with "Fisher-Price," are listed below. The toys may have a date code between 109-7LF and 187-7LF marked on
the product or packaging.
Sold at: Retail stores nationwide from May 2007 through August 2007 for between $5 and $40.

CPSC, Fisher-Price Announce Recall of Basketball Sets
In cooperation with the U.S. Consumer Product Safety Commission (CPSC), Fisher-Price, of East Aurora, N.Y.,
is recalling about 55,000 basketball sets to replace the nets. Some of the nets have enlarged openings. If
children put their heads into these enlarged openings, the net can get tangled around their necks and they could
strangle.
Fisher-Price has not received any reports of injuries. This recall is being conducted to prevent the possibility of
injuries.
The recalled Grow-to-Pro Basketball set has a black pole that adjusts from 3 to 6 feet. A nylon net with an
orange plastic hoop is attached to the pole, along with a gray backboard. The Fisher-Price logo is on the front of
the backboard; and the model number, 72408, along with the words, "Made in U.S.A." are on the other side of
the backboard.
Toy, mass merchandise and discount stores sold these toys nationwide from January 2001 through May 2001
for about $30.
Consumers should stop using the basketball set immediately. Consumers should call Fisher-Price toll-free at
(800) 247-9395 anytime to order a free replacement net. Fisher-Price will help consumers determine if their
basketball nets have enlarged openings.

CPSC, Fisher-Price Announce Recall of Swings and Toys
WASHINGTON, D.C. - In cooperation with the U.S. Consumer Product Safety Commission (CPSC), Fisher-Price, of East
Aurora, N.Y., is voluntarily recalling about 2.5 million Lift 'n Lock Swings, and about 246,000 Get Up & Go Walkers, which
are push toys.
Recall of "Lift & Lock Swings"
The Lift & Lock Swing is an outdoor swing for children 9 months to 3 years. The swing has a plastic "T"-shaped restraint
shield to hold children in the seat. Children can maneuver out of the restraints and fall out of the swings, resulting in
serious injuries.
Fisher-Price has received 110 reports of children falling out of the swings, and 38 children suffered injuries. Eight children
suffered serious injuries, including a fractured skull, leg, arm and two collarbones; two concussions; and stitches to the
forehead. Thirty children received minor injuries including bumps, bruises and scrapes.
The Lift & Lock Swings have red or purple plastic seats, yellow "T"- shaped restraint shields and blue ropes. "FisherPrice(r)" is written on the front of the restraint shield. The swings have model numbers 2092, 75960, 75970, 75973 or
75980 molded into the back part of the seat. Mass merchandise, juvenile products and discount department stores sold
these swings from January 1991 through August 2000 for about $19.
Consumers should stop using the swings immediately. Fisher-Price will provide a free repair kit with a supplemental waist
and crotch restraint belt.
Recall of "Get Up & Go Walkers"
The Get Up & Go Walker is a three-sided push toy, which looks like a car, designed for children to support themselves as they are
learning to stand and walk. Young children leaning forward on the front of the toys can tip them over and fall. Also, the windshield
wipers on some of the toys, which stick out, can injure young children when they fall. Fisher-Price has received 330 reports of
children falling while using or near the product. A total of 132 children received injuries such as bumps, bruises, scratches and cuts.
The Get Up & Go Walker is white with blue sides. It has activities such as a rotating mirror/phone, toy windshield wiper and steering
wheel with yellow horn. The back wheels of the walker can be locked into place to prevent rolling as babies learn to stand. The FisherPrice logo is located on the outside of the toy between the headlight decorations. Models with a front bar for babies to grasp and/or a
green windshield wiper are included in the recall. Fisher-Price will help consumers determine if they have a recalled product.
Toy, mass merchandise and discount stores sold these push-behind toys from July 1997 through August 2000 for about $30.
Consumers should stop children from using the toys immediately and call Fisher-Price for a free repair kit that will eliminate the front
bar and windshield wiper.

Description: The Glowin' Dino Flashlight and the Glowin' Doggy Flashlight are about 9-inches long and make an
electronic sound when switched on. The green dinosaur roars, and the white and brown dog barks. Only flashlights with

LC5J231, and LC5J311. The date codes are found on the bottom of the flashlights. Units that do not contain these codes

CPSC, Tek Nek Toys Announce Recall of Ride-On Toys
WASHINGTON, D.C. - In cooperation with the U.S. Consumer Product Safety Commission (CPSC), Tek Nek Toys
International, L.P., of Grapevine, Texas, is voluntarily recalling about 70,000 ride-on toy trucks for repair. The screw and
nut assembly attaching the steering wheel can come loose, posing a choking and aspiration hazard to young children.
Tek Nek Toys has received six reports of the screw and nut coming loose, including the death of an 18-month-old boy
who aspirated a screw.
The ride-on toys were sold under five model names including Butterfly Girl, Fire Rescue, Mermaid, Police Car and Tonka
Construction Crew. The toys can be used as a ride-on or push walker toy, with a handle on the back of the seat rest.
These toys were sold for children age 1 to 3 years old. On the dash of the ride-on toys are buttons which produce sounds
when activated. The recalled Ride-on vehicles have a date code from 20021127 to 20030319. The date code is located in
the battery compartment located on the top panel next to the steering wheel.
Wal-Mart, Toys R Us, Kmart, Meijer and Shopko sold the ride-on toys nationwide from December 2002 through March
2004 for about $30.
Consumers should take these recalled ride-on toy vehicles away from young children immediately and contact Tek Nek

