CONFIDENTIALITY ACKNOWLEDGMENT
Creighton University Institutional Animal Care and Use Committee
I acknowledge as a member of the Creighton University Institutional Animal Care and
Use Committee (IACUC) or a person assisting, and/or consulted by the IACUC, that I
will have access to and/or knowledge of Confidential Information created by, used by or
disclosed to, the IACUC in performing its institutional functions related to all research
activity involving animals.
Confidential Information, includes, but is not limited to, the research protocol, sponsor
documents, adverse event reports, IACUC minutes, correspondence, internal or external
audit reports, and any other information created by, used by or disclosed to, the IACUC
in its oversight of research activity involving animals. Confidential Information does not
include information published by the IACUC (e.g., IACUC forms, policies/procedures)
for use by institutional Investigators.
I agree that:

I will not disclose any Confidential Information that I become aware of to anyone
other than IACUC members or persons assisting and/or consulted by the IACUC,
or as otherwise required by law.

I will not use any Confidential Information for my own purposes or for personal
gain.

Any copies made of Confidential Information will be solely for use as an IACUC
member or person assisting and/or consulted by the IACUC and will either be
destroyed or returned to the IACUC when no longer needed.
I understand and agree that any violation of this Agreement can be grounds for
disciplinary action, including but not limited to my removal from participation in IACUC
activities.
This Acknowledgment will not be construed as a contract of employment.
