PUBLICATION QUESTIONNAIRE
Submission Guidelines
The questions and requests that follow pertain to you and your co-author(s) or co-editor(s), your
book, and the intended audience for your book. The information you provide will help us reach a
publishing decision about your work. If appropriate, the information will later be used to help us
develop effective marketing strategies designed specifically for your book. Also, your answers
will comprise the data required for the copyright and Library of Congress registration. Please be
as thorough as possible; attach additional sheets if necessary.
We notify all authors upon receipt of the proposal and once a decision is made, so there is
no need to follow up. If you would like us to return any materials to you, please include a selfaddressed envelope with correct postage.
When submitting your proposal, please include:
 This completed publication questionnaire
 A curriculum vitae or résumé for each author
 A table of contents with chapter descriptions
 If applicable, a list of contributing authors
 One or two sample chapters (if available)
 A list of references that will be consulted (if applicable)
 A self-addressed, stamped envelope if you would like your materials returned