Name of product: Easy-Bake Ovens

Manufacturer: Easy-Bake, a division of Hasbro, Inc., of Pawtucket, R.I.
Hazard: Young children can insert their hands into the oven's opening and get their hands or fingers caught, posing an
entrapment and burn hazard.
Incidents/Injuries: Easy-Bake has received 29 reports of children getting their hands or fingers caught in the oven's
opening, including five reports of burns.
Description: The Easy-Bake Oven is a purple and pink plastic oven that resembles a kitchen range with four burners on
top and a front-loading oven. "Easy Bake" is printed on the front of the oven. Model number 65805 and "Hasbro" are
stamped into the plastic on the back of the oven. The Easy Bake Oven is an electric toy and is not recommended for
children under eight years of age. Ovens sold before May 2006 are not included in this recall.

Description: This recall involves the Nerf N-Strike Recon CS-6 Blasters for children age 6 and up. The toy blaster is
yellow with a black handle and orange plunger, trigger, and reload clip. The word “NERF” in black lettering is on both
sides of the blaster and the word “ARMED” is indented on the orange plunger. “RECON CS-6” is on the gray cocking
mechanism. There are five interchangeable parts including the shoulder stock, the flip-up sight, barrel extension, quick reload clip and dual-mode light beam. Model number 63552 and UPC codes 653569272021 and 653569311218 can be
found on the packaging. Only blasters with an exposed orange plunger are included in this recall.

Cinderella Battery-Powered Toy Cars Recalled by Dumar

Hazard: The wires under the hood of the car and/or in the battery compartment under the seat can short circuit, posing a
fire and burn hazard to children riding in the car.
Incidents/Injuries: The firm and CPSC are aware of 40 incidents of the toy car’s wires overheating. In several incidents,
smoke was seen coming out from underneath the seat area where the battery is located. In one incident, flames shot from
under the hood while a 4-year-old girl was riding the vehicle. No injuries have been reported.
Description: The electric ride-on toy resembles the Pontiac Solstice. It is light blue and has Cinderella graphics on the
front and sides. The wheels, steering wheel and two seats are pink. The white dashboard may contain an optional radio.
“Pontiac Solstice” is printed on the back of the car. “Walt Disney’s Cinderella Special Edition” is printed on the license
plate. It is designed for children 4 to 7 years old.

Description: This recall involves Rock ‘N Ride plush rocker toys sold in eight models: brown pony, pink pony, pink

rocker bases and were sold for children at least 18 months old and up to 65 lbs. A button on the toy’s ear, hat or dash
activates songs and phrases when pressed. Rockers included in this recall have a date code from July 26, 2007 through
December 29, 2007. The date codes are printed on a sticker inside the battery compartment.

Name of product: Graco Toddler Bed
Units: About 1.2 Million
Manufacturer: Graco Children's Products Inc. of Exton, Pa.
Hazard: A child's arm, leg or foot can become entrapped between the slats in the guard rails or footboard. This
can result in broken bones, sprains and other injuries to young children.
Incidents/Injuries: Graco has received reports of 77 entrapments. This resulted in 13 broken arms and legs, 1
broken foot, a sprained ankle, and 54 other injuries including bruised, scratched, and swollen limbs.
Description: The recalled Graco toddler beds are white plastic and steel with openings between the slats in the
guard rails and footboard. The beds were sold under the names "Cozy Toddler Bed," "Glow-in-the-Dark Toddler
Bed," and "Classic Toddler Bed." There is a label on the guard rail or leg containing one of the following model
numbers: "8801, 8801WR, 8821, 8824, 8828, 8833, 30066, 34434 and 11030," a serial number, and the
manufacturer's address. Model 8828's headboard has a blue sky with a yellow moon and stars. Model 8801WR
has red legs. "Graco" is printed on all of the beds' footboards.
Sold at: Discount, department and juvenile product stores nationwide from February 1994 through March 2001
for between $50 and $70.

Remedy: Consumers should remove the guard rails from the recalled Graco toddler beds immediately and call

