Membership
The EPHS – NHS Faculty Council reviews and selects all NHS members. The selection for membership is
based on the Four Pillars of National Honor Society: scholarship, leadership, character and service. Once
selected, members have the responsibility to continue to demonstrate these qualities. Members who resign or
dismissed are no longer eligible for membership or its benefits nor may they reapply for membership.
Eligibility
• The candidate must be a current sophomore or junior student at Eden Prairie High School.
• The candidate must have a cumulative GPA of 3.70 or above as of March 11, 2013

We have read, understand, and support the EPHS National Honor Society (NHS) candidacy materials and
process. By submitting this information, we verify that the candidate has NOT had any MSHSL violations,
academic integrity violations (which includes cheating), or any chemical and or alcohol violations. We also
acknowledge that if inducted, the NHS member must continue to adhere to the standards which were the basis
for their selection. A flagrant violation of school rules or any violation of civil laws will result in the member’s
dismissal from NHS.

Instructions for completing the EPHS NHS Application
**Do not ask Student Activities staff to verify any information on your application unless they are a coach and/or advisor –in
that case, see the next instruction
**Do not ask teachers or coaches for verifications during school hours: before school, zero hour, or after school only. Those
that are reported by teachers and/or staff as having interrupted his/her work day will not be eligible/considered for
membership
1.
Compile all aspects of application in a manila file folder with your name clearly printed on tab (Last Name, First Name)
followed by your current Grade level (10 or 11). Folders are not provided by the school.
2.
The “Desire Statement” should be word-processed.
a. Your response should be truthful and specific.
b. Responses should not exceed 150 words
c. Sign the bottom of your “Desire Statement”
3.
The “Character Essay” should be word-processed.
a. Your response should include information about only you as well as specific example(s). In addition, all aspects of
the prompt should be addressed to in your completed essay.
b. Responses to the “Character Essay” should not exceed 200 words; those responses that do exceed the word limit,
may not be eligible/considered for membership.
c. Put your name, current grade level, and word count at the top of your essay as your heading.

b. If there are any discrepancies on your transcript, please explain those on a separate sheet of paper (clearly marked)

Leadership Qualifications:
a. No more than your five most important leadership positions should be listed
b. You may attach a separate sheet(s), but be sure to clearly mark the sheet(s) on both the list and additional paper(s)
6.
Activities:
a. Key Club members DO NOT need a signature from advisor as a roster of members who have documented service
has been shared with NHS Faculty Council
b. No more than your five most important activities should be listed
7.
Service Experience:
a. Do not include any experiences where you were paid or earned some type of credit for school
b. Key Club experiences are acceptable; however, any experiences you document should NOT be signed by the Key
Club Advisor
c. Copies of confirmation forms are acceptable (the half sheets and/or on-going service logs filled out in the Outreach
room); general hour print outs from the Outreach Room (that have no specifics) are NOT acceptable
d. Mission trips may not count more than 3 hours per day and/or exceed 20 hours total, whichever is greater
e. Diversity of service is more important than a single experience completed regularly.
f. Eagle Scout projects may not exceed more than 20 hours of experience

Application Final Checklist
All materials should be completed and filed in a manila file folder with your
last name, first name clearly written on the tab.
**If any aspect of the NHS application is incomplete/incorrect/false/late or not
filled out by applicant, submission will not be accepted and/or considered
_____ Application submitted to the NHS advisor in the English Resource Center (ERC), by Friday, April
12, 2013. No late applications will be accepted!
_____ Attach your essay which clearly answers the Character question.
_____ Attach your current high school transcript with Advanced, Enriched, and/or AP courses
highlighted
_____ Provide signatures for each service, leadership and activity which you have listed.
You may attach the supervisor’s email verification if you are unable to obtain a signature. No
parent signatures are accepted. All coaches, advisors or teachers’ signatures must be obtained by
April 10, 2013—two days before application is due.
_____ Make certain you and your parent/guardian have signed the first page of the application.
These signatures verify you have not had any Minnesota State High School League violations and
that you have not had any academic integrity (cheating), chemical or alcohol violations during the
current school year.
_____ All information is compiled in order in a manila file folder correctly labeled as outlined; staple
your response to the first essay question to the inside front cover of your application folder.
**No applications will be accepted after the due date of 2 p.m. on April 12, 2013
**All applicants will be notified of the status of his/her application by the end of the school year.
