201 6 College Scholarship Application
for High School Seniors
Scholarship Amount(s):
Up to four (4) scholarships of $1,000.00 each, payable in 2 installments ($500.00 for
the Fall session and $500.00 for the Spring session) upon receiving of proof of
enrollment for each semester.
Eligibility:
1. Because the local Oswego community largely supports this award, the recipients
must be High School Seniors.
2. The recipients must be current or former members of the Oswego Minor Hockey
Association.
3. The recipients must be attending a four-year, two-year, or vocational school as a
full-time student (12 credit hours or more) during the academic year immediately
following their high school graduation.
Application Guidelines:
1. The completed official OMHA Scholarship Application form, and all required
attachments, must be postmarked or hand-delivered to the Chair of the OMHA
Scholarship Committee by the due date – February 27, 2016.
2. Applications must be typed or neatly printed.
3. Applicants must provide three (3) recommendations from people who are not
relatives - using the forms provided for this purpose.
4. Applicants must provide their most recent official high school transcripts with the
application, in the original sealed envelope.