About 1 Million Simplicity Cribs Recalled Due To Failures
Resulting in Infant Deaths
NOTE: Repair Kits that immobilize the drop-side are now available
ALSO NOTE: Consumers who have participated or have yet to respond to Simplicity’s June 2007 recall of
Nursery-in-a-Box cribs are strongly urged to also participate in this recall.
WASHINGTON, D.C. - The U.S. Consumer Product Safety Commission (CPSC) is announcing today a voluntary
recall with Simplicity Inc., of Reading, Pa., of about 1 million cribs. The drop-side can detach from the crib, which
can create a dangerous gap and lead to the entrapment and suffocation of infants. CPSC is aware of two deaths
in Simplicity manufactured cribs with older style hardware, including a 9-month-old child and a 6-month-old child,
where the drop-side was installed upside down. CPSC is also aware of seven infant entrapments and 55 incidents
in these cribs.
CPSC is also investigating the death of a 1-year-old child in a Simplicity crib with newer style hardware, in which
the drop-side was installed upside down. CPSC is warning parents and caregivers to check all Simplicity cribs to
make sure the drop-side is installed right side up.
The drop-side failures result from both the hardware and crib design, which allow consumers to unintentionally
install the drop-side upside down. This, in turn, can weaken the hardware and cause the drop-side to detach from
the crib. When the drop-side detaches, it creates a gap in which infants can become entrapped.
CPSC is also aware of two incidents that occurred when the drop-side was correctly installed with older style
hardware, though the upside down installation greatly increases the risk of failure.
The recalled Simplicity crib models include: Aspen 3 in 1, Aspen 4 in 1, Nursery-in-a-Box, Crib N Changer
Combo, Chelsea and Pooh 4 in 1. The recall also involves the following Simplicity cribs that used the Graco logo:
Aspen 3 in 1, Ultra 3 in 1, Ultra 4 in1, Ultra 5 in 1, Whitney and the Trio.
The recalled cribs have one of the following model numbers, which can be found on the envelope attached to the
mattress support and on the label attached to the headboard: 4600, 4605, 4705, 5000, 8000, 8324, 8800, 8740,
8910, 8994, 8050, 8750, 8760, and 8996.
The cribs, which were made in China, were sold in department stores, children’s stores and mass merchandisers
nationwide from January 1998 through May 2007 for between $100 and $300.
As an immediate precaution, consumers should check to see if the drop-side is installed right side up. To do
this,check to see that the slightly rounded rail with the decorative groove is installed at the top and the plain rail is
on the bottom. Next, consumers should make sure the drop-side is securely attached to the tracks in all four
corners.

If the drop-side is installed upside down or not securely attached, consumers should stop using the crib
immediately. Incorrect installation can cause permanent damage to the hardware and re-installing the drop-side
using the same hardware is unsafe.
Consumers should check to see if their crib contains the recalled hardware. Recalled hardware has a flexible tab
at the bottom of the lower tracks and the top of the lower tracks are open. Newer hardware, which is not subject to
the recall, has the flexible tab located at the top of the lower track, and a permanent stop at the bottom.

Hazard: The Amber model crib included in this recall does not comply with crib safety standards due to a gap
between the side rail and the crib mattress support, posing an entrapment hazard to infants. All other model cribs
included in this recall do not have proper assembly instructions and diagrams required for cribs. They are also
missing cautionary and warning labels as required by federal law. If the cribs are not assembled properly they
could pose an entrapment hazard to infants.
Incidents/Injuries: None reported.
Description: The cribs included in this recall are the Daniella, Leeat, Noa, Naomi, Gabriella, Amber and Series
300 model cribs. The cribs are wood and come in a variety of painted and stained finishes including natural,
cherry, blonde, or white wood. Some of the cribs have built-in storage drawers on the side of the crib or
underneath the crib. The crib model name may be printed on the lower part of the headboard of each crib and the
manufacturer name is printed on each set of crib warnings when provided.
Sold at: Juvenile furniture stores in New York and New Jersey from January 2003 through August 2004 for
between $135 and $335.

WASHINGTON, D.C.- In cooperation with the U.S. Consumer Product Safety Commission (CPSC), Simmons
Juvenile Products, of New London, Wis., is voluntarily recalling more than 68,600 cribs for repair. Bracket hooks
that are used to position the height of the mattress can break, causing the mattress to collapse. Babies can
become trapped and suffocate when this happens.
In the past four months, Simmons has received more than 800 reports of bracket hooks breaking. In one case, a
6-month-old hit his chin on the side rail when the mattress dropped.
Simmons is offering a free repair kit, which includes replacement brackets and instructions. The repair can be
made easily in the home.
Most of the recalled Simmons cribs were made in 1998. Those sold at Sears were made in 1998, 1999 and 2000.
Only "98" cribs of all models are recalled, except for Sears cribs. For Sears cribs (which are model
numbers 025260 and 065060), the years "98" "99" and "00" are recalled because the problem bracket
hooks were used all three years. The brackets on all other Simmons cribs except for Sears changed to different
type attachments after 1998. The cribs were sold under the name "Little Folks". Simmons and the two-digit year of
manufacture are written on a label affixed to the crib's headboard. The year is the two-digit number following the
model number. "Little Folks" is on another label affixed to the headboard. The cribs are constructed of maple or
ash, and are painted or stained in more than a dozen different colors, including natural, golden and white.
"Simmons" is written on the top rail. Simmons will help consumers identify if their crib is recalled.
Discount, mass merchandise, juvenile product and department stores, including Sears, sold the cribs nationwide
from January 1998 through December 2000 for between $200 and $600.

Incidents/Injuries: Simmons Kids and CPSC have received one report of a 6-month old baby becoming wedged
between the mattress and crib’s frame. The baby was removed from the crib by the parent without injury.
Description: The recalled mattresses involve open coil crib mattresses manufactured between July 1, 2006 and
March 23, 2008 with a color label attached to the top or side of the mattress that has the following model names:

The crib mattresses also have a law tag that is sewn into the edge of the mattress. The law tag has the date of
manufacture and in most cases will contain one of the following model numbers: H59044.15.0014,
M59082.15.0002, M59027.15.0002 or M59065.15.0006. Pocketed Coils® and Simmons Kids or Simmons
Juvenile Products crib mattresses manufactured before July 1, 2006 or after March 23, 2008 are not included in
the recall.

