ESSAY
A)SCHOLARSHIP ESSAY
Type a two page, double-spaced essay explaining:
What important lessons have you learned this past school year that you will implement for the
upcoming school year to be a better student and a more integral part of your campus community?
B) SUPPLEMENTAL ESSAY FOR STUDENTS CUMULATIVE GPA OF LESS THAN 2.0 GPA
If your cumulative grade point average is below a 2.0, you may still receive funding. However, you
must attach a typed half-page plan on what resources and techniques you will employ next year to raise your
grades (i.e. time management, tutorial services, self-discipline, increased effort, etc.) This plan is in addition to
your two page scholarship essay. If you are already on probation from last year, you must call Rachel Cleaver at
202.289.6478 to determine eligibility to reapply.

As a potential recipient of an HDSF scholarship, I understand that to be considered for funding I must do the following:

Remain off academic probation in my future studies, or if placed on academic probation, take
the necessary steps to raise my grades and provide a signed affidavit from a tutor or advisor
outlining plan to improve.

Complete and submit Federal FAFSA form, DC ONE APP (DC TAG/DC LEAP), and provide
necessary forms to DC CAP.
In order to allow the HDSF staff to decide a necessary course of action in assisting in my progress at the school to which my
funding will be applied, I hereby give my permission to the Hoop Dreams Scholarship Fund to have access to