$150.

Remedy: Consumers should measure the width of their mattress using a reliable measuring device, such as a
yard stick or tape measure, by removing all outer coverings, placing mattress on floor and measuring the width
near the middle, from the outside edge of the tape binding to the opposite side of the mattress. Consumers should
immediately stop using the mattress if it measures less than 27 1/4 inches and contact Simmons Kids to receive a
free replacement mattress.

WASHINGTON, D.C. - The U.S. Consumer Product Safety Commission announces the following recall in
voluntary cooperation with the firm below. Consumers should stop using recalled products immediately unless

Name of Product: Spindle Crib
Units: About 7,600
Distributor: Pottery Barn Kids, of San Francisco, Calif.
Hazard: The spindles on the crib's front rail can loosen and detach from the rail. This can allow the child to fall
from the crib and poses a risk of entrapment.
Incidents/Injuries: Pottery Barn Kids has received six reports of incidents involving spindles detaching from the
crib. No injuries have been reported.
Description: This recall involves the Spindle Crib, model number 4825402. The product name and model number
are printed on a label affixed to the base of the crib. Sold in white, pink and blue, the cribs are made of hardwood
and measure 30.5 x 54.5 x 45" high.
Sold at: Pottery Barn Kids retail stores, the Pottery Barn Kids catalog and PotteryBarnKids.com nationwide from
January 2004 through July 2005 for between $300 and $500.

Infants Strangled to Death in Simplicity Bassinets: CPSC
Urges Consumers To Stop Using Simplicity 3-in-1 and 4in-1 Convertible “Close-Sleeper” Models
WASHINGTON, D.C. – The U.S. Consumer Product Safety Commission (CPSC) is urging parents and caregivers
to stop using convertible “close-sleeper/bedside sleeper” bassinets manufactured by Simplicity Inc., of Reading,
Pa. CPSC has learned that on August 21, 2008, a 5-month-old girl from Shawnee, Kan. was strangled to death
when she became entrapped between the bassinet’s metal bars. This is the second strangulation death CPSC
has learned of in the close-sleeper bassinets. On September 29, 2007, a 4-month-old girl from Noel, Mo. became
entrapped in the metal bars of the bassinet and died.
CPSC is issuing this safety alert because SFCA Inc., the company which purchased all of Simplicity Inc.’s assets
at public auction in April 2008, has refused to cooperate with the government and recall the products. SFCA
maintains that it is not responsible for products previously manufactured by Simplicity Inc.
The Simplicity 3-in-1 and 4-in-1 convertible bassinets contain metal bars spaced farther apart than 2 3/8 inches,
which is the maximum distance allowed under the federal crib safety standard. The metal bars are covered by an
adjustable fabric flap which is attached by velcro. The fabric is folded down when the bassinet is converted into a
bed-side co-sleeping position. If the velcro is not properly re-secured when the flap is adjusted, an infant can slip
through the opening and become entrapped in the metal bars and suffocate. This warning does not cover
bassinets produced in recent months that have fabric permanently attached over the lower bar.
Due to the serious hazard these bassinets pose to babies, CPSC urges all consumers to share this safety
warning with day care centers, consignment stores, family and friends to ensure that no child is placed to sleep in
a Simplicity convertible bassinet covered by this warning.
The Commission voted, using its new authorities in the Consumer Product Safety Improvement Act, to release
this warning upon making a finding that the health and safety of the public require immediate notice.

WASHINGTON, D.C. – The U.S. Consumer Product Safety Commission (CPSC), in cooperation with Delta
Enterprise Corp., of New York, New York., is announcing the voluntary recall to replace missing safety pegs
involving 985,000 drop side cribs. Failure to use or install safety pegs can cause an entrapment and suffocation
risk to infants and toddlers.
When the safety pegs in the base of each leg of the crib are missing from the lower track, the crib locks can
disengage and detach if lowered below the peg hole, creating a hazardous gap. This gap can lead to the
entrapment and suffocation of an infant or toddler.
CPSC staff is aware of a death of an 8-month-old child who became entrapped and suffocated when the drop
side of the crib detached in a reassembled crib where the safety pegs were not installed. CPSC is also aware of
two entrapments and nine disengagement incidents in cribs where the safety pegs were missing.
The recall involves all Delta cribs manufactured in Taiwan or Indonesia, with the “Crib Trigger Lock with Safety
Peg” drop side hardware design. These model numbers and country of origin can be located on the mattress
support board label: 4320, 4340, 4500, 4520, 4530, 4532, 4540, 4542, 4550, 4551, 4580, 4600, 4620, 4624
(production dates 01/06 thru 11/07), 4640, 4660, 4720, 4735, 4742, 4750 (production dates 01/95 thru 12/00),
4760, 4770, 4780, 4790, 4820, 4840, 4850, 4860, 4880, 4890, 4892, 4900, 4910, 4920, 4925-2, 4925-6, 4930,
4940, 4943, 4944, 4947, 4948, 4949, 4950, 4958, 4963, 4968, 4969, 4980.
The recalled cribs have date codes ranging from 1995 through December 2005 and one model (4624) was made
in 2007. The model numbers are located on the top of the mattress support board.
The cribs, which were made in Taiwan and Indonesia, were sold at major retailers, including Walmart, Kmart and
Target.com from January 1995 through September 2007 for about $100. These cribs have also been found to be
sold secondhand. Cribs currently sold at retail are not involved.
CPSC and Delta urge parents and caregivers to immediately stop using cribs that are missing a safety peg on any
leg of the crib and contact Delta to receive a free, easy-to-install repair kit. The kit will include safety pegs in a
bold color and warning labels to be affixed to the mattress board. Consumers with cribs that have all safety pegs
installed may continue to use the cribs; however, CPSC recommends that these consumers contact Delta for the

Parents and caregivers are encouraged to find a safe, alternative sleep environment for their child if their recalled
crib is missing safety pegs.
CPSC would like to remind parents not to use any crib with missing, broken, or loose parts. Make sure to tighten
hardware from time to time to keep the crib sturdy. When using a drop side crib parents should check to make
sure the drop side or any other moving part operates smoothly. Always check all sides and corners of the crib for
disengagement. Any disengagement can create a gap and entrap a child. In addition, do not try to repair any side
of the crib, especially with tape, wire or rope.

WASHINGTON, D.C. - The U.S. Consumer Product Safety Commission (CPSC), in cooperation with Playkids
U.S.A. of Brooklyn, N.Y. is announcing today a voluntary recall of about 2,000 convertible cribs. The sides of the
convertible crib are made of a mesh that expands, creating a gap between the mattress and the side through
which an infant can slip. This poses suffocation and entrapment hazards for young children.
CPSC is aware of the death of a 5-month-old child on August 31, 2008 in Brooklyn, N.Y. The child became
entrapped between the mattress and the stationary side rail of the convertible crib and suffocated.
This recall involves the Playkids U.S.A. convertible crib/playpen/bassinet/bed with model number PLK-909.
“Playkids U.S.A.” can be found on the packaging and on a label sewn into the side of the crib. The model number
can be found on the packaging. The convertible cribs have a drop side rail, a stationary side rail, a canopy
assembly, and a bassinet. The sides of the convertible crib, the mattress support, the bassinet, the canopy and
the bedskirt are covered in fabric and mesh. The fabric and the mesh come in a variety of colors and patterns.
The convertible cribs, which were made in China, were sold in juvenile product retailers in New York from March
2007 through September 2008 for about $100.
Consumers should stop using the recalled convertible cribs immediately and contact Playkids USA to receive a
full refund.

secondary mattress support, used for the lowest position, does not meet the full 26 inch minimum height in its lowest
position, allowing children inside to crawl over the railing, posing a fall hazard.
Incidents/Injuries: None reported.
Description: The recall includes Davenport crib models that begin with the model numbers 273-xxxx-xxxx and
have manufacture dates on or before August 2007 printed on a label located on the right side of the lower inner
panel of the crib. The recalled cribs have two mattress support systems-a metal one and a wooden one. Newer
Davenport cribs with model numbers beginning with 244-xxxx-xxxx that have one mattress support are not
included in the recall.
Sold at: Specialty juvenile product stores nationwide from December 2006 through September 2007 for between
$600 and $800.

Remedy: Consumers should stop using the recalled cribs in the third lowest position, which uses the wooden

at Costco After the Death of a 22-Month-Old Child

Hazard: The lid supports on the toy chests fail to prevent the lid from closing too quickly, posing an entrapment
and strangulation hazard to young children.
Incidents/Injuries: CPSC received one report of a death involving a 22-month-old boy of Roseville, California. He
strangled when the lid of the toy chest fell on the back of his head and entrapped his neck on the edge of the
chest of a LaJolla Boat Bed.
Description: This recall involves two styles of Bayside youth beds: the LaJolla Boat Bed and the Pirates of the
Caribbean Twin Trundle Bed. The preassembled toy chests are designed in the shape of a ship or boat’s “bow”
and attached to the beds as a footboard. The LaJolla Boat Bed toy chest has a hardwood top and white wood
base with a blue stripe. The Pirates Boat Bed toy chest has a hardwood top, wheel shape and brown wood base
with decorative carvings.
Sold at: Costco and furniture retail stores nationwide and Costco.com from January 2006 through May 2008 for
between $700 and $1,400.

Description: This recall involves Portable Rainforest Take Along Swings with a palm tree mobile and two
hanging plush toys. The swings are approximately 23-inches-high and have two carry handles on the left
and right sides. Model numbers K7203, K7192 and K7195 are included in the recall. Model numbers are
located under the right handle on the swing. No other collection of Rainforest swings or products are
included in this recall.

Name of product: Bumble Bee Toys with blue antennae sold with certain Graco high chairs and Graco
mobile entertainers, and also sold separately as an accessory and replacement part.

Hazard: The blue antennae on the Bumble Bee toy can break, posing a choking hazard to young

scratched when the child's mother removed the broken antenna from the child's mouth. `

on Stationary Entertainers
WASHINGTON, D.C. - In cooperation with the U.S. Consumer Product Safety Commission (CPSC),
Graco Children's Products Inc. of Elverson, Pa., is providing a free replacement seat ring for about 63,000
Graco stationary entertainers made prior to March 1996. The white plastic seat rings on the stationary
entertainers have sharp edges that can cut or scratch a child's legs during use.
Graco has received more than 400 reports of children's legs being cut, scratched, or scraped, usually as
the child was lifted from the seat.
Graco will send consumers a repair kit, consisting of a replacement seat ring and installation instructions.
Graco stationary entertainers made prior to March 1996 can be identified by their white plastic seat rings.
The Graco stationary entertainer has a plastic tray, measuring 27 inches across, supported by three
adjustable legs. The chair, which sits in the center of the unit, swivels around so that the child can play
with the toys attached to the tray. The white plastic seat ring is located under the padded seat cover.
The Graco stationary entertainers were sold nationwide in juvenile products and discount stores beginning in
November 1995 for about $59 to $69.

Hazard: These strollers can fail to latch properly and unexpectedly collapse while in use. This can result
in broken bones, cuts, bumps, bruises and other injuries to young children riding in the stroller and

Incidents/Injuries:
For the Duo Tandem strollers, Graco has received reports of 306 collapses causing 230
reported injuries, including a broken arm, and a cut to a child requiring 46 stitches.
For the MetroLite strollers, Graco has received reports of 223 stroller collapses causing 34
reported injuries including 18 bumps and bruises to the head or body. Other injuries associated
with both strollers include cuts, scrapes, scratches, pinched fingers and muscle pulls.
Description:
The recalled Duo Tandem strollers, manufactured between 1994 and 1999, have two seats, one
seat in front and one seat in back. The strollers have a blue, white or green plastic and steel
frame. They have four wheels in the front and two wheels in the rear. The cloth seats and tops
have various colors and patterns. There is a label with the model and serial number on the
stroller's frame. Only strollers with serial numbers and model numbers listed below are included
in this recall.

The recalled MetroLite strollers, manufactured in 2000 and 2001, have a blue, gray or black

Entertainer® Infant Walkers
WASHINGTON, D.C. - In cooperation with the U.S. Consumer Product Safety Commission (CPSC),
Graco Children's Products Inc., of Elverson, Pa., is recalling about 31,000 Tot Wheels® Entertainer®
Activity Center infant walkers for repair. These walkers can collapse unexpectedly during use and injure
infants.
Graco has received 27 reports of these walkers collapsing. Injuries to infants have included a cut toe, a
bump on the head, a sprained foot, and scrapes and bruises to legs and feet.
The recalled Tot Wheels® Entertainer® Activity Center infant walkers, with "bouncing action" and fiveposition height adjustment, have model numbers 4032LN or 4032BLA, located underneath the front of the
walker's tray. The walker has a removable, green play tray with 12 activities, including a steering wheel,
ball spinner and flashing lights. The multicolor walker has an animal-print fabric seat. "Graco Lights &
Sounds" is written on a label on the walker tray. "Tot Wheels® Entertainer® Activity Center with Bounce"
is written on a label on the walker's base.
Mass merchandise, juvenile products and major discount department stores nationwide, including WalMart and Toys R Us, sold the walkers from September 1999 through February 2000 for about $50.

Entertainers
WASHINGTON, D.C. - In cooperation with the U.S. Consumer Product Safety Commission (CPSC),
Graco Children's Products Inc., of Elverson, Pa., is recalling about 19,000 stationary Entertainers™ for
repair. The screw securing the clicker toy to the Entertainer's tray can come out, presenting a choking
hazard to young children.
CPSC and Graco have received eight reports of children rotating the toy until the screw came out, and
either handling the toy's screw or placing it in their mouths. One child started choking on the screw and
sustained scratches in the throat, and two others sustained scratches on the face or stomach.
Graco will send consumers a free repair kit, consisting of a uniquely redesigned replacement screw and
installation instructions.
The Graco stationary Entertainers involved in this recall are model numbers 4118RA, 4118C and 34429,
manufactured between April 24, 1998, and August 6, 1998. The model number and manufacturing date,
which is the first six digits of the serial number, are located on a label underneath the Entertainer's tray.
The plastic tray is supported by three adjustable legs. A chair, which sits in the center of the unit, swivels
around so that the child can play with the eight toys attached to the tray. The clicker toy is made of yellow
plastic and has three rounded knobs. "Graco" appears on a yellow label, affixed to the front of the tray.

Safety Instructions to Prevent Injuries with Portable
Play Yards with Raised Changing Tables
WASHINGTON, D.C. - In cooperation with the U.S. Consumer Product Safety Commission, Graco
Children's Products Inc., of Exton, Pa., is providing new instructions for about 538,000 "Pack ‘n Play"
portable play yards with raised changing tables. When children are placed in these portable play yards
when the changing table is still in place, they can crawl under and lift the table up. If this occurs, a child's
head and neck can become trapped between the changing table and the play yard rail, causing a
strangulation hazard. The recall is designed to warn consumers of this hazard, and to provide warning
labels to affix to the changing tables.
The commission and Graco are aware of the death of a 13-month-old girl, who strangled when her neck
became trapped between the Pack ‘n Play rail and the raised changing table.
Only "Pack ‘n Play" portable play yards with raised changing tables are part of this recall. They also are
sold with an infant bassinet. "GRACO" and "Pack ‘n Play" are written on the side of the play yard. These
play yards have model numbers 2016, 35235, or model numbers that begin with 9531 or 9533. The last
three digits of the model number will vary. The model number can be found on a label on the white plastic
center cone under the play yards. "MADE IN CHINA" also is written on the bottom label. Models 2016,
35235, 9531 and 9533 that are currently sold at retail and that have green stickers on the boxes,
indicating that the unit has been modified, are not recalled.
Discount, department and juvenile product stores nationwide sold these portable play yards from October
2001 through September 2003 for between $99 and $119.
Consumers should never place a child in these portable play yards when the changing table is still in
place. Consumers should immediately contact Graco to receive a free warning label to be affixed to the

Incidents/Injuries: Graco has received 137 reports of infants mouthing and chewing pieces of the plastic
film covering, some of whom required medical attention. Serious incident reports include 32 infants who
gagged and 49 who choked on the plastic covering.
Description: The soft block tower is a stack of three different shaped stuffed fabric blocks in red, yellow
and blue. Pictures of apples, fire engines, ducks, bananas, birds and blueberries on the blocks are
covered in plastic film. The soft blocks are one of nine toys that snap onto the tray of the activity center.
Only model number 4635BEE with serial numbers 012705 through 063005, which indicates the
manufacture dates of January 27, 2005 through June 30, 2005, are included in this recall. Model and
serial numbers are located underneath the activity center's tray.
Sold at: Army & Air Force Exchange Service (AAFES), Babies "R" Us, Toys "R" Us, Meijer, Shopko,
Target, and various specialty retailers nationwide, and on Wal-Mart.com and Target.com from February
2005 through about December 2005 for $80.

Activity Centers
WASHINGTON, D.C. - In cooperation with the U.S. Consumer Product Safety Commission (CPSC),
Graco Children's Products, Inc., of Elverson, Pa., is voluntarily recalling about 152,000 toy tracks
attached to children's activity centers. The toy track can break, presenting a cut or pinch hazard and
exposed small parts pose a choking hazard to young children.
Graco has received 11 reports of the toy tracks breaking. Four children received minor scratches and one
child's finger was pinched.
The recalled activity centers include the Tot Wheels V, models 4511 and 4521, and the Convertible
Entertainer, models 4652 and 35225. The model numbers and the words, "made in U.S.A." are printed on
a label on the underside of the tray on the activity centers. The green wavy toy track sits on the tray of the
activity center. Five objects spin and slide along the toy track.
Discount, department and juvenile product stores sold the activity centers nationwide from November
2001 through May 2002 for between $30 and $70.

CPSC, Century Announce Recall to Repair Model 10710 and 10-810 Playards
WASHINGTON, D.C. - In cooperation with the U.S. Consumer Product Safety Commission (CPSC),
Century Products Company of Macedonia, Ohio, is voluntarily providing a free repair kit for about 212,000
Century Fold-N-Go Model 10-710 and 10-810 Travel Playards (also known as playpens). The top rails on
these playards may collapse unexpectedly, trapping a child at the neck in the "V" formed by the collapsed
top rail.
An 18-month-old boy died recently in Centreville, Va., when a Century playard collapsed. The Fairfax
County, Va., Police Department provided information to CPSC regarding the death. Century is offering a
free repair kit, consisting of new Posi-Locks top rails to prevent the rails from collapsing when a child is in
the playard. This kit can be installed in the home using a Phillips head screwdriver.
The Century Model 10-710, measuring 26 inches by 38 inches, was sold in Toys R Us stores beginning in
June 1994 for about $50. The Model 10-810, measuring 28 inches by 41 inches, was sold nationwide
beginning in February 1993 in juvenile products and discount stores for about $80. Both models fold
compactly and fit into an accompanying nylon case for storing and carrying. The model number is located
on a label on the floor or on one of the support tubes under the floor. All Century Fold-N-Go Models 10710 and 10-810 are part of this recall. Other Century Fold-N-Go models have different top rails and are
not being recalled.
Consumers with either a Century Model 10-710 or Model 10-810 Fold-N-Go Playard should stop using
the product immediately and call Graco, which now owns the "Century" brand name, at 800-345-4109 for
a free repair kit and installation instructions. Consumers should not use the playard until the new top rails
are installed.

Name of product: Travel Lite Swing
Hazard: The swing's carrying handle can fail to stay in place properly and drop or be pushed down,
hitting a child in the head. Additionally, the 3-point seatbelt can fail to prevent a child from leaning forward
or to either side, posing a risk that the child can fall forward and strike his/her head on the floor or the
swing's frame.
Incidents/Injuries: Graco has received about 28 reports of incidents involving the handle falling down on
young children; in addition, Graco has received 100 reports of children falling forward or to the side.
Injuries resulting from these incidents include bloody or swollen lips, red marks, bumps and bruises.
Description: The recalled Travel Lite portable swings have an adjustable reclining seat, a rotating handle
and a canopy and include model numbers 1850JJP, 1850JGB, and 1870DAL. The swings, which were
manufactured between May 2003 and December 11, 2003, also have a serial number between 050503
and 121103. Both the model and serial numbers can be found on a white label underneath the seat. The
swings have the words, "Graco" and Travel Lite swing" printed on each side, and have buttons on the
handle to activate lights and music. On the underside of the handle are multicolored designs of the sun,
moon, and stars that light up when the light button is pressed.
Sold at: Discount, department and juvenile stores from June 2003 through June 2004 for about $60.
Manufactured in: United States.
Remedy: Consumers who have a Travel Lite swing with a 3-point seatbelt (waist belt and crotch strap
only) or a Travel Lite swing without a red handle release button should stop using it immediately and
contact Graco for a free repair kit.

CPSC, Fisher-Price Announces Recall for In-Home
Repair of Infant Swings
WASHINGTON, D.C. - In cooperation with the U.S. Consumer Product Safety Commission, (CPSC),
Fisher-Price, of East Aurora, N.Y., is recalling about 42,000 Smart Response Swings for in-home
inspection and repair. It is possible to misassemble the seats of these swings so that they appear secure,
but are not. If the seat of the swing is not properly attached, the seat and baby can flip forward.
Fisher-Price has received seven reports of the seats of the swings flipping forward, including four reports
of babies hitting their heads on the floor. Though bumps and red marks were reported, there have been
no serious injuries.
The Smart Response Swing is an indoor infant swing for use from birth until baby can sit up unassisted.
The swing operates in response to a sound sensor and plays music. The metal legs of the swing are
either blue or beige, and the seat is either beige or white. The Fisher-Price logo appears on the seat's
tray. Product numbers 79644, 79645 or 79647 are molded onto the back of the seat.
Discount department and juvenile product stores nationwide sold these swings from December 2001
through March 2002 for about $70.
Consumers should stop using swings immediately and call Fisher-Price for help in inspecting the seat to
make sure the seat is properly attached. If the seat is not properly attached, a free replacement seat and

Toy Attached to Stationary Entertainer
WASHINGTON, D.C. - In cooperation with the U.S. Consumer Product Safety Commission (CPSC), Little
Tikes Co., of Hudson, Ohio, is voluntarily recalling about 260 lobster toys attached to the activity tray of
the Ocean Friends Stationary Entertainer. The antennae on the lobster toy can break, posing a choking
hazard to young children.
No injuries or incidents have been reported. This recall is being conducted to prevent the possibility of
injury.
The recall involves Ocean Friends Stationary Entertainer model 4629 GIG. The model number is located
underneath the entertainer's tray. The green tray on the stationary entertainer is supported by three
adjustable legs and has the words "Little Tikes" printed across the front. The seat, patterned with colorful
pictures of sea animals, sits in the center of the unit and swivels so the child can play with the nine toys
attached to the tray. The recalled lobster toy is red with black antennae. Only lobsters with black
antennae are involved in this recall.
Toys R Us stores sold the stationary entertainer with the lobster toy nationwide between October 2001
through November 2001 for about $60
Consumers should remove the lobster toy from the entertainer immediately and contact Little Tikes for a

Hazard: The walkers violate the baby walker voluntary standard and can fit through a standard doorway
and are not designed to stop at the edge of a step. Babies using these walkers can be seriously injured or
killed.

Description: The recalled walkers were sold for babies 6 months and older. The walkers were sold in the
following colors: pink, red, green, blue, and ivory. The walkers have an activity tray with a steering wheel
and other toys. “My Way Corp” is printed on a sticker on the front of the walker.
Sold at: Independent discount stores in Puerto Rico from November 2004 through March 2008 for
between $18 and $25.

Description: This recall involves the Taggies™ Strollin’ Along stroller activity bars. The multicolored,
fabric, and plastic activity bars feature a yellow giraffe, a purple hippo and a blue elephant attached to a
12” long elliptical base with straps that attach to a stroller. The Taggies™ and Earlyears® logos are
located on woven and satin labels sewn into the seam on the lower left front of the activity bar.

Description: The recalled single strollers all have metal frames with three wheels, a cloth seat and
canopy. The twin strollers have a metal frame with four wheels, side-by-side cloth seats and double sun
canopies. The strollers were sold in a variety of colors including red, orange, green, black, charcoal and
navy. The Phil & Teds logo is located on the crotch piece of the harness.

Ellaroo Recalls Infant Sling Carriers Due to Fall Hazard

Hazard: The aluminum rings on the sling carriers can bend or break. This can cause the fabric to slip through the rings
and infants to fall out of the carrier.
Incidents/Injuries: Ellaroo has received four reports of the rings bending and two reports of rings breaking. No injuries
have been reported.
Description: This recall includes Ellaroo Ring Sling baby carriers with item numbers 2101 and 2102 printed on the
outside of the product box. The Ellaroo Ring Sling has a fabric carrier with two aluminum rings that is worn by the user to
carry an infant up to 35 pounds. The carriers are sold in mahogany, mango stone, brasilia and malay color prints. Only
sling carriers with lot numbers 03/07 and 07/04 printed on a label, under the size label, inside the Ring Sling are included
in the recall.
